Check freshness before retrieved documents enter a model’s prompt: compare each item’s timestamp and lifecycle state with the authoritative source, then exclude, quarantine, or visibly qualify anything expired, superseded, too old for its domain, or impossible to verify. Semantic relevance alone does not establish that a passage is still true.
Why freshness needs its own check
Retrieval-augmented generation (RAG) selects source material to provide context for a model’s response. A document can be an excellent semantic match for a question and still be out of date: a newer policy may have replaced it, a price may have changed, or a system status may no longer apply.
A 2026 arXiv preprint by Ahmed, Poech, and Röttger describes this risk as stale-document poisoning and reports experiments across medicine, law, software, and platform policy. In the study, stale evidence flipped 30% of Llama answers and 37% of Qwen answers when the models were not instructed to trust the retrieved document; the authors report higher rates when they were explicitly told to follow it. These are findings from that experiment, not estimates of error rates in production systems. The paper also reports that a fixed recency-aware hybrid reranker reduced errors by 4.6–10.0 percentage points when date metadata was accurate, underscoring that recency signals are only as dependable as the metadata behind them. Keep freshness information attached to each retrievable chunk, or make it possible to recover from the chunk to its source record. At minimum, record when the content was ingested or last observed and identify the authoritative source against which its current status can be checked. Where the domain has explicit lifecycle rules, also store a version, effective date, expiry, or state such as current, superseded, or withdrawn.

These fields are an engineering design pattern, not a universal RAG metadata standard. The key distinction is that ingestion time tells you when your system saw content; it does not prove the content remains valid. A source revision or validity state is more useful for deciding whether the item may be served as current.
Build a pre-generation freshness gate
Run the check after retrieval and before assembling the final prompt. Evaluate every candidate against the relevant policy and authoritative source, rather than assuming that a high relevance score implies freshness.

- Resolve provenance. Identify the source record and the timestamp, version, or validity state that governs the retrieved text.
- Apply a domain-specific rule. Reject expired or superseded material, and compare its age with a threshold chosen for the information type and the consequences of serving it. There is no universal staleness interval that fits all domains.
- Choose a safe outcome. Exclude the item when it is invalid; quarantine it for review if its state is uncertain; or label it clearly if the application permits qualified use. Do not silently present unverified material as current.
- Handle missing or failed checks explicitly. If the source cannot be reached or freshness cannot be established, say that currency could not be verified, or fall back to a suitable live source rather than treating the cached chunk as confirmed.
AWS’s Agentic AI Lens recommends: “Track ingestion timestamps and flag content that exceeds staleness thresholds before it is served.” Its examples of information that may require live retrieval include prices, inventory, and system status.
Choose a threshold that matches the information
Set thresholds based on how quickly the source changes and how harmful an outdated answer could be. A slowly changing reference document and a live inventory count should not necessarily follow the same age rule. Where even a short delay matters, a cached document may be the wrong source of truth; query the authoritative system at response time instead.

Assess a freshness-control design using its detection latency, the reliability of source timestamps and versions, correctness of update and deletion handling, behavior when freshness is unknown, retrieval relevance, operating cost, and serving latency. The cited guidance does not establish one refresh interval or a cross-vendor winner.
Keep the index aligned with its sources
Freshness checks cannot compensate for an index that continues to expose old chunks after a source changes. Synchronize the index when authoritative content is revised, and verify that updates and deletions propagate correctly. GOV.UK’s RAG guidance describes the retrieval workflow and notes the importance of retrieval quality and re-indexing. Re-indexing may also be necessary when chunking or embedding methods change.

Build reconciliation into operations: compare indexed records with source records, detect items that are missing or no longer valid, and make them ineligible until the discrepancy is resolved. Treat deletion as a lifecycle event to test, not merely as a change to the source system.
Use live retrieval for rapidly changing facts
When the answer depends on a fact that can change faster than the index can reliably synchronize, retrieve it from the live authoritative system. AWS specifically points to prices, inventory, and system status as cases where relying on cached information can be insufficient. If that live lookup fails, expose the limitation: the response should not imply that a cached value has been verified as current.

Test the complete path before serving
A freshness policy is only useful if it prevents the old version from reaching generation. Test the source, synchronization, retrieval, gate, and prompt assembly together:
- Change or supersede a source record and note its authoritative version or state.
- Wait for the normal synchronization process, then inspect the index to confirm the new state and check whether the old chunks remain eligible.
- Ask a query likely to retrieve the old version. Inspect retrieved candidates and verify the gate excludes, quarantines, or marks stale material as intended.
- Inspect the request trace or assembled context to confirm the old content did not enter the model prompt as current evidence.
- Repeat with a deletion and with an unavailable source or missing timestamp to verify the system’s failure behavior.
This is a practical test procedure, not a published benchmark. NVIDIA’s RAG pipeline documentation covers pipeline observability, which can help teams inspect how retrieved context moves through the system.
