Doctors diagnose possible colorectal cancer in a child or teen by assessing symptoms and medical history, examining the child, and choosing tests based on the findings. Colonoscopy can show abnormalities in the colon and rectum and provide tissue for microscopic examination. If cancer is confirmed, imaging and TNM staging help determine its extent.
When symptoms lead to an evaluation
Possible symptoms include abdominal pain, constipation or diarrhea, blood in the stool, nausea or vomiting, unexplained weight loss, an abdominal lump, appetite loss, and anemia. These symptoms can also be caused by conditions other than cancer; they do not diagnose colorectal cancer on their own. The National Cancer Institute (NCI) says, “These symptoms may be caused by problems other than colorectal cancer. The only way to know is for your child to see a doctor.” (NCI, Childhood Colorectal Cancer (PDQ®)–Patient Version)
Colorectal cancer is rare in children and teens. NCI reports an annual incidence of 0.5 cases per 100,000 people younger than 20 in the United States and fewer than 100 childhood diagnoses in the country each year. These are population figures, not a way to determine the cause of an individual child’s symptoms. (NCI, Childhood Colorectal Cancer (PDQ®)–Health Professional Version)
What happens at the first appointment?
A clinician asks when symptoms began, how often they occur, and about the child’s personal and family medical history. The clinician also performs a physical examination. The results help determine which additional tests are appropriate; not every child needs every test listed below. NCI’s PDQ summaries provide information, not a personalized diagnostic plan.

Which tests may be used?
| Test or assessment | What it examines or provides | Role in evaluation |
|---|---|---|
| Colonoscopy | A view of the rectum and colon; polyps or abnormal areas can be inspected, and tissue can be collected or a polyp removed. | Allows direct examination and tissue sampling for microscopic review. |
| Stool blood test | A stool sample is checked for blood that may not be visible. | Provides a finding for the clinician to consider; a positive result alone does not diagnose cancer. |
| Blood tests | A complete blood count (CBC) measures blood-cell counts and hemoglobin/hematocrit. Carcinoembryonic antigen (CEA) measures a substance that can be released by normal and cancer cells. Liver and kidney function studies may also be included. | Can provide supporting information. CEA is not specific to cancer, and an elevated result alone does not confirm colorectal cancer. |
| Molecular testing | Examines genes, proteins, or other molecules in tissue, blood, or bone marrow. | May inform treatment planning, assessment of response, or prognosis. |
| Other procedures or imaging | Depending on the case, sources list barium enema, video-capsule endoscopy, other radiographic studies, CT of the chest, and bone scans. | May help investigate particular clinical questions; these are not routine requirements for every child. |
Colonoscopy is important because it can both reveal an abnormal area and obtain tissue. A clinician can send a sample for examination under a microscope. Stool tests, CEA, and imaging can contribute information, but none should be treated on its own as proof of cancer.
NCI also describes its Molecular Characterization Initiative as offering free testing to children, adolescents, and young adults with certain newly diagnosed cancers. Eligibility and program details can change, so families should confirm current information directly with NCI.

How diagnosis differs from staging
Diagnosis establishes whether cancer is present, including through tissue examination. Staging is a subsequent assessment of how far a confirmed cancer has spread and helps guide treatment. NCI lists CT, MRI, and PET-based imaging as possible staging studies and says colorectal cancer in children and adults is usually described using TNM staging.
NCI notes that pediatric cases have often spread to lymph nodes, beyond the bowel, or to abdominal organs by the time of diagnosis. This is population context, not a prediction about any one child; the staging results describe the individual case.

Specialist care and inherited risk
NCI says a child diagnosed with colorectal cancer will be referred to a pediatric oncologist. Because childhood colorectal cancer can be associated with inherited syndromes, the team may also discuss genetic counseling. Counseling can review the child’s and family’s history, whether gene testing may be appropriate, and what results could mean for the child and relatives. Genes discussed by NCI include APC, NF1, MUTYH, and NTHL1, among others; the sources do not establish one testing panel for every child.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. What to do if you are concerned
Arrange an assessment with the child’s healthcare professional rather than trying to interpret symptoms or a home stool test as a diagnosis. The clinician can decide whether further testing is warranted and explain what each result means in the context of the child’s history and examination.
