Overnight irritation from an incontinence pad or pull-up often comes from skin staying damp or in contact with urine or stool, combined with friction. After leakage, gently cleanse the area, pat it dry—including skin folds—and consider a skin barrier product that is compatible with the continence product. Seek medical advice if skin is damaged, symptoms are severe or worsening, or the irritation does not improve with consistent care.
Why overnight incontinence products can irritate skin
Urine, stool, excess moisture, and friction can weaken or irritate skin, especially when exposure lasts for a long time. A pad or pull-up is designed to draw urine away from its surface, but it does not remove the need to clean the skin after leakage. MedlinePlus puts it plainly: “Even if these products are used, the skin must still be cleaned each time after passing urine or stool.” MedlinePlus guidance on skin care and incontinence
Skin irritation in this area can have different causes and may look similar across conditions. A rash’s appearance alone is not enough to diagnose it or determine the right treatment.
A gentle routine after leakage
- Clean the skin promptly. Use a gentle cleanser that does not dry or irritate the skin. MedlinePlus describes mild, diluted soap and water as well as soap-free cleansers; an NHS patient leaflet recommends pH-balanced cleansing. Follow the cleanser’s directions and avoid products that sting or worsen irritation. MedlinePlus: Skin care and incontinence; Kent Community Health NHS Foundation Trust guidance
- Pat dry rather than rub. Use a soft towel or cloth and take care to dry skin folds. Rubbing can add friction to already sore skin. NHS Ayrshire and Arran guidance
- Use a barrier product only as directed. If you use a barrier cream or sealant, apply it to clean, dry skin and follow the label or a clinician’s directions. MedlinePlus lists zinc oxide, lanolin, and petrolatum among barrier ingredients and advises reapplying cream or ointment after cleaning and drying. Formulations differ, so check compatibility with the pad or pull-up. MedlinePlus: Skin care and incontinence
- Change promptly after stool leakage. Faecal contamination can prolong skin contact with irritants; Leicester NHS advises changing promptly after faecal incontinence. University Hospitals of Leicester NHS Trust guidance
Do not make talcum or baby powder a routine fix. Kent NHS warns talcum powder may reduce pad absorbency; MedlinePlus also warns against baby powder in the context of a suspected yeast rash. Kent Community Health NHS Foundation Trust guidance; MedlinePlus: Skin care and incontinence

Check product fit and barrier compatibility
If the product leaks or the skin is often wet, ask a clinician, pharmacist, or continence service to review fit and suitability. Pads and pull-up pants use absorbent layers intended to draw urine away from the product surface, but individual needs vary; there is no single absorbency level that is right for everyone. NHS: Products to help with urinary incontinence
- Check the fit. Leicester NHS notes that size and fit matter. A product that does not fit the wearer well may not manage leakage as intended. University Hospitals of Leicester NHS Trust guidance
- Do not double-pad. Leicester NHS advises against placing one pad inside another because this can contribute to leakage and prolonged skin contact. University Hospitals of Leicester NHS Trust guidance
- Check cream directions against the pad. Some thick or oil-based formulations may reduce absorbency or clog pads. Ask a clinician or pharmacist if you are unsure which barrier product suits the person’s continence product. Kent Community Health NHS Foundation Trust guidance; University Hospitals of Leicester NHS Trust guidance
- Use a bed pad for bedding protection, not as rash treatment. NHS guidance describes washable bed pads as an option for overnight leaks; they do not replace skin care or treat irritation. NHS: Products to help with urinary incontinence
When to get medical advice
Contact a healthcare professional if the skin appears damaged or a pressure sore is suspected, if irritation is severe or worsening, or if it persists despite consistent cleansing and drying. A clinician can assess the cause and recommend an appropriate care plan; local protocols and available formulations can differ. Kent Community Health NHS Foundation Trust guidance; University Hospitals of Leicester NHS Trust guidance
