When a file-system or storage-path failure is followed by SQL Server corruption errors, treat the incident as both an I/O problem and a database-consistency problem. Preserve the evidence, investigate the storage path, run a complete DBCC CHECKDB, and restore to a safe target from a known-good backup whenever possible. Use REPAIR_ALLOW_DATA_LOSS only when restoration is unavailable, because repair can discard data and still leave logical or business-integrity defects.

1. Preserve evidence before changing anything

Do not begin with a repair command or an online file-system check. Record the exact SQL Server error text, database and file names, file offsets, timestamps, and the sequence of events. Preserve the SQL Server error log and relevant Windows event-log entries before logs roll over.

  • Note every affected database file, including data and log files.
  • Record whether errors are intermittent, recurring, or confined to one volume or storage path.
  • Capture the SQL Server version, edition, recovery model, availability configuration, and current backup inventory.
  • Keep copies of the original error output and the complete later DBCC CHECKDB report.

2. Diagnose the storage path, not just the database

What error 823 means

SQL Server error 823 is raised when an operating-system file I/O call fails. Microsoft says it usually indicates a problem in the underlying storage system, hardware, or driver path, although file-system inconsistency or a damaged database file can also be involved. Review the SQL Server error log and the related Windows system events, then involve the owners of the disks, controllers, storage network, firmware, and drivers. What error 824 means

Error 824 reports a logical consistency problem detected during a read. It can also point to an I/O-subsystem fault, so do not assume that the database alone is damaged. Check the SQL Server error log and the msdb.dbo.suspect_pages table. Microsoft documents that this table records suspect pages for certain 823 and 824 errors and helps determine whether restoration is needed.

Check the complete I/O chain

Correlate SQL Server timestamps with Windows storage, disk, controller, multipath, virtualization, and driver events. Ask the responsible infrastructure teams to check failing media, path failovers, firmware, and driver changes. Microsoft also describes SQLIOSim, shipped with SQL Server 2008 and later, as a way to test whether 823 errors can be reproduced outside normal SQL Server I/O requests. It is a diagnostic aid, not a substitute for correcting the underlying fault; see the 823 documentation.

3. Run a full database consistency check

After the storage condition has been stabilized or isolated, run a full DBCC CHECKDB and save all output:

DBCC CHECKDB (N'YourDatabase') WITH NO_INFOMSGS;

CHECKDB assesses physical and logical consistency of database structures. It can show what is inconsistent, but it does not identify why the storage path failed. A clean result is not proof that an intermittent or recurring I/O problem is fixed; continue storage-path diagnosis if the operating-system or SQL Server errors persist. Consult DBCC CHECKDB (Transact-SQL) for version-specific behavior and options.

4. Restore from a known-good backup before repairing

If CHECKDB reports permanent consistency errors, Microsoft recommends restoring from a database backup rather than running a REPAIR_* option. The official wording is: If any errors are reported by DBCC CHECKDB, we recommend restoring the database from the database backup, instead of running DBCC CHECKDB with one of the REPAIR_* options. (Microsoft)

Evaluate the backup chain

Identify the latest usable full backup, applicable differential backup, and subsequent transaction-log backups. The correct set depends on the recovery point you need and on the actual recovery model and backup history. Do not assume a backup is clean merely because it completed; use a known-clean restore candidate and test the associated log sequence.

Restore on a safe target first

  1. Use a separate server, instance, or isolated storage target where the suspected failing path is not reused.
  2. Restore the selected full, differential, and log sequence according to your documented recovery point.
  3. Run DBCC CHECKDB on the restored copy and retain its output.
  4. Validate that required application data and recovery objectives are met before redirecting production activity.

This approach preserves the original files for investigation and avoids turning a recoverable backup into an untested production change.

5. Use repair only when restoration is impossible

REPAIR_ALLOW_DATA_LOSS is a last resort when no usable restoration path exists. The repair level printed in CHECKDB output is not an instruction to skip backup recovery. A representative command is:

DBCC CHECKDB (N'YourDatabase', REPAIR_ALLOW_DATA_LOSS);

Run it only under an approved incident plan for the exact SQL Server version and after preserving the original database and evidence. Repair may deallocate or discard pages and data, and Microsoft warns that it can lose more data than restoring a last-known-good backup.

Validate more than physical consistency

A successful repair does not guarantee transactional, logical, or business consistency. Afterward:

  • Run DBCC CHECKDB again and save the complete report.
  • Check foreign keys, unique constraints, indexes, and other required database rules.
  • Reconcile critical totals and workflows with application owners.
  • Back up the recovered database immediately and monitor the storage path for recurring I/O errors.

6. Treat chkdsk as a separate, offline operation

Do not run chkdsk against volumes containing active SQL Server database files while SQL Server is running. Microsoft warns that active writes can create transient errors and says to stop SQL Server before file-system repair. The /f and /r switches can move file bytes; a disk-error fix can therefore corrupt database files, which is why current database backups should exist before proceeding.

  1. Coordinate downtime and stop SQL Server cleanly.
  2. Confirm that database backups and the incident evidence are preserved.
  3. Follow the operating system and storage vendor’s version-specific instructions for chkdsk, especially when considering /f or /r.
  4. After the file-system work, review new Windows and SQL Server events and rerun consistency checks before returning the database to service.

When to escalate

Escalate to your storage and hardware teams when 823 or 824 errors recur, suspect pages continue to accumulate, or Windows reports device, path, controller, or driver faults. Involve SQL Server specialists when the backup chain is incomplete, CHECKDB output is extensive, repair is being considered, or application reconciliation cannot establish that recovered data is trustworthy. The exact procedure depends on your SQL Server version, edition, storage design, availability configuration, and backup history.
