GLP-1 medicines imitate a gut hormone involved in blood-sugar control and appetite. By increasing fullness and slowing digestion, they can reduce food intake. In the United States, weight-management prescriptions are generally considered for people who meet the specific medicine’s age and health criteria, alongside reduced-calorie eating and increased physical activity. Meeting a BMI threshold alone does not mean a medicine is right for you.

How do GLP-1 medicines affect appetite and blood sugar?

GLP-1, or glucagon-like peptide-1, is a hormone involved in signaling after meals. GLP-1 receptor agonists mimic some of its activity: they stimulate insulin release when blood sugar is high and reduce glucagon secretion, which helps lower blood glucose. They also slow digestion and increase feelings of fullness, which can reduce food intake, as the World Health Organization explains.

These effects can support weight management, but they do not guarantee a particular amount or speed of weight loss. Individual outcomes vary, and a medicine’s expected benefits and risks depend on the specific product and the person taking it.

Tirzepatide acts on two hormone receptors

Tirzepatide, the active ingredient in Zepbound, is not a GLP-1-only drug. It activates receptors for both GLP-1 and glucose-dependent insulinotropic polypeptide (GIP), hormones secreted from the intestine. The FDA says this activity reduces appetite and food intake in its Zepbound approval announcement.

Who may be prescribed a medicine for weight management?

In the United States, eligibility depends on the medicine’s approved indication and a clinician’s assessment. FDA-labeled weight-management medicines are generally used with reduced-calorie eating and increased physical activity. The criteria below describe two products; they are not a complete list of all medicines or all of their approved uses.

Zepbound (tirzepatide) Adults with a BMI of at least 30 kg/m², or at least 27 kg/m² with one or more weight-related conditions, such as hypertension, type 2 diabetes or high cholesterol. FDA approval announcement, dated 8 November 2023.
Wegovy (semaglutide) Adults with obesity; adults with overweight and at least one weight-related comorbidity; and pediatric patients aged 12 years and older with obesity. FDA-hosted prescribing information, retrieved label revision. The label also includes additional indications, including certain cardiovascular and liver-disease indications.

A BMI category or diagnosis does not establish that treatment is suitable. A prescriber considers the exact product indication, age, medical history, other medicines, treatment goals and product-specific safety information. Brand names and active ingredients are not interchangeable: for example, Ozempic and Wegovy both contain semaglutide, but their labels and indications differ.

WHO guidance is global, not a substitute for local prescribing rules

In a December 2025 global guideline, WHO conditionally recommended GLP-1 therapies as a long-term treatment option for adults living with obesity, excluding pregnant women. It emphasizes individual medical history and clinical indications. This global recommendation does not replace a country’s product approvals, prescribing information or clinician evaluation.

What treatment involves beyond the prescription

Obesity is a chronic, relapsing disease, and WHO frames these medicines as potentially long-term treatment rather than a short course. WHO also conditionally recommends that intensive behavioral support—including structured healthy-eating and physical-activity support—may be offered alongside medication. The recommendation is conditional partly because long-term evidence on efficacy, safety, maintenance and discontinuation remains limited, and because cost, health-system readiness and equitable access are concerns.

For adults with overweight or obesity and type 2 diabetes, the American Diabetes Association’s 2025 standards prefer a GLP-1 receptor agonist or a dual GIP/GLP-1 agonist for many patients, considering both weight-reduction and glucose-lowering efficacy. That guidance applies to this particular clinical context; it is not a universal recommendation for everyone seeking weight loss.

Questions to weigh with a prescriber

  • Does the exact medicine’s approved indication cover your age and health circumstances?
  • Which outcome matters for your care—weight management, blood-sugar control or another product-specific indication?
  • What side effects, warnings and contraindications apply to this medicine and your medical history?
  • How will nutrition, activity, follow-up, cost and coverage fit into the treatment plan?
  • What should you do if you are pregnant, may become pregnant, or take medicines that could affect treatment?

Side effects, warnings and safer use

Nausea, vomiting, constipation and diarrhea are among the common side effects WHO lists for GLP-1 therapies. The frequency and seriousness of possible effects vary by medicine, dose, health history and other medicines. The Wegovy prescribing information includes product-specific adverse reactions, warnings and precautions; review the current label for the medicine being considered with a qualified prescriber.

Do not start a prescription medicine based only on BMI, use another person’s medication, or combine products on your own. The Wegovy label says concomitant use with other semaglutide-containing products or another GLP-1 receptor agonist is not recommended. Pregnancy-related instructions and all contraindications should be checked in the current label for the exact product.

What the evidence can—and cannot—tell you about results

In the chronic-weight-management trials described in its 8 November 2023 announcement, the FDA reported statistically significant body-weight reduction versus placebo after 72 weeks at all three studied Zepbound doses; greater proportions of treated participants achieved at least 5% weight reduction. Those are trial findings, not a forecast for an individual patient. They do not establish how much weight any one person will lose or how quickly.
