June 2026 reports confirm a three-member committee investigating a fatal accident at the Vizag Steel Plant. They do not confirm that Andhra Pradesh adopted or formally proposed recurring, statewide three-member industrial safety audit teams. The distinction matters: an inquiry into one accident is not the same as a standing inspection programme.
What the reported three-member committee is investigating
Reports describe an explosion at Steel Melt Shop-1 of Rashtriya Ispat Nigam Ltd’s Vizag Steel Plant during casting operations involving molten steel. Hindustan Times reported eight worker deaths and six injuries. The cited reporting said the cause remained under investigation.
Hindustan Times described a three-member committee examining whether safety protocols were followed and safety audits conducted. India Live separately reported that SAIL constituted a three-member committee for a preliminary inquiry. The available reports do not establish whether these descriptions refer to precisely the same committee, or provide enough detail to define its membership, powers, or timetable.

What officials and workers said—and what remains unproven
Andhra Pradesh Deputy Chief Minister Pawan Kalyan questioned whether industrial managements were conducting safety audits properly and taking necessary precautions. That reported scrutiny is not a finding that audits were absent or deficient.
Workers’ and union representatives raised allegations involving staffing, maintenance, raw materials, and oversight. Those claims have not been established as findings of the inquiry. The cited reports do not provide a final determination of the accident’s cause or establish that a particular audit failure caused it.

Why OISD audits do not establish a statewide proposal
A 2020 Ministry of Petroleum and Natural Gas account describes audits by the Oil Industry Safety Directorate (OISD) in the oil and gas sector. It refers to external and surprise audits at existing installations and pre-commissioning audits for new facilities. That is sector-specific context, not evidence of a new Andhra Pradesh policy covering all industries.
The same account reported major onsite incidents reported to OISD as follows. These are historical figures, not current totals or counts for all Indian industrial facilities.
