Doctors assess stroke risk by reviewing multiple health and lifestyle factors—not by relying on one number—and investigate cognitive changes through medical history, examination, and, when appropriate, cognitive testing. A risk checklist can guide a prevention discussion, but it is not a diagnosis. Sudden possible stroke symptoms are different: call 911 immediately.
What a stroke-risk assessment considers
The American Stroke Association’s Stroke Risk Assessment, last reviewed May 1, 2026, is a patient-facing checklist for discussing risk with a healthcare professional. It asks about several factors rather than treating a single reading as a complete assessment:
- Blood pressure, including whether it is above the checklist’s stated threshold of 120/80 mmHg
- Atrial fibrillation and blood sugar
- Body mass index (BMI), cholesterol, and diabetes
- Diet and physical activity
- Personal or family history of stroke, transient ischemic attack (TIA), or heart attack
- Tobacco use or vaping
For the listed items, the checklist treats “yes” or “unknown” as a reason to regard that factor as higher risk. It also identifies factors that cannot be changed, including age, family history, race, gender, and prior stroke. These prompts help identify topics to discuss; they are not a universal clinical scoring instrument and do not determine an individual’s diagnosis or treatment.
If you already monitor your blood pressure at home, bring your readings to your clinician. The checklist does not say that buying a monitor is required, and a home reading alone cannot establish overall stroke risk.

How prevention guidance fits in
The American Heart Association and American Stroke Association’s 2024 primary prevention guideline summary covers screening and management of risk factors, social determinants of health, blood pressure management, physical activity, and dietary guidance. Its scope is prevention for people who have not previously had a stroke. The checklist and guideline can inform a clinician conversation, but neither supplies personal treatment instructions by itself.
How doctors investigate cognitive changes after stroke
Changes in thinking can affect attention, memory, and everyday reasoning, even when a person has no obvious movement or communication difficulty. Possible problems include confusion, trouble processing information, difficulty planning or following conversations, and changes in reasoning or judgment. The American Stroke Association notes that damage to particular brain areas can affect cognition even without motor or communication problems.

The Association’s patient guidance, “Cognitive Challenges Post-Stroke,” describes medical history followed by physical and neurological examination as the first diagnostic steps. In its words: “Medical history assessment, followed by physical and neurological examination of the stroke survivor are the first steps in proper diagnosis.”
Screening and more detailed assessment
A clinician may use a brief cognitive screen soon after a stroke to flag areas that need closer evaluation. The Montreal Cognitive Assessment (MoCA) is one example cited by the American Stroke Association. A screening result is not, by itself, a diagnosis.

When more detail is needed, a neuropsychological assessment can examine cognitive abilities more comprehensively. The American Heart Association/American Stroke Association’s 2023 scientific statement summary says tailored neuropsychological evaluations can improve diagnostic accuracy and describe a person’s pattern of strengths and difficulties. History and neurological examination help put any test findings in context.

There is no one test or schedule established for everyone in the cited guidance. The appropriate approach depends on the person’s situation and the questions the clinician needs to answer.

Why clinicians consider other explanations
A cognitive change after stroke should not automatically be attributed to the stroke alone. Assessment may take account of prior cognitive ability, age-related brain conditions, and other medical issues or complications that can affect thinking. The 2023 AHA/ASA statement summary identifies possible contributors including metabolic abnormalities, medication side effects, infection, delirium, sleep disorders, hearing or vision impairment, and depression. Clinicians interpret results alongside these factors rather than treating a screening score as a complete explanation.

- Sudden weakness or numbness, especially on one side of the body
- Sudden confusion or trouble speaking or understanding speech
- Sudden vision or balance changes
- A severe headache with no known cause
How common are cognitive effects after stroke?
The 2023 AHA/ASA scientific statement summary reports that post-stroke cognitive impairment occurs in up to 60% of stroke survivors in the first year after stroke. This is a population-level estimate, not a prediction of any one person’s outcome.
