Apple Watch Vitals can flag overnight readings that fall outside your personal typical range, but it cannot tell you that you are getting sick or diagnose why a reading changed. To get the most useful readings, enable sleep tracking and Sleep Focus, wear your watch overnight for seven days to establish a typical range, and keep it comfortably fitted against your skin.
What Apple Watch Vitals can—and can’t—tell you
Vitals compares overnight measurements with a typical range based on your own collected data. Depending on your watch, those measurements can include heart rate, respiratory rate, blood oxygen, wrist temperature and sleep duration. The feature is designed to show changes from your baseline, not to identify a disease.
Apple says illness is one factor that may affect readings. By default, Vitals sends a notification when at least two overnight metrics are outside their typical ranges. It may also show possible contributing factors such as medication or elevation changes. An alert is a reason to review a pattern—not confirmation that you have an infection or any other condition.

Apple also states that “Your Apple Watch is not a medical device.”

How to get a more useful Vitals baseline
Apple’s setup guidance focuses on collecting overnight data consistently and wearing the watch so its sensors can contact your skin. These steps help the feature establish a typical range; Apple does not claim that they guarantee accurate readings or make Vitals an illness detector.
- Update your devices. Use an iPhone with the latest iOS and an Apple Watch with the latest watchOS, as Apple’s setup checklist recommends.
- Enable sleep tracking. In the Health app on iPhone, set up Sleep and turn on Track Sleep with Apple Watch. Use Sleep Focus during your scheduled sleep period.
- Wear the watch to bed for seven days. Apple specifies seven days of sleep wear to establish a typical range. Until that period is complete, your baseline may not yet be established.
- Check the fit. Wear the watch comfortably, with its back in contact with your skin. Apple warns that a watch that is too loose may not capture accurate readings. Do not overtighten it to compensate.
- Review the pattern, not just one number. Check whether multiple metrics are outside their typical ranges and look at the surrounding nights. A single reading does not identify its cause.
Why did my Apple Watch say my vitals were out of range?
The notification means that at least two overnight metrics were outside your personal typical ranges under the default setting. It does not mean that Apple Watch identified a specific illness. Apple notes that factors such as medication, elevation changes and illness may be relevant, but the notification cannot determine which factor caused a change.

You can review Vitals on your Apple Watch or in the Health app on iPhone. Apple says the watch shows the last seven days of history; the iPhone Health app provides daily, weekly, monthly and six-month chart views. Use those views to understand when readings changed, rather than treating the alert as a diagnosis.

Which Vitals measurements depend on your watch model or region?
Available metrics vary by hardware and, for blood oxygen, by region and model identifier. In particular, wrist temperature is supported on Apple Watch Series 8 or later and all Apple Watch Ultra models. Apple lists daytime vitals for Apple Watch Series 12 and Apple Watch Ultra 4.

Blood Oxygen is not available in Vitals on certain Apple Watch models purchased in the United States on or after January 18, 2024. Apple identifies affected models by part numbers ending in LW/A or LS/A; on those models, blood-oxygen data analysis is performed on iPhone and results appear in Health. Check your exact model and region before assuming a metric should appear.
Apple Watch Series 12 and Apple Watch Ultra 4 show heart rate variability in place of sleep duration and also support the listed daytime vitals view. The metrics shown can therefore differ from those on other supported watches.

Is wrist temperature the same as checking a fever?
No. Wrist temperature is an overnight metric, not a direct body-temperature reading. A change in wrist temperature can be part of a broader pattern, but it does not establish that you have a fever or explain why a measurement changed.
