Lowering your risk of a first stroke is a combination of everyday habits and preventive medical care. Healthy routines matter, but they cannot replace screening or clinician-directed treatment when blood pressure, cholesterol, or blood sugar needs attention.
Start by finding the risks you can address
More than 600,000 people in the United States have a first stroke each year, according to the American Stroke Association in 2024. Its statement that up to 80% of strokes may be preventable is a population-level estimate—not a promise that lifestyle changes alone can prevent a particular person’s stroke.
Make regular preventive care part of prevention
Ask a primary-care professional to review your blood pressure, cholesterol, blood sugar, and weight, and to discuss how often you should be screened. The 2024 American Heart Association/American Stroke Association (AHA/ASA) primary-prevention guideline emphasizes regular primary care, screening, and management of risk factors. Depending on your results and overall health, care may include medication such as an antihypertensive or statin; do not start, stop, or change a medicine without your clinician.
Access to care, affordability, neighborhood conditions, and other social factors can make prevention harder. If cost, transportation, food access, or time is a barrier, tell your care team so you can discuss realistic options and available support.

Bring relevant personal history to the conversation
For women and people with relevant reproductive or hormone-related histories, the 2024 guideline identifies oral contraceptive use, pregnancy complications, early-onset menopause, and estrogen use for gender affirmation as considerations in risk assessment. These factors do not affect everyone in the same way; discuss your own history rather than assuming it determines your risk.
Make blood pressure a priority
High blood pressure is a leading modifiable risk factor for cardiovascular disease, including stroke. The 2025 AHA/ACC blood-pressure guideline classifies adult readings as follows. These are categories, not personal treatment instructions; a clinician interprets readings in context.

Use home readings as part of a care plan
The 2025 guideline recommends home blood-pressure monitoring for patients to help confirm an office diagnosis and track progress as part of integrated care. If your clinician recommends a monitor, choose an upper-arm cuff that fits, check its validation or accuracy status, and consider whether the display, memory, or sharing features will make it practical to use. Pay attention to sodium
For blood-pressure prevention and management, the 2025 guideline advises adults to limit sodium to less than 2,300 mg per day and move toward 1,500 mg per day. Checking food labels can help you see where sodium adds up and compare options.

Keep alcohol guidance in its blood-pressure context
The 2025 blood-pressure guideline says ideally consume no alcohol. For people who choose to drink, it advises no more than two drinks per day for men and one per day for women. This is guidance in the context of blood-pressure prevention and management, not a personal prescription.

Move regularly and break up long periods of sitting
The 2024 stroke guideline summary recommends at least 150 minutes per week of moderate-intensity aerobic activity, or 75 minutes of vigorous activity, or a combination, preferably spread through the week. It also associates prolonged sedentary time while awake with increased stroke risk and recommends counseling to avoid being sedentary.
Build a routine that fits your health and schedule
Start gradually if you are inactive, and distribute movement through the week in a way you can maintain. The 2025 blood-pressure guideline separately describes 75–150 minutes per week, including aerobic and/or resistance exercise, in the context of blood-pressure prevention and management. If you have a chronic medical condition, check with a health professional before beginning an exercise program.

Avoid tobacco and support healthy sleep
Avoid smoking, vaping, and secondhand smoke
Public stroke-prevention guidance advises not to smoke or vape and to avoid secondhand smoke. If quitting is difficult, support may include a health professional, a quit line, a cessation program, or medication when appropriate.
Make room for adequate sleep
Sleep is among the AHA’s Life’s Essential 8 health metrics. The American Stroke Association’s public guidance gives 7–9 hours as general advice for adults; individual sleep needs vary, and this range should not be treated as a stroke-specific dose or guarantee.
Set realistic goals and review progress
The 2025 blood-pressure guideline describes a goal of at least 5% body-weight reduction for adults with overweight or obesity as part of blood-pressure guidance. Weight change alone is not a guarantee against stroke, and a clinician can help determine whether this goal is suitable for you. Focus on manageable changes and regular follow-up rather than trying to change everything at once.

Prevention may involve both daily habits and treatment. For specific patients with diabetes and elevated cardiovascular risk or established disease, GLP-1 receptor agonists are among the treatments mentioned in the 2024 guideline; they are not general-purpose stroke-prevention drugs. Ask your clinician what applies to your health profile, and do not use supplements, consumer tests, or gadgets as substitutes for diagnosis or treatment.
Know when this prevention advice is not enough
This article concerns primary prevention—a first stroke. Anyone with a prior stroke or transient ischemic attack (TIA) needs individualized secondary-prevention care; this guidance is not a substitute for it. If stroke warning signs appear, call 911 in the United States. The American Stroke Association’s FAST reminder is face drooping, arm weakness, speech difficulty, and time to call 911.
