Screenless fitness trackers use motion sensors to estimate activity, optical sensors to estimate pulse, and movement and heart-rate patterns to infer sleep. They do not directly measure every result: steps, calories, heart rate, and sleep stages are algorithmic estimates whose quality depends on the device, fit, activity, and conditions.

How do screenless fitness trackers measure activity?

Steps and movement

A three-axis accelerometer senses movement in different directions. Software analyzes its frequency, duration, intensity, and pattern to classify likely steps and activity. Fitbit describes this approach in its activity documentation; other brands may use different sensors or algorithms.

Step counts can be affected by how the tracker is worn and what the wearer is doing. Fitbit notes that counting may differ when the arm stays still while the body moves, when a device is clipped rather than worn on the wrist, or when a loose accessory interferes with the sensor. It recommends a secure fit and says the arm needs to swing during a walk or run for wrist-based step counting; GPS may help when both hands are occupied. These are Fitbit-specific recommendations, not a guarantee of perfect counts. Fitbit accuracy guidance

Distance, floors, and calories

Distance may be estimated by multiplying steps by an estimated stride length. Fitbit says that on a GPS-tracked activity, distance instead comes from GPS once a signal is acquired. On supported Fitbit devices, an altimeter and barometric-pressure changes help estimate floors climbed.

Fitbit says its calorie estimates combine basal metabolic rate with activity data; devices that measure heart rate also use heart-rate data, especially during exercise. These are estimates, not direct measurements of calories burned. Google’s Fitbit activity documentation

How do fitness trackers measure heart rate?

Many wrist trackers use photoplethysmography (PPG), an optical method. LEDs shine light into the skin, and a sensor measures reflected light. Because blood absorbs some wavelengths, changes in blood volume near the skin create a waveform that software analyzes to estimate pulse.

WHOOP describes its use of PPG this way: “PPG, or photoplethysmography, is a technique that involves measuring blood flow by assessing subtle changes in blood volume in capillaries close to your skin.” This is WHOOP author Emily Capodilupo’s explanation of the company’s technology. WHOOP says it also derives heart-rate variability (HRV) and respiratory rate from PPG data. WHOOP’s technical explanation

Fit and motion matter. WHOOP says it combines PPG with a three-axis accelerometer and three-axis gyroscope, and that strong wrist movement—for example, during weightlifting or boxing—can affect data quality. It advises snug sensor contact. A secure fit can help the sensor maintain contact, but it cannot eliminate every source of error.

How does a fitness tracker know when you’re asleep?

It does not read sleep directly from the brain. Supported Fitbit devices can automatically detect sleep when worn to bed, using extended stillness, movement, and heart-rate patterns to estimate when sleep begins and ends. Supported models also classify periods as Light, Deep, or REM sleep. Availability varies by device and region, and some features may require a premium subscription. Google says these features are for general wellness, not medical use. Google sleep help

Sleep-stage labels are estimates, not the equivalent of a clinical sleep study. Polysomnography (PSG) uses clinical sensors to measure brain and other signals, including eye, heart, and muscle activity. WHOOP says its sleep-stage models are trained against PSG data, but a model trained on clinical measurements still makes an inference when applied to wrist-sensor data. How accurate are fitness trackers for sleep, steps, and heart rate?

There is no single accuracy figure that applies to all screenless trackers. Results depend on the model and algorithm, how and where it is worn, the activity or sleep condition, and what reference method is used to check it. For that reason, a percentage from one company or one test should not be treated as a universal guarantee.

Sleep estimates

A record of a 2024 wearable sleep-tracking study reports that every evaluated device misclassified some motionless wakefulness as sleep. Oura and Fitbit did so less often than some other devices in that particular comparison; it is not a general ranking of all models. The study record also says Oura funded data collection for evaluation of its sleep-staging algorithm, while stating the company did not influence the design, analysis, interpretation, or presentation. Heart-rate estimates

A validation-study record describes commercial wrist wearables compared with ECG during rest and walking at varied intensities. Some devices performed less accurately during transitions than under steady-state conditions. The available record does not establish a single broadly applicable error figure for screenless trackers. Study record

What to make of the numbers

For everyday use, treat tracker readings as estimates and patterns as context—not as exact clinical measurements. A consistent change over time may be useful to discuss with a clinician, but a wearable result alone cannot diagnose a sleep or heart condition. Seek clinical evaluation for health concerns.
