Current evidence does not establish that decaf coffee is generally healthier than caffeinated coffee. Both contain compounds such as polyphenols, and some protective associations observed for caffeinated coffee also appear with decaf—but findings differ by health outcome, and association is not proof that either drink causes a benefit.
What is different about decaf coffee?
Decaffeinated coffee is compositionally similar to caffeinated coffee, except that it contains little or no caffeine, according to a BMJ umbrella review of coffee and health meta-analyses. Harvard T.H. Chan School of Public Health also notes that both forms contain polyphenols. That means removing caffeine does not remove every compound that may be relevant to health.
Harvard’s Amy Roeder, writing on January 14, 2026, quoted epidemiologist and nutrition professor Edward Giovannucci as saying, “Most of the metabolic effects are probably not related to caffeine,” and, “A lot of the protective associations that we see in caffeinated coffee are also seen in decaf.” These are interpretations of observed evidence, not a finding that decaf outperforms regular coffee. The available findings do not identify a general winner. Results depend on the outcome studied, how coffee intake was measured, the population and follow-up, and whether a study directly compared caffeinated with decaffeinated coffee.

Cardiovascular disease
The BMJ review found no clear association between high versus low decaffeinated coffee consumption and cardiovascular disease. This does not prove that decaf has no effects; it means the reviewed evidence did not establish a clear benefit for that comparison.
Type 2 diabetes
A 2014 systematic review and dose-response meta-analysis in Diabetes Care included 1,109,272 participants and 45,335 cases of type 2 diabetes. In its continuous analysis of the difference between caffeinated and decaffeinated coffee per additional cup per day, the difference was not statistically significant. Dementia and cognition
A 2025 JAMA prospective cohort analysis examined coffee and tea intake in relation to dementia risk and cognitive function. Its findings concern those outcomes; they cannot establish that decaf is healthier overall or superior across unrelated health measures. View the JAMA study.
How to assess a claim that decaf is healthier
Before treating a headline as evidence of an advantage, check what the underlying study actually compared and measured:

- Outcome: Is the claim about diabetes, cardiovascular disease, cognition, or another specific measure? A result for one outcome does not answer every health question.
- Study design: Was the evidence observational or experimental? Observational associations cannot by themselves show that coffee caused the outcome.
- Participants and follow-up: Who was studied, and over what period? Results may not apply equally to other populations.
- Intake definition: How much coffee counted as a serving, and how was consumption recorded?
- Direct comparison: Did researchers compare decaf directly with caffeinated coffee, or compare each with a different reference group? A direct comparison is needed to support a claim that one is better than the other.
Harvard also notes that unfiltered brewing methods have been linked to higher LDL cholesterol. That is a brewing-method consideration, not evidence that decaf itself has a general advantage over caffeinated coffee.

Some health associations reported for caffeinated coffee also appear with decaf, and the two drinks share many components aside from caffeine. But the available evidence is mixed across outcomes: for example, the BMJ review reported no clear high-versus-low decaf association for cardiovascular disease, while the diabetes meta-analysis did not find a statistically significant difference between caffeinated and decaffeinated coffee in its continuous one-cup-per-day comparison.

No numerical estimate of a general decaf advantage is established by these sources. A 2019 dose-response meta-analysis indexed on PubMed concerns all-cause mortality, but its indexed record does not by itself support a claim that decaf is better than caffeinated coffee. View the PubMed record.
