GLP-1 weight-loss medicines imitate signals involved in appetite and food intake, helping some people feel less hungry or full sooner. They are prescription treatments used alongside nutrition and physical-activity measures—not a guaranteed amount of weight loss. What you experience, whether a medicine is appropriate, and how its dose is managed depend on your health and the specific product.

How do GLP-1 weight-loss drugs work?

GLP-1 is a hormone involved in appetite and food intake. Medicines called GLP-1 receptor agonists mimic some of its signaling, including signals to brain areas that regulate appetite. A person may feel less hungry or feel full sooner. These medicines do not all work in exactly the same way, and an individual response cannot be predicted from the mechanism alone.

For example, semaglutide mimics GLP-1. Tirzepatide acts on both GIP and GLP-1 receptors; the FDA describes its effects as involving appetite and food intake as well. NIDDK explains how prescription weight-management medicines work, and the FDA’s Zepbound announcement describes tirzepatide’s receptor activity.

These medicines are intended to be part of a weight-management plan that includes nutrition and physical activity. The National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) reports that adults using prescription weight-management medicines as part of a lifestyle program lost, on average, 3% to 12% more of their starting body weight after one year than people in a lifestyle program without medication. That is a broad average across weight-management medicines, not a GLP-1-specific comparison or a prediction for any one person.

Who may be considered for treatment?

In the United States, NIDDK says a health professional may consider prescription weight-management medicine for an adult with a body mass index (BMI) of 30 or higher, or a BMI of 27 or higher with a weight-related health problem, such as high blood pressure or type 2 diabetes. BMI is one part of the decision: having a high BMI does not mean that medication is right for everyone.

Product indications are specific to each medicine and may change. The FDA’s Zepbound announcement describes use for adults with obesity or overweight and a weight-related condition, alongside reduced-calorie eating and increased physical activity. A clinician can assess your health context and check the current indication for a particular product.

What should I expect during treatment?

A plan tied to the specific medicine

Your prescriber should explain the product, formulation, dose schedule, follow-up, and what to do if a dose or side effect raises a concern. Ask how the dose is meant to change over time and whom to contact with questions. Do not copy a dosing schedule from an online post or adjust a dose on your own.

Possible digestive effects and other symptoms

Commonly reported effects differ by product, but can include nausea, diarrhea, vomiting, constipation, abdominal discomfort or pain, and fatigue. The Wegovy label also lists headache and indigestion (dyspepsia) among common adverse reactions; the FDA’s Zepbound announcement also lists reflux. Not everyone experiences these effects, and their severity and tolerability vary.

Medication review and follow-up

Tell your prescriber and pharmacist about all medicines you take and relevant health conditions. The Wegovy label warns that low blood sugar may be a concern when it is used with insulin or insulin secretagogues, and that delayed stomach emptying may affect oral medicines. Ask whether either issue matters for your treatment and what monitoring or medication changes, if any, are appropriate.

Important safety cautions

Wegovy’s thyroid C-cell tumor warning

The Wegovy prescribing information has a boxed warning: semaglutide caused thyroid C-cell tumors in rodents, but it is unknown whether it causes them in humans. Wegovy is contraindicated for people with a personal or family history of medullary thyroid carcinoma or for people with multiple endocrine neoplasia syndrome type 2 (MEN 2). This is product-specific label language; do not assume the same warning or contraindications apply to every medicine without checking its current label.

Compounded semaglutide and dosing errors

The FDA has warned about dosing errors with compounded injectable semaglutide, including errors involving conversion between milligrams and syringe units, different concentrations, and multidose vials. Use only the exact product and instructions supplied through your care team and confirm any uncertainty with a clinician or pharmacist. Do not convert units or measure, compound, or adjust a dose based on online instructions. The FDA page explains how to report medication errors and adverse events associated with compounded semaglutide.

Check current safety information

Safety communications and product labels can change. The FDA reported that its review found no increased risk of suicidal ideation or behavior with GLP-1 receptor agonists and requested removal of that warning from affected labeling, including for Saxenda, Wegovy, and Zepbound. Because label changes may be implemented at different times, consult the FDA safety communication and the current label for the specific medicine rather than relying on older warnings or summaries.
