Start with the clinician or rural clinic already managing your care. Ask which specialty you need, whether a referral or prior authorization is required, and whether a first visit can happen by telehealth. Before booking, confirm network status, coverage, likely costs, and any travel needs with both the provider and your insurer. These steps help you find a workable route without assuming a specialist, remote visit, or transportation benefit is available locally.

Start with the clinician who knows your case

Ask your primary-care clinician, hospital clinic, community health center, or Rural Health Clinic to help identify the right specialty and connect you with regional providers. Medicare describes Rural Health Clinics as sources of outpatient primary and preventive care in non-urbanized areas with provider shortages; having one nearby does not mean the specialist you need practices there. Medicare.gov’s Rural Health Clinic services page explains the clinic role.

Ask the clinician or referral staff:

  • Which specialty should evaluate this problem, and how urgent is the appointment?
  • What records, test results, imaging, or medication list should go with the referral?
  • Does the specialist need to examine you in person, or could an initial or follow-up visit be remote?
  • Who will send the referral and confirm that the specialist received it?

There is no single national referral rule that applies to every plan and service. Ask the receiving specialist’s office whether it requires a referral, and check the rule with your insurer before scheduling.

Search across the region, not just the nearest town

Ask the referring office about specialist clinics and regional hospital systems that accept your insurance. The office may know referral pathways or care coordinators who can help locate a suitable service. Availability, wait times, and whether a clinician is taking new patients depend on the area, so confirm those details directly with the provider.

If you are comparing routes, check the factors that affect whether an option will actually work:

  • Clinical fit: whether the clinician treats your condition and whether an exam or test requires an in-person visit.
  • Travel: distance, likely number of trips, and whether follow-up tests are at another location.
  • Coverage: network status, referral and prior-authorization rules, and an estimate of your share of the cost.
  • Access: appointment availability, follow-up arrangements, and the provider’s video or audio-only options.

Ask whether telehealth is appropriate and covered

A remote appointment can reduce travel when the specialist considers it clinically suitable, but it may not replace an examination, procedure, or test. Ask the referring clinician and specialist whether your particular case can be handled remotely, what technology is needed, and whether you may still need an in-person visit. Then confirm coverage for that service with your insurer.

What Medicare’s current rules say

Policy dates and service categories matter. HHS Telehealth.gov’s page, last updated February 5, 2026, describes Medicare non-behavioral and mental telehealth at home without geographic restrictions through December 31, 2027, and says audio-only telehealth is available through that date. It also describes permanent Medicare flexibilities for behavioral and mental telehealth, including home access and Rural Health Clinic and Federally Qualified Health Center participation as distant-site providers. These are Medicare rules, not a promise that every specialist service is covered remotely or that Medicaid and commercial plans follow the same rules. See the HHS Telehealth.gov policy updates.

For the specific Rural Health Clinic and Federally Qualified Health Center distant-site provision, later CMS guidance issued May 28, 2026 says the Consolidated Appropriations Act, 2026 extended the flexibility until January 1, 2028, describing payment through calendar year 2027. Check the CMS guidance MM14468 and verify current rules when arranging care, since legislation and policy can change.

If you have Medicare, ask whether the particular service is on the current covered-service list, whether the provider participates, and whether audio-only care is appropriate and available. HHS notes that telehealth reimbursement continues to evolve and provides links for Medicare, state Medicaid, and private insurance. Medicaid rules vary by state; private plans also need to confirm their own coverage. See HHS guidance on paying for telehealth.

Confirm insurance, authorization, and expected cost before booking

Call the number on your insurance card and ask about the named specialist and service—not just whether the health system accepts your plan. Confirm that the specific clinician is in network, whether the referral must come from a particular clinician, whether prior authorization is required, and what you may owe for the visit and related tests. Ask the provider’s billing office for a pre-visit estimate and whether separate facility charges may apply.

For context, Medicare.gov says that after the Part B deductible, patients generally pay 20% of covered charges for Rural Health Clinic services. That is general Medicare information, not a price quote for a specialist visit; the amount can vary with other insurance, provider charges and assignment, facility type, and service location. Ask whether telehealth has different cost-sharing from an in-person visit and whether tests, imaging, or follow-up visits are billed separately. Write down the plan representative’s name, the date, and any call reference number, then confirm practical details with the provider before making a long trip.

Ask for help with travel and care coordination

Long travel distances and limited public transportation are documented barriers to rural care. If getting to an appointment is difficult, ask the clinic’s care coordinator, insurer, county or community social-service navigator, or the specialist’s patient-services office whether local transportation or lodging support exists. Do not assume a nationwide ride benefit: availability depends on the program, location, and eligibility rules.

Care coordination can connect patients with regional providers and support services. The 2023–2027 Rural Health Care Coordination Program Directory describes ten initiatives funded in ten states and gives examples of local networks addressing conditions including heart disease, cancer, chronic lower respiratory disease, stroke, and maternal health. Its examples show possible coordination approaches, not a guarantee that the same services operate in every community. The directory says over 60 million people live in geographically isolated rural and tribal communities in the United States; that figure is attributed to the directory and is not a new 2026 population estimate. HRSA points people to the Rural Health Information Hub (RHIhub), which offers state guides, a resource library, toolkits, program models, and free phone or email help. Its resources can help you investigate local options, but they do not themselves guarantee an appointment or ride. HRSA’s page was last reviewed in April 2025. Explore HRSA rural health resources.

A practical booking checklist

  1. Ask your current care team: identify the specialty, records needed, urgency, and whether an in-person examination is necessary.
  2. Get referral details: ask who submits the referral and confirm the specialist’s office received it; check your plan’s referral and authorization rules.
  3. Find regional options: ask about nearby systems and clinics, then confirm that the clinician is accepting patients and in network.
  4. Check telehealth: ask whether the specialist offers a remote visit for your case, which formats are available, and what would still require travel.
  5. Verify the bill and logistics: obtain a cost estimate, ask about separate tests or facility charges, and contact a care coordinator or local navigator about travel help.
