Keep lasting project decisions in version-controlled documentation, then point both Claude Code and Codex to that shared record from their respective instruction files. Use CLAUDE.md for Claude Code project guidance and AGENTS.md for Codex guidance; neither conversation history nor a session alone should be treated as the authoritative project record.
Choose a shared, reviewable record for decisions
Create one canonical decision document in the repository so the team can inspect changes, correct mistakes, and carry decisions into future sessions. An architecture decision record (ADR), a decision log, or a clearly marked section in the project documentation can work. This is a governance approach, not a required file format from Anthropic or OpenAI.
Make each entry dated and explicit about its scope and status: current, superseded, or still open. Include the decision and enough rationale to explain why it was made. When a decision changes, update its status or add a replacement entry rather than leaving contradictory guidance scattered across assistant instructions.

Connect Claude Code and Codex to the same record
Claude Code: use project-level CLAUDE.md
Anthropic documents ./CLAUDE.md as a location for team-shared project instructions and ~/.claude/CLAUDE.md for personal preferences. Its documentation says memory files load at startup. Put a concise pointer in the repository’s root CLAUDE.md telling Claude Code where the decision record and relevant architecture documents live, and how to propose a change.

If shared instructions become long, Claude Code supports explicit imports using @path/to/import; imports can be nested up to five hops. Applicable parent memory files are read recursively, and nested files can be read when Claude Code works in those subtrees. Discovery details can vary by installed version, so check the current documentation: Anthropic’s Claude Code memory documentation.
Codex: use repository-level AGENTS.md
Put the equivalent pointer in the repository’s root AGENTS.md. OpenAI’s Codex prompting guide describes instruction discovery from the global Codex directory and repository path through the current working directory, with guidance injected root-to-leaf and later directory guidance taking precedence in that implementation. This evidence describes a Codex CLI implementation, not a universal guarantee for every Codex product, version, or custom deployment. CLAUDE.md and AGENTS.md are not interchangeable by filename alone. OpenAI’s migration guidance maps repository or workspace guidance to AGENTS.md, reusable workflows to skills, and agent behavior to agent instructions. Keep tool-specific directions in the file each tool recognizes, while both files point to the same decision record: OpenAI’s migration guide.

Use repository files for decisions that should apply to a team and travel with the code. Keep user-level files for preferences that should follow one person across projects; Claude Code explicitly distinguishes personal memory from project memory. A session transcript can provide context for one interaction, but it is not a dependable substitute for a reviewed project record.

OpenAI’s agent documentation distinguishes conversation history in an SDK session from sandbox memory carried across later runs. These are different forms of continuity, and neither replaces a team-controlled record of why the project is built a certain way: OpenAI’s Sandbox Agents documentation and Building Reliable Agents with Memory and Compaction.
Use a repeatable start-to-finish workflow
- Establish the record. Add a decision log, ADR, or documentation section to the repository. Define how entries are dated, scoped, and marked current, superseded, or open.
- Point Claude Code to it. In root
CLAUDE.md, identify the canonical record and relevant architecture documents, and explain how to propose an update. Use imports if detailed shared instructions should live in separate files. - Point Codex to the same record. In root
AGENTS.md, give Codex the matching pointer and concise instructions for consulting and proposing updates. - Start fresh sessions with explicit context. Ask the assistant to read the applicable instruction file and canonical decision document before making consequential changes. This is a useful safeguard even when automatic discovery is configured.
- End consequential work with a proposed update. Ask the assistant to draft a dated change to the decision record when the work changes an architectural or project-level decision. Have a person review it before treating the update as canonical.
- Verify what actually loaded. In Claude Code, use
/memoryto see loaded memory files. For Codex, inspect the effectiveAGENTS.mdchain for the actual working directory and confirm behavior against the installed version.
Diagnose missing or conflicting context
- The assistant repeats a settled question: confirm the decision is in the canonical document, not only in a past chat, and ensure the tool-specific instruction file points to it.
- A nested project area seems to ignore guidance: check the working directory and the applicable parent and nested instruction files. Codex’s described root-to-leaf precedence means a more specific instruction can take precedence in the documented CLI implementation.
- Claude Code did not load expected guidance: inspect
/memory, verify the file location and any import path, and check the current Claude Code documentation for version-specific discovery behavior. - The two assistants give contradictory advice: compare their instructions with the canonical record, resolve the conflict in the reviewed documentation, and then update the relevant pointers or guidance. Avoid copying the full decision history into both instruction files, where divergence is harder to manage.
Anthropic also recommends keeping memory specific and structured and reviewing it as projects evolve. Treat instruction files as maintained project documentation, not an archive that grows without review.
