In the United States, pilot fitness is addressed through recurring FAA medical certification, an ongoing duty to avoid flying when medically unfit, and additional screening or support that individual airlines and unions may provide. These are separate layers: airline or peer-support programs do not grant FAA medical eligibility, and a valid medical certificate does not mean a pilot should fly through a new or worsening impairment.
How the U.S. pilot fitness system works
The available authoritative guidance here is U.S.-specific. It does not establish how every country regulates pilots or how any particular airline currently screens its crews.

What happens during FAA medical certification?
The process starts with an application through FAA MedXPress and an examination by an AME. Medical exams recur at intervals ranging from six months to five years, depending on the type of flying and the pilot’s age, according to the FAA. The AME reviews the applicant’s medical history and disclosures, performs the examination, and may request more information or testing. Depending on the case, the AME can issue a certificate, defer the application for FAA review, or deny it in limited circumstances involving a specifically disqualifying condition. Applicants report relevant medical history, medications, and visits to health professionals during the preceding three years. The psychiatric portion of the exam is a general impression of cognition and mental wellness based on observation and general questioning; it is not described by the FAA as a universal, stand-alone psychological test. People with current or past conditions, or who take medication, should consult the current AME Guide and bring condition-specific documentation. A recent, detailed clinical progress note may be needed in some cases. The FAA can also request further records, psychiatric or psychological evaluations, or other assessment; it may seek additional medical information after an incident that appears medically related. Requirements depend on the circumstances and the applicable certification pathway.
Do pilots receive mental-health screenings?
There is a mental-health component to the FAA medical examination, and the FAA says commercial airlines often have their own mental-health screenings and requirements. The available guidance does not identify which airlines currently use which methods or how each carrier’s process interacts with FAA certification. Airline procedures should therefore not be treated as a single, uniform system.

FAA guidance says an AME may ask follow-up questions or request documentation. Depending on the condition and case, further evaluation can include psychological testing or FAA review. The FAA explains that psychiatric and psychological evaluations may both be required because a condition and its treatment can affect safe performance, and the professionals involved have different expertise. No—not automatically. The FAA encourages pilots to seek help and states that most treated mental-health conditions do not disqualify a pilot. Some conditions are disqualifying, while others require documentation, assessment, or FAA review. The same caution applies to antidepressants. Whether a pilot can fly while taking a particular medication depends on the condition, medication, and certification pathway. A general yes or no would be misleading: pilots should check the current FAA Guide and consult an AME about their specific circumstances.
The FAA’s Pilot Mental Fitness page also reports that “about 0.1%” of medical certificate applicants who disclose health issues are denied. What support can airlines and unions offer?
FAA recommendations encourage airlines and unions to organize peer-support programs. Such programs can provide a route to assistance with personal, psychological, physiological, or medical challenges, distinct from formal certification decisions. The FAA Pilot Fitness Aviation Rulemaking Committee’s 2015 report cited Delta’s Pilot Assistance Network and American Airlines’ Project Wingman as examples. That historical reference does not verify that either program remains available, or establish its current eligibility, confidentiality rules, or terms. Support programs and certification have different roles. A peer program may help someone seek assistance, but it does not issue an FAA medical certificate or determine FAA eligibility. The report’s examples should not be taken as evidence of current carrier policy or of a program’s authority to approve a pilot’s return to duty.

Why fitness to fly remains an ongoing responsibility
Medical certification is a recurring checkpoint, not permission to fly despite a change in health. FAA guidance says a pilot must not perform crew duties when a known or worsening medical condition makes the pilot unable to meet applicable medical standards. For a personal question about symptoms, treatment, medication, or certification, the appropriate next step is to consult an AME and current FAA guidance rather than infer eligibility from a general description or another pilot’s outcome.
