Ask the prescriber or pharmacist to resolve anything unclear—especially missed doses, food instructions, or whether a tablet can be split or crushed. Do not change a medicine or its form without their guidance.
Start by identifying what makes the instructions difficult
Ask the older adult which part is hardest: remembering dose times, reading small print, opening a container, swallowing a tablet, or understanding terms such as “as needed.” The solution should address the specific obstacle rather than assume that a reminder or organizer will work for everyone.
FDA’s questions for older adults about medicines can help structure a conversation with a pharmacist or prescriber.
- Is the schedule hard to remember or too complicated?
- Is the print difficult to read, or are the directions unclear?
- Is the container hard to open, or is taking the medicine physically difficult?
- Are directions such as “as needed” or instructions about food confusing?
Get a complete written schedule
Ask the prescriber or pharmacist for a current schedule that explains how and when to take every medicine. It should be specific enough that the person does not have to guess. Include the medicine’s purpose, exact dose, timing, duration, food or drink directions, what to do after a missed dose, side effects to monitor, and whom to contact with questions.

FDA recommends asking, “What is the proper dose?”, “How and when do I take the medicine and for how long?”, and “What do I do if I miss or forget a dose?” Ask the professional to write down the answers, particularly when a direction is conditional or easy to confuse.
Read the schedule together. Then invite the older adult to explain in their own words how they will take the medicine. This makes unclear wording easier to catch before it causes a mistake. If the schedule feels difficult to manage, ask whether it can be simplified; only the prescriber should decide whether to change the regimen.

Make each direction easier to understand
Use straightforward words and emphasize the action the person needs to take. Repeat essential directions verbally and in writing, while keeping every important detail: dose, timing, duration, food or drink instructions, missed-dose advice, monitoring, and contact information. Simpler wording should clarify instructions, not remove them.
Older adults may experience changes in memory, attention, processing speed, vision, or hearing that make health information harder to take in. The CDC recommends plain language, repetition, and emphasis on key actions when communicating with older adults. Ask the pharmacy or care team whether written information is available in large print or another language. Availability varies.

Use reminders that fit the person’s routine
A reminder is useful only if the person can read, understand, and reliably use it. Link doses to an established daily routine when that fits the prescribed directions. A written check-off chart or pill organizer may help, but neither is appropriate for every person or every medicine.
Choose an organizer with a pharmacist
Organizers vary in the number of days they hold and the number of compartments per day. Match those features to the actual schedule, then consider whether the person can open, see, and fill the organizer. Ask a pharmacist which medicines, if any, can safely be placed in it. MedlinePlus advises getting a pharmacist’s help when choosing an organizer.

If opening the original container is difficult, ask the pharmacist whether an easier-to-open container is available. Consider child-safety needs in the home before using packaging that is easier to open. Do not move medicines into an organizer or other container without checking whether that is suitable.

Maintain a single up-to-date record that is easy for the person to read, update, and share. FDA recommends including prescriptions, over-the-counter medicines, vitamins, and supplements—not just prescription drugs. Record each product’s strength, purpose, and directions.

Update the list whenever a medicine or dose changes, and make it available to the patient, relevant caregivers, and health professionals. An accessible paper list or digital record can work; choose the format the person can maintain and share. FDA provides guidance on creating and keeping a medication list.
Revisit directions after a change
When a medicine, dose, or schedule changes, ask the prescriber or pharmacist to explain the new directions and update the written schedule and medication list. Bring up memory difficulties or other barriers at follow-up. Do not stop a medicine, change a dose, combine medicines, or split, crush, or chew a tablet on your own. FDA advises asking a health professional before altering a tablet because not every medicine can be handled that way.
