Yes. Weight regain is common after stopping GLP-1-based medicines used for weight management, but the amount varies from person to person. In one tirzepatide withdrawal trial, participants who switched to placebo gained an average of 14.0% of their body weight over 52 weeks; that figure is not 14% of the weight they had lost. If you are considering stopping, discuss a follow-up plan with your prescribing clinician.
What happens to weight after stopping treatment?
Weight often increases after GLP-1-based treatment is discontinued. The American Diabetes Association (ADA) says that sudden discontinuation of semaglutide or tirzepatide results in regaining about one-half to two-thirds of the lost weight within a year. That is a group-level estimate, not a prediction for any one person.
In its 2025 Standards of Care, the ADA recommends continuing obesity medication after treatment goals are reached to maintain health benefits, since stopping often leads to weight recurrence and may worsen or bring back cardiometabolic risk factors and obesity-related conditions. The guidance also calls for shared decision-making when considering long-term treatment.

How much weight may come back?
Different studies report regain using different measures. The distinction matters: a percentage change in total body weight is not the same as a percentage of previously lost weight.

What the tirzepatide trial can—and cannot—tell you
SURMOUNT-4 enrolled 783 adults with overweight or obesity in a 36-week open-label tirzepatide period. Of those, 670 were randomized to continue tirzepatide or switch to placebo for 52 weeks. Participants had no diabetes, had completed the lead-in, and tolerated a maximum tolerated dose of 10 or 15 mg. They also received lifestyle counseling. The results show what happened to this selected trial population; they cannot establish how much or how quickly an individual will regain weight.
Why do published estimates differ?
The figures describe different populations and use different denominators. The SURMOUNT-4 result is a percentage change in total body weight after randomization. The ADA’s range and the 2026 review’s estimate describe the fraction of treatment-related weight loss regained. They should not be read as interchangeable measures.

The 2026 review included 48 studies overall, but its trajectory model drew on six randomized trials. Most included studies were judged to have a moderate risk of bias. Its estimate after one year is a pooled model result; projected outcomes beyond 52 weeks are less certain because they are extrapolated rather than observed in long-term randomized follow-up.
What should you do if you are thinking about stopping?
Make the decision with the clinician who prescribes the medication. The ADA recommends shared decision-making about long-term use, including continued treatment at the lowest effective dose or stopping with close weight monitoring. The sources cited here do not establish a universal tapering schedule or show that tapering prevents regain, so do not change or stop a prescribed medicine based on trial averages alone.

Useful points to discuss include:
- Your treatment goals, preferences, and experience with the medicine.
- Tolerability, other health considerations, and whether the medication is accessible and affordable.
- How weight and health will be monitored if you stop, and when you will follow up.
- What additional support is available, such as dietary guidance or multidisciplinary weight-management care.
Can lifestyle changes prevent regain?
Dietary support and ongoing weight-maintenance care can be part of a plan after treatment ends. An international expert consensus statement recommends monitoring for regain and active dietary support, including referral to a registered dietitian as part of multidisciplinary care. These are care recommendations, not proof that a specific diet, exercise routine, or product reliably prevents regain.
Weight tracking can help you and your clinician notice a change and discuss what to do. A scale is only a monitoring tool: it cannot prevent regain or replace clinical follow-up.

What remains uncertain
Evidence supports the general pattern of weight recurrence after stopping, but it cannot tell an individual exactly how much weight will return or how quickly. Trial results depend on the participants, medicine, treatment period, and follow-up studied. The cited evidence also does not establish one proven stopping, tapering, or lifestyle protocol that prevents regain.
