Care for a dog with suspected dementia by keeping daily life predictable, offering gentle enrichment, and making the home safer—but arrange a veterinary assessment rather than assuming behavioral changes are simply aging. Pain, sensory loss, seizures, and other illnesses can look similar, and a veterinarian can help identify the cause and tailor a care plan.
Recognize possible signs—and arrange a veterinary assessment
Canine cognitive dysfunction (sometimes called dog dementia) can involve gradual changes in behavior, including:
- Disorientation, getting stuck, or seeming lost in familiar places
- Changes in social interaction or anxiety
- Interrupted sleep or increased nighttime wandering
- House soiling or loss of previously learned behaviors
- Changes in activity level
These signs are not specific to cognitive dysfunction. Arthritis or other pain, systemic illness, seizures, and reduced hearing or vision can cause similar changes. A new or worsening behavior change warrants a veterinary appointment, especially if it is sudden, severe, or accompanied by physical symptoms.

Diagnosis is based on clinical signs and ruling out other causes. Cornell describes a veterinary examination and blood and urine tests, with MRI in some cases; the American Animal Hospital Association (AAHA) also describes examination, neurologic assessment, biochemical evaluation, and urinalysis, and recommends MRI to rule out structural disease in its guideline. The veterinarian will determine which tests fit your dog.

Make the day predictable without making it rigid
Keep the most important anchors consistent: meals, walks, resting place, furniture, and the locations of food and water. Dependable walk times and roughly consistent distances can support regular elimination. You can vary a familiar walk route for new smells while keeping the outing manageable.
Provide a calm, familiar place to rest. Let your dog choose company or quiet, and allow a retreat when they seem tired or overwhelmed. Predictability matters particularly when a dog is anxious: Dr. Gene Pavlovsky of the University of Illinois College of Veterinary Medicine has noted that disrupted routines and unfamiliar stressors can increase stress in dogs with cognitive dysfunction.

During the day, interaction and light may help support a day-night pattern; at night, keep the sleeping place and bedtime consistent and the room darker. Adapt lighting and routine to your dog’s vision, mobility, and response.
Make elimination easier
Ask the veterinarian to check for medical causes of house soiling, such as a urinary infection, before treating it as a cognitive change. More frequent chances to go outside may help. If your dog already uses an indoor potty area, keep it easy to reach and conveniently located; starting new training can be difficult at this stage.

Calmly guide your dog toward a familiar, safe activity or place instead.

Work with your veterinarian on treatment and quality of life
There is no single cure for canine cognitive dysfunction. A veterinarian may discuss enrichment, diet, medication, and supplements as parts of an individualized plan, while also addressing other health problems that affect comfort or behavior. Selegiline is described by Cornell as approved for canine cognitive dysfunction treatment in North America; AAHA calls it the only drug labeled for the condition in its guideline context. Product labeling and availability depend on jurisdiction, so medication decisions belong with your veterinarian.

Do not start human medicines, melatonin, supplements, or a therapeutic diet based on an article alone. Cornell advises discussing melatonin and supplements with a veterinarian. AAHA notes that evidence quality and rigor are limited for various supplements, nutraceuticals, acupuncture, and diets. A 2025 survey of 318 US veterinarians described reported diagnostic and management practices, including uncertainty about treatment efficacy; it does not establish that a particular treatment works.
How common are signs in older dogs?
Prevalence figures vary with the population and method used, so they should not be treated as a prediction for an individual dog. Merck Veterinary Manual reports a cited study in which signs consistent with cognitive dysfunction were observed in 28% of dogs aged 11–12 and 68% of dogs aged 15–16. It separately reports an internet survey estimate of 14.2% among dogs older than 10, with more than 85% of estimated cases undiagnosed. These estimates come from different methods and should not be combined.
