First identify whether the problem is headset detection, video output, tracking, or PC performance: each points to a different fix. The steps below focus on wired HTC VIVE systems using SteamVR because that is where the cited official guidance applies. Quest Link, wireless PC VR, and inside-out-tracked headsets use different connection and tracking systems; check the headset maker’s instructions for those models.
Identify what is failing
Before changing cables or settings, note what you see in SteamVR and in the headset. A headset missing from SteamVR is a different problem from one detected over USB but showing no picture. A grey view or floating controllers suggests tracking trouble, while motion stutter with tracking indicators intact may point to PC performance.
- Headset absent from SteamVR: investigate USB detection, power, and SteamVR startup.
- Detected over USB, no picture: check the video path and display mode.
- Grey view or devices drifting: check tracking and, on lighthouse-based VIVE systems, base stations and the room.
- Stutter while tracking stays normal: inspect SteamVR frame timing and PC load.
Steam’s indicators and timing graph are diagnostic clues, not proof that a component is either healthy or defective.
Fix a headset SteamVR cannot find
For the HTC VIVE errors covered by Steam Support—108, 125, 126, and 211—start with reversible connection checks before considering hardware. [Steam Support’s headset-not-detected troubleshooting flow]

- Check headset and link-box state. HTC VIVE says an unlit headset LED can warrant checking link-box power; dim green indicates standby, while red indicates a cable-connection or driver error. Check that each cable is firmly seated at both ends and connected to the correct link-box side. See HTC VIVE’s headset troubleshooting guide.
- Try another USB port. HTC recommends trying a different port, including switching between USB 3.0 and USB 2.0 when available. Steam also advises trying another port, including one known to work with another device. Follow the connection requirements for your specific headset.
- Restart the headset and SteamVR. Restart the headset from SteamVR, then relaunch SteamVR. If detection still fails, restart the PC.
- Power-cycle the link box. Quit SteamVR, disconnect the link box’s power and its USB connection on the PC side, wait a few seconds, then reconnect them. If Windows prompts to install drivers, let that finish before reopening SteamVR.
Fix USB detection with no headset display
Steam labels Vive Error 208 “HMD detected over USB, but monitor not found.” That points to a display-path problem rather than the same failure as a headset that SteamVR cannot detect. The label is specific to the HTC VIVE/SteamVR troubleshooting context; do not assume Error 208 has the same meaning on another headset.
- Reseat the video connections. Check that the headset, link box, and PC video cables are secure.
- Verify which graphics output you are using. For the VIVE setup covered by HTC, connect the video-output cables to the same graphics card—not to motherboard video ports. If needed, try another available output on that graphics card. This is a wired-display check, not a universal adapter recommendation.
- Check Direct Mode. Consult Steam’s SteamVR troubleshooting guide for Vive for its Error 208 and Direct Mode steps, and use the guide for your exact headset if it differs.
Fix grey views, lost tracking, or floating devices
A grey headset view or a pulsing green device icon in SteamVR can signal tracking loss. For lighthouse-tracked VIVE systems, check that the base stations are powered and synchronized, and that the headset and controllers can see at least one powered, synchronized station. - Position base stations at least 6.5 feet above the floor.
- Angle them down 30–45 degrees.
- Keep them no farther than 5 meters (16 feet) apart.
- Check reflective surfaces if tracking drops or controllers float; covering them may help.

Tell tracking loss from PC performance stutter
If movement looks unstable but SteamVR still shows normal tracking, open SteamVR’s frame timing graph and look for spikes. Steam says spikes make CPU performance a more likely cause than tracking. A timing spike is a clue to investigate PC load; it does not, by itself, identify a specific process or prove the cause.

When to consider a USB expansion card
Consider hardware only after trying other ports, controlled restarts, and the headset maker’s connection checks. Steam Support reports that its testing found the Inateck 2-Port USB 3.0 PCIe card, serial KTU3FR-2O2I, worked reliably with HTC VIVE and could address some USB chipset issues. This is a model-specific, historically documented fallback—not a general fix for PC VR. Check the exact card’s current availability and compatibility with your motherboard, operating system, and headset before buying it. Steam’s guide does not establish that it will resolve every intermittent USB problem.
