In the United States, there is no single price for a GLP-1 weight-loss drug, and commercial insurance coverage depends on the specific plan, medicine, formulation, and prescribed use. As of October 4, 2026, some manufacturer offers list lower prices for eligible patients, while eligible Medicare Part D beneficiaries may use a temporary Medicare GLP-1 Bridge with a $50 copayment for certain products. Neither an offer nor a national program headline establishes what you personally will pay: confirm coverage and cost with your plan and prescriber.
What might you pay for a GLP-1 weight-loss drug?
Your out-of-pocket cost depends on the exact drug and formulation, your insurance benefits, and whether you qualify for a manufacturer offer. The amounts below are conditional offers or program cost-sharing—not a universal retail price or an estimate of every patient’s bill. The manufacturer terms were checked October 4, 2026 and can change.
| Product and route | Published amount | What the amount means |
|---|---|---|
| Wegovy pill, self-pay | $149 for each month of 1.5 mg and 4 mg in the listed introductory offer; the page says the 4 mg offer changes to $199 per month after April 15, 2026. | Manufacturer offer, subject to eligibility and terms; not a general cash price. Novo Nordisk’s Wegovy pill offer. |
| Wegovy pill, commercial insurance | As little as $25 per month | Conditional savings-card amount for eligible commercial-insurance users, subject to maximum savings and other terms. Government beneficiaries are excluded from the commercial savings-card terms. Novo Nordisk’s Wegovy pill offer. |
| Zepbound KwikPen, self-pay | As little as $299 for 2.5 mg; $399 for 5 mg; or $449 for 7.5 mg, 10 mg, 12.5 mg, or 15 mg | For eligible prescriptions under Lilly’s offer. Lilly defines a month as 28 days and one single-patient-use KwikPen; dose, refill timing, eligibility, taxes, or fees can affect the amount. The cited card offer expires December 31, 2026 and may be changed. Lilly’s Zepbound savings terms. |
| Zepbound single-dose pen, commercial coverage | As little as $25 for a one-, two-, or three-month fill | For eligible commercially insured patients whose plan covers the single-dose pen, subject to card limits and other terms. This is not a prediction of a particular member’s copay; Lilly describes a separate offer for some commercially insured people whose plan does not cover the listed formulation. Lilly’s Zepbound savings terms. |
| Qualifying product through Medicare GLP-1 Bridge | $50 per one-month supply | Beneficiary copayment for an eligible enrollee and covered product; it does not count toward the Part D deductible or out-of-pocket limit. CMS says participating manufacturers provide eligible products at a net price of $245 per monthly supply, which is a program price—not the beneficiary’s copayment. CMS program details. |
These figures are not directly interchangeable: a savings-card amount, self-pay offer, insurance copay, Medicare Bridge copayment, and manufacturer-to-program net price describe different costs and have different eligibility rules. The cited sources do not establish one national average patient price or a complete list-price comparison across products.

It may, but there is no single coverage rule for employer or individual commercial plans. The plan can make separate decisions about a drug, its formulation, and the condition or indication for which it is prescribed. Coverage of the same active ingredient for diabetes or another condition does not establish coverage for weight management.

Before filling a prescription, ask the insurer or pharmacy-benefit manager these questions:
- Is this exact medicine and formulation covered for my prescribed use?
- Is it on the plan formulary, and are prior authorization or step requirements required?
- What will my copay or coinsurance be at my pharmacy, and are there quantity or refill restrictions?
Manufacturer coverage tools can help identify next steps, but they are not a guarantee of coverage. Lilly specifically advises patients to check whether their plan covers the particular Zepbound formulation. See Wegovy cost and coverage information and Zepbound savings and insurance options.

What is the Medicare GLP-1 Bridge?
As of October 4, 2026, the Medicare GLP-1 Bridge is a temporary CMS demonstration, not a permanent or universal Medicare benefit. It began July 1, 2026 and is scheduled to run through December 31, 2027. It is separate from ordinary Part D coverage and is nationwide, including U.S. territories, for certain people enrolled in a Medicare drug plan. Medicare.gov’s weight-loss drug guidance and CMS’s Bridge overview describe current terms.
Who may qualify?
You must be at least 18, have qualifying Medicare drug coverage, meet one of the listed BMI and health-condition combinations, and have a provider prescribe a covered drug. Medicare.gov lists these combinations:
Rank #3
- BMI of at least 35.
- BMI of at least 30 plus diastolic heart failure, uncontrolled hypertension, or stage 3a-or-higher chronic kidney disease.
- BMI of at least 27 plus prediabetes, a prior heart attack or stroke, or symptomatic peripheral artery disease.
Qualifying drug coverage includes a standalone Part D plan, a Medicare Advantage plan with drug coverage, a Special Needs Plan, an Employer/union Group Waiver Plan, or LI NET. The provider must certify that the medication is part of a lifestyle program focused on diet and exercise. Prior authorization applies when requested. Existing receipt of a GLP-1 paid for through Part D makes a person ineligible for Bridge access under the current Medicare.gov guidance.
Which products are included?
Medicare.gov currently lists Foundayo tablets, Wegovy injection or tablets, and Zepbound KwikPen. The Bridge excludes Zepbound single-dose pens and vials. The $50 copayment is for a one-month supply, defined as 28 or 30 days depending on the drug; it does not accrue toward the Part D deductible or out-of-pocket limit.

When should you use regular Part D instead?
Some uses may be covered through ordinary Part D rules rather than the Bridge. CMS examples include Zepbound for moderate-to-severe obstructive sleep apnea in adults with obesity and Wegovy for cardiovascular risk reduction in eligible adults. CMS also identifies type 2 diabetes and certain noncirrhotic MASH indications with moderate-to-advanced fibrosis as Part D indications. Coverage still depends on the plan’s formulary and utilization rules; existing Part D coverage-determination and formulary-exception processes remain available. Medicare.gov directs people with type 2 diabetes, moderate-to-severe sleep apnea, or fatty liver disease to ask about regular Part D coverage. Check the exact indication with your plan and prescriber; do not assume Bridge eligibility means an indication is covered under regular Part D. See CMS guidance for Part D plans.
What about Medicaid?
Medicaid coverage should be checked state by state and plan by plan. CMS’s BALANCE Model involves Medicaid and Part D sponsors, with participation depending on manufacturers and states and on negotiated qualifications such as prior authorization. Participation does not guarantee that a particular person will be covered. Contact your state Medicaid agency and managed-care plan to ask about the current formulary, the weight-management indication, and any authorization requirements. CMS’s BALANCE Model information describes the model.

How to confirm your actual price before filling the prescription
- Identify the exact prescription. Ask your prescriber to confirm the drug, formulation, dose, and intended indication. A tablet, KwikPen, single-dose pen, and vial may follow different coverage or offer rules.
- Check the correct coverage route. For commercial insurance, contact the plan or pharmacy-benefit manager. For Medicare, ask whether your situation fits the GLP-1 Bridge or whether the prescribed indication should be submitted under regular Part D. For Medicaid, contact the state agency and managed-care plan.
- Ask about requirements and cost sharing. Confirm formulary status, prior authorization, step requirements, eligible pharmacies, copay or coinsurance, and refill limits. Ask whether the quoted amount is before or after any applicable deductible.
- Check offer terms separately. If considering a manufacturer offer, verify the dose, formulation, eligibility, refill timing, savings limits, expiration date, and any taxes or fees. Do not assume an offer applies to government coverage or to a different formulation.
- Get the result for your own prescription. Ask the plan or pharmacy to confirm the expected amount for the prescribed product and fill before relying on a coverage-checker estimate or a price advertised for another patient.
