Choose overnight incontinence protection by matching the style to who will put it on and change it, the absorbency and design to the wearer’s leakage pattern, and the size to the product’s own chart. A snug, comfortable fit and reliable containment matter alongside absorbency; no product can be assumed to last all night without a change.
Choose a style the wearer can manage
Protective underwear is a pull-on garment that resembles regular underwear. It may work well when the wearer can dress and toilet with limited help. A tab-style brief wraps around the body and fastens at the sides; it can be easier for a caregiver to change, including when the wearer is lying down.
Neither style is automatically more absorbent. Compare the capacity and construction of the specific products rather than assuming the garment type determines performance. Consider the wearer’s mobility and dexterity, who will handle changes, and which design feels comfortable and manageable.
Match absorbency to the overnight pattern
Start with what happens overnight: how much leakage occurs, when it tends to occur, and whether the wearer is comfortable with the current product. Then check the product’s stated absorbency and whether it is designed for nighttime use. The Wound, Ostomy and Continence Nurses Society (WOCN) guide recommends disposable briefs with additional absorbent capacity designed for nighttime use in certain moderate- or heavy-incontinence situations. That guidance is not a guarantee that a particular item will last through the night.

As the Centers for Medicare & Medicaid Services (CMS) puts it: “Absorbent products can be a useful, rational way to manage incontinence; however, every absorbent product has a saturation point.” If leakage continues, the product may be reaching capacity, may not fit or contain well, or may not suit the wearer’s pattern.
Check fit with the exact product’s size chart
Measure as the manufacturer directs and use the chart for the specific product. Do not assume that the same size label fits the same way across brands or product lines.

The garment should sit securely without feeling constricting. A loose fit can undermine containment, while a tight fit may be uncomfortable. Check the leg openings and waist as well as the overall fit, and follow the product’s directions if the wearer’s measurements fall between sizes.
Compare containment and overnight construction
Look beyond the absorbency label. Compare where the absorbent area sits, the leg and waist elastics, leakage barriers, and how the garment fits when the wearer is lying down. Those details can affect whether the product suits the person’s sleep position and leakage pattern.

For example, TENA describes one of its overnight products as having absorbency positioned in the back for lying down. Treat that as a feature of that product, not a promise about overnight underwear generally. Product pages and packaging can change, so verify the current design and instructions before buying.
Weigh comfort and skin considerations
WOCN lists comfort, odor control, skin protection, breathability, and appearance among factors to consider when choosing absorbent products. For overnight use, also consider whether the material feels comfortable during sleep and whether noise or bulk is likely to bother the wearer.

Persistent moisture can put skin at risk, as CMS explains. Absorbent underwear is one part of incontinence care, not a substitute for evaluating ongoing skin problems with an appropriate healthcare professional.
Plan changes around the person, not a universal time limit
There is no single change interval that applies to everyone. WOCN notes that optimal timing depends on elimination patterns, sleep-wake patterns, the properties of the absorbent product, and wet comfort. Follow the product instructions and consider the wearer’s comfort and care needs rather than relying on a fixed number of hours.

WOCN describes booster pads or absorbent wraps as possible adjuncts when containment is inadequate. They are not a universal fix or something to add solely for caregiver convenience; check product compatibility and consider whether a different size, style, or absorbency is more appropriate.
