When a state changes a workers’ compensation form or filing process, check the agency responsible for the claim, confirm the current form and instructions, and note the effective date, transition deadline, and required filing channel. Replace saved templates only after verifying the specific state’s rules: forms and cutoff dates are not uniform nationwide.

Start with the state and claim—not a saved form

First identify which state’s workers’ compensation process governs the claim and who must file or receive the document. An employer’s headquarters alone does not establish which state’s process applies. Confirm the jurisdiction for the particular claim, then use the responsible state agency’s official forms page.

State agencies maintain their own catalogs and instructions. For example, Texas organizes forms by categories that include employer, carrier, employee, and medical forms; New York’s listings identify form owners, filing destinations, and current-version details. Search the official page by the form number and title, not just by a generic description.

Connecticut’s Workers’ Compensation Commission tells system participants to use its official forms to ensure they are up to date, accurate, and meet statutory requirements. Idaho, meanwhile, cautions that its policy statements and guidance do not have the force and effect of law. Read agency guidance alongside the form-specific instructions and any controlling requirement.

Check version dates and transition rules

Record four dates separately: when a revision takes effect, when the new version becomes required, the last date the prior version is accepted, and any deadline for filing the completed form. An update’s publication or effective date does not necessarily mean that every older copy becomes unusable immediately.

Maine illustrates the distinction: its forms index says revised forms were effective June 1, 2026, and required as of September 1, 2026. Those dates describe Maine’s transition, not a general workers’ compensation schedule.

Do not assume an older version will be accepted. New York says the Workers’ Compensation Board may announce that older versions of updated forms will not be accepted. Georgia’s forms page stated that outdated forms would be returned beginning October 1, 2025. Check the current policy for the exact form and state before sending it.

Confirm how the form must be filed

A revised form can come with a revised submission method. Check the agency’s instructions for the portal or eForm, paper-mail rules, recipient copies, and any separate electronic transaction needed to update the claim record.

New York: RFA-2 eForm

In an April 9, 2026 payer update, New York said use of the RFA-2 eForm became mandatory effective April 8, 2026. Paper forms postmarked after April 7 would not be accepted or acted on. This rule concerns the RFA-2 process described in that update; it should not be generalized to other forms.

Pennsylvania: document delivery and EDI

Pennsylvania’s LIBC-764 instructions say to mail the notice to the employee and upload it to WCAIS or mail it to the Bureau. They also state that an EDI transaction is needed to update claim status in WCAIS. Uploading or mailing the document should not be treated as a substitute for that separate status transaction. The PDF carries an April 2023 revision date, so verify that the form and instructions remain current before use.

Oregon: revised Form 3058

Oregon Bulletin 232, revised April 8, 2026, republishes Form 3058 after wording changes related to House Bill 4040. The bulletin encourages immediate use and directs use no later than June 1, 2026. Follow the bulletin’s stated requirements for that form rather than assuming the timing applies elsewhere.

Use a repeatable update workflow

  1. Identify the governing state and filer. Confirm which jurisdiction’s claim process applies and whether the employer, carrier, employee, self-insured entity, or another participant must act.
  2. Open the state agency’s official forms page. Find the form by number and title. Check its revision date, instructions, filing destination, and any linked bulletin or update notice.
  3. Write down the transition dates. Distinguish effective date, required-use date, old-form cutoff, and the claim’s filing deadline. If an old version is accepted only during a transition, record the precise cutoff.
  4. Verify the full submission path. Check whether the agency requires an eForm, portal upload, paper mailing, recipient copies, or an accompanying EDI transaction. Confirm the correct destination for each copy.
  5. Update internal processes. Replace saved templates and checklists, tell staff or vendors responsible for filing, and retire superseded copies so they are not selected accidentally. This is a practical control; the particular agency notice determines the legal filing requirement.
  6. Keep the notice with the process record. Save the official update or bulletin alongside the revised template and instructions. If directions are unclear, contact the relevant agency or the carrier or claims administrator before filing.

If a state offers more than one form version or filing route, use this checklist to compare the options:

  • Which form version is current for this claim and filing date?
  • When does the revision take effect, and when is it mandatory?
  • Can the prior version still be used during a transition, and what is its final acceptable date?
  • Must the filing go through a portal or eForm, or is paper still accepted?
  • Who must receive a copy, and where must each copy go?
  • Is a separate EDI transaction required to update claim status?
  • Does the form or route depend on whether the filer is an employer, carrier, employee, or self-insured entity?

Use the current instructions for the specific form and claim. A process described for one state, form, or participant does not establish the rule for another.
