Fitness trackers estimate steps, sleep, and heart rate by interpreting sensor signals with software; they do not directly count every footfall, measure brain activity, or observe blood pumping like clinical instruments. Their accuracy depends on the metric, device, fit, activity, and setting—so a useful reading is a trend or estimate, not automatically a medical measurement.
How a tracker turns sensor data into numbers
A wearable collects signals—such as movement or changes in reflected light—and applies algorithms to classify or estimate what is happening. The process is indirect: the device recognizes patterns associated with walking, pulse, or sleep, rather than directly observing those events. A tracker can therefore perform differently across metrics, and results from a controlled study may not predict what it records during an ordinary day.
How fitness trackers estimate steps
Wrist trackers use motion sensors to identify patterns that look like walking or running. Fitbit, for example, says its step-counting algorithm looks for motion and intensity patterns indicative of those activities. That description is specific to Fitbit’s guidance; brands do not necessarily use identical algorithms. Fitbit’s accuracy guidance also describes how wear location and wrist preference can affect sensitivity. Height may be used to estimate stride length for distance, which is different from counting steps.
Wrist placement explains some everyday mismatches. Walking while pushing a stroller can produce fewer recognizable arm movements and be undercounted, while arm movements during cooking or desk work can sometimes be mistaken for steps. The tracker’s number is thus a classification from movement patterns, not a direct count of every time a foot touches the ground.

A 2020 systematic review covering 158 publications on nine wearable brands found that step-count validity varied by brand. In controlled studies, Apple and Samsung had higher validity and less variability than several other brands in the reviewed evidence; study counts and device placement affect such comparisons. Everyday results were less consistent: among 69 free-living comparisons with calculable group error, 42% were within ±10% of the reference count, 17% were more than 10% low, and 41% were more than 10% high. The comparisons showed slight average overestimation (mean error 5%; median error 6%). These are pooled historical study results, not a forecast for a particular current tracker or person. Fuller et al.’s systematic review explains the evidence and its limits.
How wrist trackers estimate heart rate
Many wrist devices use photoplethysmography (PPG). LEDs shine light into tissue, and an optical sensor detects changes in reflected light associated with pulsatile changes in blood volume near the skin. Software processes that signal into a heart-rate estimate. This is not the same as measuring the heart’s electrical activity.
The optical signal can be harder to interpret when the wrist is moving, the device is loose or compressed, or conditions such as sweat and skin temperature change. Individual differences can also affect the signal. These limitations are described in a review focused on Garmin activity trackers, but they are relevant to understanding the general PPG method. The review discusses the mechanism and evidence.

Outbyte Driver Updater FREEFix the driver behind crashes, sound loss and screen glitchesFind Drivers →Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →Heart-rate accuracy depends on context. In the 2020 multi-brand systematic review, 56.5% of controlled-setting comparisons with calculable group error were within ±3% of the reference; 24.9% were more than 3% low and 18.6% more than 3% high. The median error across comparisons was −1%. The same review reported results within ±3% in 71% of Apple Watch comparisons, 51% of Fitbit comparisons, and 49% of Garmin comparisons. Those figures describe the studies and models included at that time, not a current brand ranking. The Garmin-focused review found heart-rate validity generally better at rest than during activity, with readings degrading alongside arm movement and rising intensity in the studies it examined.

How trackers estimate sleep and sleep stages
Trackers infer sleep from movement and physiological signals; they do not measure brain activity as a sleep laboratory does. The Garmin review describes devices using accelerometer and heart-rate data, with newer models also using heart-rate variability. The precise current algorithms used across brands are not established by the available evidence here.
Quiet wakefulness can look like sleep to a movement-based system. The Garmin review notes a broader tendency for activity trackers to overestimate sleep time and sleep efficiency when this happens, and to underestimate wake after sleep onset. This is a reported limitation, not a guaranteed result for every current device.
Sleep-stage labels require particular caution. The Garmin review found only two studies validating Garmin sleep estimates, and neither used polysomnography—the reference method used in sleep laboratories. That small, variable evidence base does not establish the accuracy of current sleep-stage algorithms across brands. Treat stage labels as algorithmic estimates rather than clinical sleep-study results.

A disagreement does not necessarily mean one device is defective. The devices may use different sensors, algorithms, thresholds, or reference assumptions; they may also be worn in different positions or compared during different activities. For sleep, the comparison method matters too: a diary and polysomnography are not equivalent reference standards.

When comparing trackers, match the conditions as closely as possible:
- Compare the same metric: a device’s step performance does not establish its heart-rate or sleep accuracy.
- Use the same kind of reference: distinguish controlled or laboratory comparisons from free-living measurements.
- Match the circumstances: consider device generation, wearing position and fit, activity type, and intensity.
- Check the sleep reference: note whether a study used a diary or polysomnography.
The 2020 multi-brand review did not identify one brand as best across all measures. Its findings also show why controlled results should not be treated as a guarantee of everyday accuracy: free-living step-count errors were more widely distributed.
What to trust—and what not to infer
Use tracker readings primarily as estimates, especially when watching patterns over time. A step total may be useful for tracking activity, and a heart-rate trend may help you observe how your readings change, but a number on a consumer wearable is not automatically a clinical measurement. Garmin states that its wearables are tools intended to encourage an active and healthy lifestyle, that their metrics are close estimates that may not be precisely accurate, and that they are not intended for medical purposes. Garmin’s accuracy disclaimer sets out that qualification.

Accuracy is metric-specific: evidence for steps cannot be assumed to validate sleep or heart rate. In particular, the cited sleep evidence is sparse, and sleep-stage labels should not be treated as diagnoses or substitutes for a clinical assessment.
