A workflow run history can show recorded runs, but it cannot show an expected event that never became a run. To find those silent misses, track each event outside the workflow platform: record it before sending, capture whether the platform accepted it, collect receipts from workflow actions, and reconcile the records by a unique event ID.

Why a run history cannot reveal every failure

Run history is evidence about runs the platform recorded—not a complete ledger of events you expected it to process. If a trigger stops firing, an event is deduplicated before run creation, or a quota edge prevents a run from being created, the missing event may leave no row to inspect. As Hao puts it, “A log cannot record a run that was never created.”

There is a second blind spot: a successful transport response does not necessarily mean the intended operation succeeded. An endpoint can return 200 OK while its response body contains an error object. A platform that checks only transport status may mark the step successful. Distinguishing transport success from application-level success requires an explicit response assertion or another signal that reflects the intended outcome.

Measure expected events and actual outcomes

Build a small ledger outside the workflow platform. The controller and receipt destination should be under your control, so their records do not depend on the workflow’s own run history.

  1. Record the expected event before sending it. Assign a unique run_id for each event, then record the workload or workflow (wf), platform, sequence number (seq) and event time (fired_at). This event record is your denominator: it captures what you expected to happen.
  2. Record the send outcome separately. Log whether the platform accepted the event. If it refused or the send failed, retain the reason, such as a timeout, connection reset or non-2xx response. Acceptance and refusal are different observations, not interchangeable failures.
  3. Have each relevant action send a receipt. Each receipt should echo the event’s run_id and identify the producing step. Store the receipt time as received_at at a destination you control. The identifier must remain unchanged from the original event through to the receipt destination.
  4. Reconcile receipts against expectations per event. For each accepted event, count receipts with that event’s run_id and compare the count with the expected number of outputs. Keep the records at event level: matching totals can conceal that one event is missing while another is duplicated.

The elapsed time received_at - fired_at is end-to-end destination-to-controller latency. It includes platform processing and both network legs; it is not the platform’s internal execution time and should not be presented as such.

Classify each event before calculating a rate

Use the expected output count and the send outcome to assign one of seven outcomes:

The Silent Failure Rate is (missed + partial) / accepted runs expected to produce output. Exclude refused sends from both numerator and denominator, and report them separately: the platform did not accept those events, so they are not accepted-then-lost operations. Show all seven outcome counts alongside the rate so readers can understand what it includes and what it leaves out.

Report the denominator and uncertainty, not just the percentage

A measured zero is not proof that the true failure rate is zero. Hao’s 2026 illustrations show why sample size matters: zero failures in 40 runs has an upper bound near 7%, while zero in 4,000 runs has an upper bound well under 0.1%. These are interval examples, not estimates of any platform’s reliability.

For rare or zero observed failures, Hao recommends a Wilson score interval with z = 1.96; a normal approximation can collapse to a zero-width interval when no failures are observed. Report the sample size and interval rather than presenting a bare zero. For each rate, also state the workload, as-of date, and whether the measurement is refreshed or a one-off. Those details make the scope and age of the result visible.

What the worked example does—and does not—show

Hao reports a ledger of 2,880 events over five hours in 2026. Four sends were refused, and the example records zero accepted-then-lost events. Under the stated formula, that is zero silent failures among 2,876 accepted events expected to produce output, with the four send rejections reported separately. It is an author-reported example, not an independently audited test or a platform-wide benchmark; it does not establish comparative reliability across platforms.

Choose a measurement level that fits your operation

The specification describes three self-declared conformance levels. They distinguish use of operational data from controlled experiments; they are not independent certifications.

  • L1: Calculate and report the rate from operational data using the stated rules. A test harness, paid plan or experiment is not required.
  • L2: Add a controlled measurement with both endpoints under the operator’s control.
  • L3: Add three edge probes: destination outage, success-wrapped failure, and sustained load.

The specification is described as free under CC BY 4.0, with no code to install. It is not audited, has no certifying body or registry, and its planned reference implementation is unpublished. One section is reserved because its proposed requirement rests on a single observation that did not recur. Treat the levels as the specification’s own framework, not as proof of external validation.

Check whether your setup can support the measurement

Before relying on a report, verify that the implementation preserves the data needed to interpret it:

  • Does each expected event have a unique identifier that survives unchanged to the receipt?
  • Can each relevant action send a receipt to a destination you control?
  • Can the controller record per-event acceptance and the reason for refusal?
  • Can response-body checks distinguish transport success from business success where needed?
  • Does the report show all seven classifications, its denominator and interval, the workload, the as-of date, and whether it is refreshed or one-off?

If any of these records is missing, the resulting figure may still describe the data you have, but it cannot answer the same question as a complete per-event reconciliation.
