For confirmed female pattern hair loss, topical minoxidil is generally the first treatment to discuss: it is FDA-approved for women’s hair loss in the United States and is the first-line option in a 2019 multidisciplinary clinical report. Prescription medicines and procedures may suit selected patients, but they have different evidence, risks, and maintenance demands. First, make sure the thinning is actually female pattern hair loss—other causes need different treatment.

Start by identifying the cause of the hair loss

Gradual widening of the part or thinning over the crown can be consistent with female pattern hair loss, but not all hair loss follows that pattern. Shedding, patchy loss, or other changes may have a different cause. The American Academy of Dermatology (AAD) emphasizes that treatment depends on the diagnosis.

If the pattern is atypical or the cause is unclear, seek an evaluation from a clinician, such as a dermatologist. A diagnosis helps avoid spending time on a treatment that does not address the reason for the loss. The AAD’s patient guidance, “Thinning hair and hair loss: Could it be female pattern hair loss?” and “Hair loss: Diagnosis and treatment,” was retrieved October 4, 2026.

How the main options compare

Option How it is used Evidence and role Ongoing commitment and key cautions
Topical minoxidil Applied to the scalp; AAD describes 2% solution and 5% foam options for women in the United States. FDA-approved for women’s hair loss in the United States. A 2019 multidisciplinary report calls it an effective first-line treatment for female pattern hair loss. Continued use is needed to maintain benefit. AAD advises avoiding it during pregnancy, when planning pregnancy, and while breastfeeding. Follow the current product label and clinician guidance.
Prescription medicines, including spironolactone or finasteride Selected by a clinician; these are prescription approaches, not over-the-counter substitutes for minoxidil. Hair-loss use is off-label in the United States. The 2019 report discusses them as possible additions in selected cases, including severe loss or hyperandrogenism; evidence is uneven. Potential adverse effects and reproductive safety need individual discussion. The report recommends secure contraception for women of reproductive age taking antiandrogens or 5-alpha-reductase inhibitors because of teratogenic risk.
Low-level laser therapy Device-based treatment, generally involving repeated sessions. AAD describes it as a possible option; the 2019 report characterizes benefit as modest and evidence as limited. Repeated sessions over many months may be needed. Discuss suitability and practical commitment with a clinician.
Platelet-rich plasma (PRP) A clinician processes a blood sample and injects the resulting preparation into the scalp. AAD describes it as an option; the 2019 report calls it promising but limited by evidence. Repeat visits are involved. Discuss expected benefit, burden, and suitability before proceeding.
Hair transplantation Surgical transfer of hair from a donor area to areas affected by loss. May be considered for selected patients. Diffuse sparse hair may not leave enough healthy donor hair; the 2019 report says transplantation should be paired with continued medical therapy. Requires clinician assessment and surgery. The extent and pattern of loss affect whether there is suitable donor hair.

The comparison is not a head-to-head ranking: the options have different evidence bases and outcomes, and not every option is suitable for every diagnosis or patient. The 2019 clinical recommendations are from the multidisciplinary Androgen Excess and PCOS Committee report on female pattern hair loss.

Topical minoxidil: the usual starting point for female pattern hair loss

In the United States, AAD identifies topical minoxidil as an FDA-approved treatment for women’s hair loss and discusses 2% solution and 5% foam. The committee report states: “Topical minoxidil is an effective treatment of FPHL and should be used as the first-line treatment (level of evidence B).” That recommendation applies to female pattern hair loss; it does not mean minoxidil treats every cause of thinning.

Minoxidil is a maintenance treatment, not a one-time course. AAD says stopping can lead to loss of hair maintained or regrown during treatment. Women who are pregnant, planning pregnancy, or breastfeeding should avoid it, according to AAD guidance. Check the current product label and discuss personal circumstances with a clinician rather than assuming a particular strength or schedule is right for you.

Prescription medicines: selected, off-label treatment with safety trade-offs

The 2019 multidisciplinary report describes spironolactone and 5-alpha-reductase inhibitors such as finasteride as possible additions for selected cases, particularly severe loss or hyperandrogenism. These are not interchangeable with an over-the-counter scalp treatment, and their use for hair loss is off-label in the United States. The report notes that much of the evidence comes from postmenopausal women, so it should not be read as establishing the same benefit for every patient.

Reproductive plans are important to the decision. The report recommends secure contraception during antiandrogen or 5-alpha-reductase inhibitor treatment for women of reproductive age because of teratogenic risk. A clinician should weigh potential adverse effects and decide what monitoring is appropriate for the individual; there is no universal regimen in the cited guidance.

A specific caution about compounded topical finasteride

Compounded topical finasteride deserves separate caution from general discussion of prescription medicines. In a 2025 safety alert, the U.S. Food and Drug Administration (FDA) said absorption through skin is expected and highlighted possible exposure through handling or transfer to women. The agency reported 32 cases to its adverse-event reporting system between 2019 and 2024. That is a surveillance report count, not an incidence rate or proof that the product caused each reported event. The FDA also said reported effects were consistent with those known for approved oral finasteride products.

Procedures and devices: consider suitability as well as evidence

Low-level laser therapy

Low-level laser therapy is a device-based option rather than a medicine. AAD notes that repeated sessions over many months may be needed, while the 2019 report describes the potential benefit as modest and the evidence as limited. Ask the clinician recommending it what course is proposed and how progress would be assessed.

Platelet-rich plasma

PRP involves processing a sample of the patient’s blood and injecting it into the scalp, with repeat visits described by AAD. The committee report calls it promising but notes the evidence limitation. It is an office-based procedure, not a proven substitute for diagnosis or a guaranteed route to regrowth.

Hair transplantation

Transplantation may be an option for some patients, but the donor area matters: diffuse, sparse hair may not provide enough healthy hair to move. The committee report advises pairing transplantation with ongoing medical therapy. A surgical consultation should address donor suitability, likely goals, and the continuing treatment plan.

For these options, visit frequency, invasiveness, suitability, and cost differ. Costs vary, and the clinical sources cited here do not establish comparable prices.

How to choose a next step

  1. Clarify the diagnosis. If the cause or pattern is uncertain, arrange a clinical evaluation before choosing a treatment.
  2. For confirmed female pattern hair loss, discuss topical minoxidil first. Review product options, label directions, pregnancy-related cautions, and the need for ongoing use.
  3. Ask whether a prescription medicine fits your situation. Discuss why it is being considered, its off-label status for hair loss in the United States, potential adverse effects, reproductive safety, and any monitoring.
  4. Evaluate procedures on their own terms. Ask about evidence, suitability, visit or surgery burden, realistic goals, and whether medical treatment should continue.
  5. Be especially cautious with compounded topical finasteride. Review the FDA’s safety concerns with a clinician, including the possibility of skin absorption and transfer to others.
