Cashless claims let an insurer or its third-party administrator (TPA) pay an approved amount directly to a network hospital after preauthorization. With a reimbursement claim, you generally pay the hospital bill first and then submit a claim to the insurer. Neither route guarantees payment: what the policy covers and the admissible amount still depend on its terms, limits, exclusions and the claim details.

What is the difference between a cashless and reimbursement claim?

The routes differ mainly in who pays the hospital first and how the claim is submitted. Cashless settlement is conditional on the hospital’s current network arrangement and the insurer’s preauthorization. Reimbursement means paying first and asking the insurer to assess the claim afterward.

Question Cashless Reimbursement
Who initially pays the hospital? The insurer or TPA pays the network provider to the extent authorized. The patient or family pays first, then submits a claim.
Which hospitals can be used? A hospital with a current cashless arrangement with the insurer. IRDAI says reimbursement may be allowed at any hospital or medical establishment, subject to the policy and applicable licensing or registration rules.
What process applies? Preauthorization is required for cashless authorization. Follow the policy’s notice, claim-submission and document requirements.
What determines the amount payable? The approved authorization and the policy’s coverage and admissibility rules. The policy terms, evidence submitted and the insurer’s admissibility decision.
Main practical consideration Cashless service may not be available or approved in full; costs outside approval or coverage may remain payable by you. You need to arrange the upfront payment and submit the records the insurer requires.

How does a cashless claim work?

Cashless does not mean the hospital bill is automatically paid in full. The insurer’s payment is limited to the approved extent and remains subject to the policy. IRDAI’s health-insurance FAQ says, “Cashless facility shall be offered only at Network Providers which have entered into an agreement with the insurer to extend such services.” (IRDAI health-insurance FAQ.)

  1. Check with the insurer whether the hospital is currently in its cashless network. Network lists and arrangements can change.
  2. Ask the insurer or hospital how preauthorization is submitted and what information or forms are required for your admission.
  3. The insurer or TPA assesses the request. If approved, it authorizes direct payment up to the approved amount, subject to policy terms.
  4. At discharge, the hospital sends the final authorization request. Review the settlement details and ask the insurer about any amount not authorized or covered.

How do I claim health insurance reimbursement?

With reimbursement, you or your family settle the hospital bill and submit a claim afterward. IRDAI says this route may be available at any hospital or medical establishment, but that does not make every treatment or expense payable: the policy and applicable licensing or registration rules still apply.

  1. Check your policy or contact the insurer for its current claim form, notice requirements, submission method and deadline.
  2. Keep the records the insurer identifies as necessary. Depending on the claim and policy, these may include itemized bills, payment receipts, discharge papers, prescriptions and diagnostic reports.
  3. Submit the completed claim and required records using the insurer’s stated process. Keep copies of submitted documents and communications.
  4. Review the insurer’s decision and settlement calculation. If a claim is reduced or denied, ask for the reason and the policy provision relied on.

This is not a universal document checklist: forms, deadlines and required evidence can vary by policy. IRDAI’s FAQ says documents absent from the policy terms should not ordinarily be treated as necessary, except where fraud is suspected, and that necessary documents should be requested together rather than piecemeal. Check the current policy procedure and the insurer’s instructions for your claim. (IRDAI health-insurance FAQ.)

Does a denied cashless request mean the treatment is not covered?

Not necessarily. A cashless request concerns direct settlement through a network hospital and preauthorization. If that route is unavailable or declined, ask the insurer why and whether you can pay the hospital and submit a reimbursement claim under your policy. Reimbursement is not automatic, and the insurer will still assess the treatment and expenses against the policy terms.

Likewise, cashless approval does not establish that every final expense is admissible. Limits, co-payments, exclusions, deductibles and other policy conditions can affect what the insurer pays. Only the policy wording, insurer confirmation and facts of the claim can establish the result for an individual patient.

How long does an insurance claim take in India?

IRDAI’s health department guidance, referencing its Master Circular dated 29 May 2024, states these service timelines:

Rank #3
J. J. Keller 2024 Emergency Response Guidebook (ERG), Spiral
  • The 2024 ERG guide helps satisfy 49 CFR 172.602 DOT requirement. This requirement states that hazmat shipments be accompanied by emergency response info.
  • Pocketbook aids in emergency preparedness, planning, and training with ERGs numerically indexed and color-coded to help emergency responders find vital information fast.
  • 2024 Updates: The Pipeline and Hazardous Materials Safety Administration (PHMSA) released a comprehensive summary of updates. Most significantly a QR code on the back cover that provides access to critical incident reporting information.
  • Other changes for 2024 have been made to continue to provide the most accurate emergency response information to help all front-line persons and all first responders stay safe during transportation emergencies.
  • Specifications: 4" x 5 1/2" Pocketbook Size, English, Spiralbound. Copyright 2024.
Claim stage Stated IRDAI timeline
Decision on a cashless preauthorization request Immediately, and within 1 hour
Final cashless authorization after the hospital’s discharge authorization request Within 3 hours
Settlement of claims other than cashless Within 15 days

These are timelines stated in IRDAI’s guidance, not guarantees that a claim will be approved or that the full amount claimed will be admissible. Check the insurer’s current procedure and the relevant IRDAI guidance for updates. (IRDAI health department.)

What should I check before a planned admission?

  • Confirm that your policy is active and review the relevant coverage and claim conditions.
  • Verify the hospital’s current cashless status with your insurer; do not rely only on an old network list.
  • Ask the insurer or hospital how preauthorization works and which forms or records are needed.
  • Ask how to proceed if cashless service is unavailable, declined or approved only in part, and make a plan for any upfront payment.
  • Keep copies of bills, receipts, medical records and claim communications that the insurer requires. A simple expanding file folder can help keep paper records together.

IRDAI directs policyholders to insurers’ published hospital lists, cashless and reimbursement procedures, and turnaround times. Follow the current instructions for your specific policy rather than relying on a generic checklist. (IRDAI health department.)

How can I challenge a denial or disputed settlement?

  1. Ask the insurer for the decision in writing, including the reason and the relevant policy condition. IRDAI’s FAQ says a denial or repudiation communication should give reasons, refer to policy conditions and include grievance-redressal details.
  2. Use the insurer’s grievance procedure and provide the policy or claim details along with relevant records and correspondence.
  3. If the issue remains unresolved, you can use IRDAI’s Bima Bharosa complaint channel. Its online form asks for a policy or claim identifier for claim complaints and accepts PDF, JPG, JPEG and PNG attachments.

For a complaint, keep copies of the denial, policy wording, bills and prior communications so you can refer to the decision and supporting records. (IRDAI health-insurance FAQ; Bima Bharosa.)
