Usually, no. You can continue an existing Baldur’s Gate 3 save to receive game updates, but a patch does not rewind your campaign or automatically replay a scene you have already passed. Whether a particular Karlach or Astarion scene is still available depends on your campaign progress and the choices your save recorded.
What to do with your current save
Update the game and continue your current campaign if you want to see content added or repaired by a patch. If a specific scene does not appear, the important question is whether your save can still reach that scene’s trigger—not whether you started the campaign before the update.
- Your save is before the relevant scene: Continue and meet the story conditions for it.
- Your save has already passed the scene: Load an earlier save if you have one. If not, a new playthrough may be necessary to reach that particular scene again.
- The scene depends on past choices: Updating the game does not change those choices or guarantee that the scene will trigger.
Larian’s notes document specific fixes involving scenes and saves, not a guarantee that every old save can access every update-related scene.
Which Karlach and Astarion content changed?
Karlach’s optional ending
In Community Update #24 on 29 August 2023, Larian said it was expanding the epilogues and adding an optional ending with Karlach in Patch 2. This was an additional ending, not a general reset of existing campaigns. Larian’s Community Update #24.

Epilogue and scene changes in Patch 5
Patch 5, released 30 November 2023, added a section after the Netherbrain’s defeat intended to give the story closure with allies. Its notes also describe restored cinematic portions of Astarion’s Act III Ascendant scene and a fix for the screen going black during his endgame romance scene. For Karlach, the notes say her camp reflection on leaving Act II could also trigger during the transition into Act III if needed. Larian’s Patch 5 notes.
More epilogue reactivity in Patch 6
Patch 6, released 16 February 2024, added Karlach and Astarion reactions in God Gale’s epilogue dialogue, where they can ask for a cure for their conditions. It also added a new version of Astarion’s kiss scene after he ascends, and included epilogue behavior fixes involving saves and relationships. Larian’s Patch 6 notes.

Targeted fixes and later polish
Patch 7, released 5 September 2024, addressed a Karlach Avernus scene-flow issue and an Astarion romance edge case during the camp celebration, among other dialogue and save-related fixes. These are examples of particular problems being corrected; they do not mean that every scene missed in an earlier campaign becomes available retroactively. Larian’s Patch 7 notes.

Why an update may not show the scene you want
“Karlach and Astarion content” covers different things: an optional Karlach ending, Astarion romance and Ascendant scenes, and character-specific epilogue dialogue. Their triggers are not interchangeable. The official notes identify some additions and fixes, but do not provide a complete checklist of romance flags or trigger conditions for every scene. If one does not appear, check your campaign progress and choices rather than assuming the patch failed to install.
So, start by continuing your current save. Use an earlier save to revisit a trigger your campaign has already passed; start a new campaign only if your current save cannot reach the scene and you have no suitable earlier save.
