A false positive reports a defect when the tested software has none; a false negative misses a defect that is present. In a conventional test runner, that means a red result is not automatically proof the application is broken, and a green result is not proof it is defect-free. The reference point is whether the software’s actual behavior matches the intended behavior or specification—not simply whether the command returned red or green.
What the terms mean in software testing
The ISTQB glossary defines a false-positive result as reporting a defect that does not exist in the test object, and a false-negative result as failing to identify a defect that is present. These terms depend on what counts as a “positive” result: here, a positive means that the test reports a defect.

A test result is evidence about the checks that ran under particular conditions. A red test can arise from a defect, a mistaken expectation, a faulty test, bad fixtures, or an environmental problem. A green run establishes only that the executed assertions passed in that run; it cannot establish that untested behaviors are correct.
The FDA-hosted software terminology glossary dates to August 1995. It is a historical terminology resource, not current regulatory guidance. ISO/IEC/IEEE 29119-1:2022 is titled “Software and systems engineering — Software testing — Part 1: General concepts”; ISO describes Part 1 as informative and Parts 2–4 as normative for organizations claiming conformance. Citing the overview does not certify a test suite or a team.

How false positives happen: flaky and brittle tests
A flaky test sometimes passes and sometimes fails without an intentional, relevant change to the code or its expected behavior. A failure on unchanged code may be a false alarm, but intermittency alone does not prove that the test is wrong: an intermittent production defect can also be real. The distinction requires investigating what changed, what conditions the run experienced, and what the specification requires.
pytest’s documentation warns that unreliable failure signals can erode trust, cause teams to overlook real failures, and consume time in reruns and investigations. Potential causes include:
- Uncontrolled system state, shared global state, or incomplete setup and cleanup.
- Dependencies on test ordering or parallel execution.
- Timing assertions that are too strict for the environment.
- Floating-point comparisons that require exact equality where approximation is appropriate.
- Unstable external services, resources, or inputs.
Improve isolation and cleanup, make timing and numeric checks fit the behavior being tested, and reproduce failures with logs and inputs preserved. Randomizing test order can help expose hidden order dependencies. Rerun or replay tools can help assess intermittency, but a rerun is a diagnostic aid, not an explanation.

Terminology can differ across teams. Chromium’s CQ documentation uses “false negative” in its own local terminology for a flaky failure that should have passed. That usage describes Chromium’s retry-policy discussion; it is not the convention used here, where a false negative means that a test misses a defect.

A suite can miss a defect when it never checks the relevant behavior, checks only a superficial proxy, or cannot distinguish correct from incorrect outcomes. For example, checking that an HTTP response arrived does not test that its status, contents, or side effects are correct. A test that asserts only one typical input may miss boundary conditions, error paths, or interactions between components.

- Does the test check the actual result, not merely that execution completed?
- Does it cover important boundaries, invalid inputs, and failure handling?
- Would it fail if the suspected defect were introduced?
- Are the assertions specific enough to catch a wrong result but not so brittle that irrelevant variation triggers failure?
Mutation testing probes some of these gaps by making deliberate, small changes to code and checking whether tests detect them. Microsoft Learn’s .NET guidance describes Stryker.NET: a mutant is “killed” when the tests catch the change and “survives” when they do not. A survivor is a reason to review whether the tests lack coverage or have weak assertions—not automatic proof that the production code is defective. Some changes are equivalent with respect to observable behavior, and mutation operators sample only some possible faults.
Google’s Testing Blog cautions that tests added to kill mutants must themselves be valuable. Mutation score is therefore not the probability that the suite will detect every real defect. Use mutation testing selectively on high-risk or business-critical code, and evaluate whether the resulting tests protect meaningful behavior rather than chasing a perfect score.

There is no universal answer. The consequences depend on what the test controls and what happens when it is wrong; no general, comparable statistic establishes that false positives or false negatives are always costlier.

- Impact: What is the likely harm if a defect ships, compared with the cost of blocking an innocent change?
- Decision point: Is the test an early local feedback aid, a merge gate, or a release or safety gate?
- Likelihood and other detection: How likely is this class of defect, and will other checks or production monitoring catch it?
- Investigation cost: How much time does a noisy failure consume, and how quickly can the team reproduce it?
- Recovery: Can a shipped defect be rolled back or detected downstream, or would its effects be hard to reverse?
For a low-risk local check, a false alarm may mainly cost developer time. For a release gate protecting a critical behavior, a missed defect may carry greater consequences. The team should set the test’s strictness, review process, and escalation path according to the decision it supports.

- Preserve the evidence. Keep the initial logs, inputs, environment details, test order, and relevant build information. Check whether code, environment, inputs, or ordering actually stayed constant.
- Reproduce and assess intermittency. Rerun or replay the failing case to learn whether it is reproducible. Record the original failure; a later pass does not explain it or erase it.
- Inspect likely sources of instability. Review shared state, setup and cleanup, timing assumptions, external dependencies, parallelism, and whether assertions are too strict.
- Compare behavior with the specification. If the failure is deterministic, determine whether the implementation, test, or expectation conflicts with the required behavior. Fix the one that is wrong rather than automatically weakening the assertion.
- Probe for missed defects. Identify untested behavior or boundary conditions, add targeted assertions, and consider mutation testing where it can reveal whether meaningful code changes are detected.
- Quarantine only with ownership. If temporary quarantine is necessary to unblock work, assign someone to investigate and track a follow-up. pytest warns that permanent non-strict expected-failure quarantine is dangerous because it can make failures invisible.
Retries can reduce disruption from intermittent failures, but they can also allow a flaky test to pass without resolving its cause. Chromium’s CQ documentation describes that tradeoff in its own retry policy: retries can make flaky tests more likely to land while reducing disruption to unrelated changes. Treat retries as controlled handling, not as evidence that the test is reliable.
