After a child has a heart valve replaced, doctors monitor the valve and the heart with pediatric cardiology visits and echocardiograms. The plan is tailored to the child: valve type and position, heart function, other heart conditions, test findings, and symptoms all matter. A mechanical valve may also require ongoing anticoagulation management.
What doctors check at follow-up
Clinic visits and examination
The cardiology team asks about the child’s health and symptoms and examines the child. New symptoms or examination findings can change when the next assessment is needed.
Echocardiography
Transthoracic echocardiography (TTE) is an ultrasound test performed from the chest and is usually the first imaging test. It evaluates blood flow through the prosthetic valve and how the heart is functioning. A post-procedure TTE establishes a baseline; clinicians compare later studies with that child’s own baseline to assess for change.

The 2020 ACC/AHA guideline states: “A baseline TTE study is recommended after all valve interventions, including replacement with a prosthetic valve.” It says the baseline is ideally obtained 1 to 3 months after the intervention, although the treating team determines timing for the individual child. 2020 ACC/AHA Guideline for the Management of Patients With Valvular Heart Disease.

Why there is no single follow-up calendar
Follow-up frequency depends on the prosthesis and its position, time since surgery, valve function, ventricular size and function, other cardiac conditions, and whether the child has symptoms. Pediatric congenital-heart-disease imaging criteria consider different scenarios rather than prescribing one interval for every child. For example, they rate annual TTE surveillance as appropriate for a child with normal prosthetic mitral-valve function and no left-ventricular dysfunction; other scenarios use different intervals depending on severity, ventricular function, or symptoms. Pediatric congenital-heart-disease appropriate-use criteria.
The ACC/AHA guideline describes annual clinical follow-up after valve intervention, but its prosthetic-valve recommendations are general guidance and should not be treated as a universal pediatric echocardiogram schedule. Pediatric imaging criteria and the child’s anatomy and clinical course inform the cardiologist’s plan. TTE should be repeated sooner if symptoms develop or examination findings change.

When doctors may use other imaging
Additional tests are selected to investigate a concern; they are not a routine package for every child. Depending on the suspected problem and what TTE shows, clinicians may use:
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Repair Windows errors before they cause bigger problems3Fix the driver behind crashes, sound loss and screen glitches- Transesophageal echocardiography (TEE): may supplement TTE when prosthetic mitral-valve dysfunction is a concern.
- CT, fluoroscopy, cardiac MRI (CMR), or PET: may be considered when valve dysfunction or endocarditis is suspected.
If prosthetic-valve thrombosis is suspected, echocardiography is part of the evaluation. Fluoroscopy or TEE may be considered when TTE does not resolve the question. The pediatric AHA statement discusses these diagnostic considerations: Prevention and Treatment of Thrombosis in Pediatric and Congenital Heart Disease.

Anticoagulation is a separate part of monitoring
Mechanical valves generally require anticoagulation management. For children taking warfarin, INR is a blood-test measure used to guide treatment, but an individual child’s target and testing schedule must come from the treating cardiologist and anticoagulation team. Valve type, position, and clinical risks affect the plan; pediatric evidence is limited, and the AHA statement is from 2013. Recommendations should not be converted into a one-size-fits-all regimen.
Anticoagulation considerations differ between mechanical and bioprosthetic valves and among valve positions, including aortic, mitral, pulmonary, and tricuspid. Do not stop or change a child’s anticoagulant based on general information; contact the care team for medication questions.

- Simple to Use Without a Subscription: No Bluetooth, Wi-Fi, cords or PC needed. Place the device near your smartphone. Monitor your heart by placing your fingers or thumbs on the silver KardiaMobile EKG sensors. Know in 30 seconds whether your heart rhythm is normal.
What families should do if something changes
Contact the child’s cardiology team if symptoms appear or the child’s condition changes; clinicians may advise an earlier examination or TTE. Follow the individualized action plan from the care team, and seek urgent medical care for severe or rapidly worsening symptoms. No single symptom checklist applies to every child’s operation and congenital anatomy, and absence of a particular symptom does not establish that a valve is functioning normally.
