The specific Karlach and Astarion dialogue added in Baldur’s Gate 3 Patch 6 is available in the endgame epilogue if you play as Avatar Karlach or Avatar Astarion and speak with God Gale. In that conversation, either character can ask him for a cure for their condition. The patch notes describe these as Avatar-route reactions, not dialogue available on every playthrough that includes Karlach or Astarion as a companion.
How to access the Patch 6 dialogue
- Reach the ending and defeat the Netherbrain. The epilogue takes place after the Netherbrain’s defeat. Larian introduced it in Patch 5 as a post-ending section for catching up with allies. Read Larian’s Patch 5 notes.
- Play as Avatar Karlach or Avatar Astarion. The specific reactions added in Patch 6 are for these player-character routes; the notes do not say that the same lines are available when either character is only a companion.
- Speak with God Gale during the epilogue. Larian says Avatar Karlach and Avatar Astarion can ask him for a cure for their conditions. See the Patch 6 notes.
Does this work on a companion Karlach or Astarion route?
Not for the particular Patch 6 reactions described here, based on Larian’s wording. The update specifically names Avatar Karlach and Avatar Astarion in God Gale’s dialogue. It does not establish that a player who keeps Karlach or Astarion as a companion will get these same cure-request lines. Other character scenes or epilogue interactions may have different conditions; they should not be confused with this Patch 6 addition.
Do you need DLC or a separate purchase?
Larian’s Patch 6 notes describe the dialogue as game content added by an update and do not identify a separate DLC purchase to unlock it. The requirement supported by the notes is reaching the epilogue on one of the named Avatar routes and speaking with God Gale. Larian’s official game page lists a Physical Deluxe edition, but the cited material does not present it as an unlock for these lines.

It is “new” in the sense that Patch 6 added it, not a newly released 2026 scene. In an announcement dated April 15, 2025, Larian said, “Outside of minor bug fixing, Patch 8 will be the last game patch to introduce new content.” That statement refers to new content in game patches; it does not change the route conditions for the earlier Patch 6 dialogue.
