If you’re an adult hiker without a diagnosed foot condition or persistent pain, a prefabricated over-the-counter (OTC) insole is a reasonable, lower-commitment place to start—provided it fits your hiking footwear comfortably. Custom orthotics are not automatically more effective. The available comparisons concern people with foot pain, not hiking performance, so they cannot tell us which option is better on trails, under a pack, or on uneven ground.

What’s the difference?

Both are removable, in-shoe devices intended to cushion or off-load parts of the foot. The main difference is how they’re made: custom orthoses are shaped from an impression of an individual foot, such as a cast or 3D scan, while prefabricated OTC insoles are mass-produced in foot sizes. That distinction describes fabrication, not a guarantee that one will feel or work better for you. CADTH’s overview of foot orthoses explains these categories.

What does the evidence say?

For plantar heel pain, custom devices have not shown a consistent advantage

A 2019 CADTH review found no difference between custom-made and prefabricated orthoses for pain reduction or functional improvement in adults with plantar heel pain at 6 weeks, 12 weeks, or 12 months. The evidence was rated very low to low quality, so the finding is not proof that the options are equivalent for every person. Read the CADTH review.

A 2018 systematic review and meta-analysis also found no difference between customized and prefabricated orthoses for pain or function in plantar heel pain, and no difference between soft and firm materials. It did find moderate-quality evidence that orthoses reduced pain more than sham supports in the medium term, defined as 7–12 weeks. That comparison does not establish that custom devices outperform OTC ones. See the review in the British Journal of Sports Medicine.

Results depend on the diagnosis

Cochrane’s 2022 review covers several foot-pain diagnoses and finds that results vary by condition and comparison. Its evidence base included 11 trials with 1,332 participants; that is not a hiking-specific sample. The review notes limited evidence for clinical decisions about custom orthoses, so a result for one condition should not be generalized to all foot pain or to hikers generally. A trial of custom insoles did not establish a hiking benefit

In a 2020 randomized trial of 185 people with plantar heel pain, referral for custom insoles did not produce better outcomes at 12 weeks than sham insoles or GP-led usual care; 176 participants completed the 12-week follow-up. The trial studied people with plantar heel pain and those treatment comparisons—not hikers, trail conditions, or performance under a pack. See the STAP study abstract.

How to choose for your situation

No diagnosis and no persistent pain

An OTC insole is a practical first option to consider if it fits the boot or shoe you plan to hike in. Judge it by how it fits and feels in that footwear. The cited evidence does not establish that a particular arch shape, firmness, or brand prevents injury or performs best on trails.

Diagnosed condition, persistent pain, or worsening symptoms

Use an individualized assessment rather than this comparison as a diagnosis or prescription. Evidence differs by condition, and a qualified foot-health clinician can assess your symptoms and discuss whether a device is appropriate for your circumstances.

Comparing devices for a particular boot

Try to evaluate fit and comfort in the intended footwear rather than assuming that a custom-made device will fit a hiking boot better. The clinical sources cited here do not establish a hiking-specific break-in schedule, validated boot-fitting protocol, or product ranking. They also do not compare outcomes on long hikes, uneven terrain, or while carrying a pack.

Possible downsides

Reported adverse effects of foot orthoses include additional foot pain, ankle instability, and skin irritation. Evidence about complications is imprecise, so these reports do not predict whether a particular device will cause problems for you. If an insole brings on new or worsening symptoms, stop relying on it as a self-treatment and seek appropriate clinical advice. Cochrane’s review summary.
