Buy medicines only from trusted, licensed pharmacies, and do not rely on appearance alone to authenticate a product. If a medicine seems unusual, is not working as expected, or causes an unexpected reaction, contact a pharmacist or clinician; if you feel seriously unwell, seek urgent medical care.
What “fake medicine” means
“Fake” is a useful everyday term, but health authorities distinguish two problems. A substandard medical product fails to meet quality standards or specifications. A falsified product deliberately misrepresents its identity, composition, or source. A medicine can be poor quality without being deliberately faked, and a suspicious package is not proof of either problem.
The scale estimates need context. WHO’s 3 December 2024 fact sheet says at least 1 in 10 medicines in low- and middle-income countries are substandard or falsified, citing an underlying WHO analysis of medicines that failed quality-control tests in 2017. That is not a current rate for every country or a U.S. prevalence estimate. WHO also estimates that countries spend US$30.5 billion per year on substandard and falsified medical products; the fact sheet does not specify the estimate’s base year. Why a suspect medicine matters
A product may contain the wrong ingredient, too much or too little of an ingredient, no active ingredient, or harmful substances. Treatment may fail, illness may worsen, or a person may be directly harmed. Medicines that do not work properly can also contribute to antimicrobial resistance. These are possible consequences, not a way to diagnose a particular package.

How to reduce the risk when buying medicine
Choose a licensed source
Use a trusted pharmacy licensed in the jurisdiction where it operates. In the United States, FDA advises buying prescription medicines only from state-licensed pharmacies. Elsewhere, check with the national medicines regulator or an appropriate pharmacy-verification service; licensing and verification systems vary by country. Avoid treating a general online marketplace as a pharmacy.
Check whether the seller is accountable
Before ordering online, consider whether the seller is licensed by the relevant authority, whether a pharmacist or clinician is available to answer questions, and whether the site clearly identifies its location and contact details. WHO flags unregistered sites that conceal a physical address or landline number as a concern. A polished website or a visible phone number alone does not establish that a seller is legitimate.

Ask before using an unfamiliar product
If a medicine is new to you, the source is unfamiliar, or you have questions about its packaging or use, ask a pharmacist or prescriber before taking it. Do not substitute a consumer scanner, app, or visual inspection for professional advice; the sources do not establish a consumer gadget that can reliably authenticate medicines.
What warning signs can—and cannot—tell you
Damaged or unusual packaging, spelling errors, unexpected dates, a change in appearance or smell, treatment that seems ineffective, or an unexpected reaction can justify checking with a healthcare professional. None of these signs proves that a product is falsified, and a counterfeit may look convincing. WHO cautions that “Some falsified medical products are almost visually identical to the genuine product and very difficult to detect.” See WHO’s advice for patients and consumers.

What to do if you suspect a medicine is unsafe or fake
- Contact a pharmacist, doctor, or other healthcare professional. Explain what seems wrong and when you used the medicine. If it is not working properly or you have a reaction, seek professional advice immediately. Do not make treatment changes without getting appropriate clinical advice.
- Keep the product information available. If practical, retain the packaging and note where and when you obtained the medicine, its name, and what happened. Share these details with the professional or regulator; do not use a questionable product as a test.
- Report the concern to your national medicines regulator. Reporting routes differ by country. In the United States, FDA provides routes for suspected counterfeit drugs and unsafe online pharmacy sales, as well as MedWatch for adverse effects. Start at FDA’s Counterfeit Medicine page for the appropriate channel.
- Get urgent care for serious symptoms. If you have severe or rapidly worsening symptoms after taking a medicine, seek emergency medical help rather than waiting for an investigation or report to be resolved.
