Offline-first design also needs clear sync states, privacy safeguards, and a fit with the person’s care—not just a local save button. Google’s guidance for healthcare apps recommends keeping the primary workflow available offline; applying that principle to patient-facing CBT is a design inference, not evidence that an app is clinically effective.
What should work without internet?
The essential practice flow should remain usable offline: the user can open the exercise, read or hear its instructions, work through it, and record practice or reflections. If the network disappears mid-session, the app should preserve the work rather than interrupt the therapeutic task with a demand to reconnect.
Google Open Health Stack’s Design Guidelines for Offline & Sync says, “Ensure users can complete their primary workflow offline.” Its guidance is aimed at community healthcare worker apps in unreliable-connectivity settings. Applying it to a patient-facing CBT practice flow is a practical design analogy, not a clinical efficacy finding.
Separate setup downloads from practice
If an exercise or content pack must be downloaded before offline use, make that requirement explicit. Tell the user which material needs downloading, where to do it, how long it may take, and whether the app needs to stay open. Give a clear confirmation when the material is ready. Do not imply that an exercise is available offline until it actually is.

For an app intended to support intermittent connectivity, the first-run setup should make offline readiness easy to check. Users should be able to distinguish content stored on the device from content that still requires a connection.
How should a CBT app save work and sync later?
Saving and synchronizing are different events. A completed local save means the work is on the device; it does not necessarily mean a clinician, another device, or an online account can see it. The interface should say which state applies and preserve enough context—such as when an exercise or entry was created—for saved work to remain understandable.

Show the status the user needs
- Offline: The app has no connection; explain that locally available exercises remain usable.
- Saved on this device: Confirm that an entry has been saved locally, without suggesting it has synced.
- Syncing: Indicate that transfer is in progress when a connection is available.
- Synced: Confirm completion only after the service verifies the transfer.
- Sync failed: State what failed and offer a useful next action, such as retrying when connected.
An ordinary offline state is not itself a synchronization failure. Avoid alarming error language when the user can continue practicing and the app has retained the work. Conversely, do not show a reassuring “done” state if the entry is only stored locally.
Set a realistic reconnection expectation
Tell users how often they need to reconnect, if at all, and what synchronization enables. Choose any suggested interval for the actual service and care context. Google’s examples concern healthcare-worker workflows; they do not establish a universal reconnection schedule for consumer CBT apps.

When syncing fails, explain what happened in plain language and what the user can do next. Preserve the local entry while the problem is unresolved, and make clear whether the user needs to retry or take another action. Do not claim that data is backed up or available to a care team unless that transfer has succeeded.
How can readers assess whether an offline CBT app is suitable?
Offline capability addresses continuity, not clinical quality. AHRQ describes behavioral health apps as including established psychotherapies such as CBT, alongside features such as psychoeducation, self-management, tracking, and goal setting. Its app-selection guidance emphasizes evidence, privacy and security, usability and accessibility, monitoring, interoperability, and alignment with the patient’s goals and wider plan of care. Marketplace availability or user ratings alone cannot establish that an app is appropriate or effective.

Use these questions to compare options:

NICE recommends evidence-based behavior-change techniques including goals and planning, feedback and monitoring, and social support. Its recommendations also call for considering user preferences, accessibility, cost, hardware and operating-system availability, internet and phone-signal access, and data use. These are evaluation considerations, not proof that a particular app improves outcomes.

CBT exercises and reflections can contain sensitive personal and health information. Local storage does not make that information private by itself: someone else may use the device, and saved data may later be transmitted. Before choosing or designing an app, understand what it collects, where and how it stores information, how it uses and transmits it, who can access it, and what sharing the user can control. AHRQ recommends examining privacy policies as well as security, ownership, and funding; WHO’s evidence review identifies privacy and informed-consent concerns among digital-health feasibility issues.

Offline-first behavior should be explained in those same terms: tell users what remains on their device, what syncs later, who can access synced information, and how they can manage sharing. Do not equate “works offline” with “secure,” “confidential,” or “not shared.”
Keep the role of self-guided practice clear. The World Health Organization states that digital health interventions are not substitutes for functioning health systems. A CBT exercise flow should not be presented as a replacement for professional care when a person’s needs call for clinical support.
What offline-first design cannot solve
Offline access can help when a signal is intermittent, but it cannot overcome every access barrier. A person still needs a usable device and, in many cases, electricity; language, accessibility, cost, and cultural fit also affect whether the tool can be used. NICE explicitly includes connectivity, hardware, accessibility, preferences, and cost among relevant considerations, while AHRQ highlights usability, access, and fit with care.
A paper CBT workbook or self-help manual may be an optional aid when a device or connection is unavailable. The cited app and digital-health guidance does not evaluate paper workbooks or establish that they are clinically equivalent to an app or therapist-guided care.
