Prioritize vulnerabilities by combining evidence that they are being—or are likely to be—exploited with the consequences of a successful attack on the affected asset. Use CVSS to understand technical characteristics, EPSS to estimate exploitation likelihood, CISA’s Known Exploited Vulnerabilities (KEV) Catalog to identify known in-the-wild exploitation, and local asset context or CISA SSVC to decide what action to take. None of those signals alone captures the whole risk to your organization.

Exploitability and impact answer different questions

Exploitability concerns how feasible it is for an attacker to exploit a vulnerability and whether exploitation is occurring or likely. Impact concerns what successful exploitation could do: for example, affect a system, expose information, or disrupt a service. The practical priority also depends on where the vulnerable system sits in your environment and what your organization relies on it to do.

CVSS v4.0 separates technical exploitability and impact characteristics. Its Consumer implementation guidance also recommends considering Threat and Environmental metrics to make assessments more relevant to real-world conditions. A base score is useful for describing technical severity, but it does not by itself encode the consequences for a particular organization, service, or asset.

Use the signals together, not as interchangeable scores. A high technical severity does not automatically mean an exposed, business-critical system is at greater immediate risk than every other finding; confirmed exploitation or a high-consequence asset can change the order. Conversely, a low exploitation-likelihood estimate does not make severe local consequences irrelevant.

What each prioritization signal tells you

Signal or method What it contributes Best use Important limit
CVSS v4.0 Standardized technical exploitability and impact characteristics, with Threat and Environmental metrics available for consumer context. Understand and compare technical properties, then add organizational context. A base score does not capture the full business or mission consequences for a specific asset.
EPSS A probability-oriented estimate of exploitation activity. Help distinguish vulnerabilities more likely to be exploited, particularly when known-exploitation evidence is not available. It estimates likelihood, not impact. A low EPSS score does not negate confirmed KEV evidence, and EPSS can differ from a particular observed case.
CISA KEV Catalog Evidence that a vulnerability is known to have been exploited in the wild, along with catalog remediation direction. Raise known-exploitation findings in the queue and check the specified remediation guidance. CISA describes KEV as an input to prioritization, not a complete risk rating. NIST’s 2025 research notes that KEV lists may not be comprehensive.
CISA SSVC A stakeholder-specific decision tree with Track, Track*, Attend, and Act outcomes. Turn exploitation and impact context into a response decision. Use the tree for the relevant stakeholder and supply asset-specific information; a generic outcome cannot replace local context.

When comparing any two findings, ask what each signal measures, whether exploitation is observed or predicted, what technical impact is possible, how the affected asset is used locally, and whether the signal leads to a concrete action. FIRST’s EPSS guidance gives an approximate effort-level comparison: the 90th percentile is at least 0.04, or 4% probability of exploitation. This is FIRST’s example for comparing effort, not a universal risk threshold or remediation deadline.

A practical workflow for setting vulnerability priority

  1. Confirm the finding and identify the affected asset

    Check the product and version, whether the deployed system is actually vulnerable, where it is used, and whether it is internet-facing or otherwise reachable. Connect the finding to a maintained asset inventory and the business-critical functions that depend on the system; CISA’s mitigation guidance emphasizes that connection.

  2. Check for evidence of exploitation

    Look for the vulnerability in CISA’s KEV Catalog and review credible, current threat intelligence. Treat a KEV listing as a high-priority exploitation signal, then read the catalog entry and vendor instructions for the specific remediation. CISA says organizations should use KEV as an input to their vulnerability-management prioritization framework. FIRST advises treating KEV inclusion as active exploitation evidence regardless of EPSS. Absence from KEV, however, is not proof that exploitation has not occurred.

  3. Estimate likelihood when exploitation is not confirmed

    Use a current EPSS score as one input to your threat assessment. It can help distinguish likely-to-be-exploited findings outside known-exploitation catalogs, but it says nothing by itself about the damage an exploit could cause. Do not turn a score or percentile into a universal deadline unless your organization’s policy defines that threshold and its consequences.

  4. Assess technical impact and local consequences

    Review the CVSS exploitability and impact details, then assess the affected asset in context. Consider its external exposure and reachability, how widely the system is deployed, and whether compromise could affect critical services, sensitive information, safety, or mission delivery. Account for controls and mitigations already in place. FIRST’s Consumer guidance recommends Threat and Environmental context when using CVSS for real-world prioritization.

  5. Choose and document a response

    Use a documented decision approach, such as SSVC, to decide whether to Track, Track*, Attend, or Act in the applicable stakeholder context. For findings requiring action, CISA’s guidance describes remediation, mitigation, and acceptance as possible treatments. Choose the feasible treatment that addresses the assessed risk, and document the rationale and any conditions attached to an acceptance.

  6. Assign the work and verify the result

    Give each remediation or mitigation a responsible owner and a due date under your organization’s policy. Acquire and deploy the patch or mitigation, then validate that it took effect—for example, by checking the affected version or rescanning as appropriate. NIST SP 800-40 Rev. 4 defines enterprise patch management as identifying, prioritizing, acquiring, installing, and verifying patches, updates, and upgrades; verification is part of the process, not an optional final step.

  7. Reassess when the evidence changes

    Exploitation intelligence, asset exposure, vendor fixes, and catalog entries can change. Refresh relevant signals and reprioritize when those changes alter the risk picture. For a live decision, check the current KEV entry, EPSS score, affected versions, vendor guidance, and your organization’s exposure on the day you act.

How to handle conflicting signals

Do not average conflicting indicators into a single arithmetic score unless your organization has validated and documented a specific method for doing so. The available guidance supports combining evidence and local context, not treating a formula such as CVSS multiplied by EPSS as a universal or validated risk score.

  • KEV lists the vulnerability, but EPSS is low: Treat the KEV listing as evidence of known exploitation; the low EPSS value does not cancel it. Confirm affected versions and follow the catalog and vendor remediation direction.
  • EPSS is high, but the vulnerability is not in KEV: Treat the estimate as a likelihood signal, not proof of exploitation. Weigh it alongside technical impact, reachability, asset importance, and available controls.
  • CVSS is high, but the asset is isolated or has strong compensating controls: Record those conditions as environmental context, verify they are real and maintained, and make a documented decision rather than assuming the base score alone settles the order.
  • The technical score appears modest, but the asset supports a critical service or sensitive data: Assess the possible local consequences directly. Technical severity and organizational consequence are related inputs, not substitutes for one another.

Set response timing through policy, not a universal score cutoff

The reviewed guidance establishes a prioritization and patch-management process, not one patch deadline that applies to every organization or vulnerability. Set response times through your organization’s policy and account for applicable jurisdictional, contractual, and advisory requirements. Where a deadline is mandated by a current directive or advisory, follow that requirement rather than substituting a general score threshold.
