For keratosis pilaris (KP), gentle consistency beats vigorous scrubbing. The harmless bumps are plugs of dead skin cells in hair follicles, often on the upper arms and thighs; you do not have to treat them unless their appearance or symptoms bother you. If you choose to exfoliate, use a gentle method, moisturize afterward, and ease off if your skin becomes irritated.
Why scrubbing KP can make it worse
KP is caused by dead skin cells plugging hair follicles. Scraping or vigorously rubbing those bumps can irritate the surrounding skin and make KP look or feel worse. The American Academy of Dermatology Association (AAD) advises: “Avoid scrubbing your skin, which tends to irritate the skin and worsen keratosis pilaris.”
Exfoliation is optional. If you try it, think light strokes and a gradual routine—not removing bumps by force.

Light physical exfoliation
During bathing, you can use a soft washcloth or, if your skin tolerates it, a loofah or buff puff. Use light strokes; do not press hard, scrape, or use a harsh scrub. If even gentle rubbing causes redness or discomfort, stop.

Topical keratolytics
A topical product with a keratolytic ingredient can help loosen the buildup. AAD lists alpha hydroxy acids, including glycolic and lactic acid, as well as salicylic acid, urea, and retinoids as options. These are alternatives to consider, not ingredients you need to layer together. Follow the product label or your clinician’s directions: using too much or applying it too often can leave skin raw or irritated.
How to decide what to try
There is no established universally best method. Dry, sensitive, or acne-prone skin may tolerate a washcloth and a mild chemical exfoliator better than mechanical exfoliation. Be especially cautious if you already use retinoids or benzoyl peroxide, or if your skin tends to become dry or irritated. Do not exfoliate open cuts, wounds, or sunburn; aggressive exfoliation can cause redness and may increase pigmentation, particularly in people prone to dark spots.

Build a low-irritation routine
- Wash gently. Use a mild cleanser and warm rather than hot water. Keep baths or showers to 20 minutes or less, as AAD recommends in its KP self-care guidance.
- Use one gentle exfoliation step, if desired. Try light strokes with a soft washcloth or use one topical keratolytic as directed. Do not scrub harder to speed results.
- Moisturize after bathing. Apply a thick, oil-free cream or ointment to damp skin within five minutes. AAD recommends looking for a moisturizer containing urea or lactic acid; reapply when skin feels dry.
- Adjust to your skin. Follow the product’s or clinician’s instructions for amount and frequency. If the active medicine makes your skin dry or irritated, stop it for a few days before deciding whether to resume.
Frequency varies by person and product. AAD’s home-care advice describes daily care, while its treatment guidance emphasizes the dermatologist-recommended schedule and notes that maintenance may eventually be as little as twice a week. Use your product label or clinician’s plan rather than treating either schedule as universal. AAD’s treatment page also advises moisturizing at least two or three times a day.

Pause the active product for a few days if your skin becomes dry or irritated, and avoid adding more exfoliation to compensate. If your treatment plan has not improved things after four to six weeks, contact a dermatologist. That interval is guidance for follow-up, not a promise that KP will clear by then.

KP treatment does not cure the condition, so maintenance may be needed. Shaving or waxing can cause more bumps for some people with KP, so consider whether hair removal is aggravating your skin.
