Children and teens with colorectal cancer may be treated with surgery, chemotherapy, radiation, immunotherapy in selected cases, or a clinical trial—the same broad treatment categories used for adults. What differs is how rarely the disease occurs in young people, the evidence available to guide care, and the ways tumor location, stage, pathology, molecular findings, and inherited risk can shape a plan. Treatment should be directed by a team experienced in childhood and adolescent cancer.
What is different about colorectal cancer in children and teens?
The treatment categories overlap with adult care, but pediatric colorectal cancer is rare and the evidence base is much smaller than it is for adult colorectal cancer. The National Cancer Institute (NCI) says pediatric and adolescent cases are more likely to have certain tumor features and to be diagnosed after the cancer has advanced. Those are patterns across reported cases, not predictions about an individual child. Adult regimens or statistics should not be applied to a child without pediatric oncology guidance.
| Reported feature | Children and adolescents | Adults |
|---|---|---|
| Mucinous adenocarcinoma | Reported in 40%–50% of pediatric and adolescent cases in the NCI clinician summary. | About 15% of adult lesions, as reported in the same NCI summary. |
The NCI clinician summary says that most reports found 80%–90% of pediatric patients presented with Dukes stage C/D or TNM stage III/IV disease. This figure describes the reports summarized by the National Cancer Institute PDQ Pediatric Treatment Editorial Board in 2024; it does not establish the stage or outlook for a particular patient. See the NCI childhood colorectal cancer clinician summary.

There is no single pediatric colorectal cancer regimen. The oncology team considers the extent of disease and whether it can be removed, whether the tumor is in the colon or rectum/lower colon, its pathology and molecular features, inherited cancer risk, the child’s overall health, and any treatment already received. These factors help determine which options are relevant; age alone does not identify the right treatment.

What are the types of treatment for childhood colorectal cancer?
Surgery
When the cancer can be removed, surgery is central to treatment. The NCI clinician summary identifies complete surgical excision as the primary goal and an important prognostic factor. With extensive metastatic disease, complete removal may not be possible, and removing only a large portion of the tumor may offer limited benefit. The pediatric surgeon and oncology team can explain what is feasible for the tumor’s site and extent.
Chemotherapy and radiation
Chemotherapy may be used alone or in combinations. The NCI patient summary lists capecitabine, fluorouracil (5-FU), irinotecan, and oxaliplatin among drugs used for children. These are examples, not a recommended combination for every patient.

Radiation is more location-specific: the NCI clinician summary describes it with chemotherapy for rectal and lower-colon tumors, including fluorouracil and leucovorin; it also notes that other agents, including irinotecan, may be useful. The plan depends on the individual cancer and should come from the treating team.

Immunotherapy and tumor testing
Immunotherapy is an option only for selected cases. The NCI patient summary lists nivolumab and pembrolizumab and describes their use in some cancers linked to inherited syndromes or specific tumor gene changes. Testing results and the patient’s treatment history matter; neither drug is appropriate simply because a patient is young or has colorectal cancer.

The NCI clinician summary, last updated August 27, 2024, described FDA accelerated approval of nivolumab for patients aged 12 years or older with MSI-H or mismatch-repair-deficient metastatic colorectal cancer that had progressed after combination fluoropyrimidine, oxaliplatin, and irinotecan. Because eligibility and regulatory labeling can change, the treating oncologist should confirm current criteria. The NCI clinician summary explains the context; the NCI patient summary outlines treatment types in family-facing language.
Inherited-risk management
For children with certain inherited cancer syndromes, care may include discussing ways to reduce future cancer risk, such as colon surgery before cancer forms or medicines that reduce colon polyps. These approaches concern specific inherited risks and are not routine treatment for every child with colorectal cancer. A genetics professional and specialist team should guide whether they apply.

Who treats children with colorectal cancer?
“A pediatric oncologist, a doctor who specializes in treating children with cancer, oversees treatment of colorectal cancer,” according to the NCI patient summary. Depending on the case, the multidisciplinary team may also include pediatricians, pediatric gastroenterologists and surgeons, radiation oncologists, pathologists, genetic counselors, oncology nurses, social workers, rehabilitation specialists, psychologists, and child-life specialists. The NCI clinician summary recommends care at centers with multidisciplinary teams experienced in childhood and adolescent cancer.

When should families ask about a clinical trial?
Because the disease is rare in children, it is reasonable to ask the oncology team whether a clinical trial is appropriate. A match depends on factors such as age, diagnosis, prior treatment, tumor features, location, and whether the study is currently enrolling. The NCI patient summary directs families to its childhood colorectal cancer treatment information for trial-search resources, including NCI-supported trials and ClinicalTrials.gov for studies sponsored by other organizations. A trial search is a starting point, not confirmation of eligibility.
What these treatment summaries can—and cannot—tell you
The NCI PDQ summaries are evidence resources to inform care; they do not provide individualized treatment recommendations. Without details such as stage, tumor location, pathology, test results, inherited risk, and prior treatment, it is not possible to determine a child’s regimen, prognosis, immunotherapy eligibility, or clinical-trial match. The child’s pediatric oncology team is the appropriate source for those decisions.
