Possibly at the level of molecular biomarkers: a September 2026 preprint reports that biological-age measures in transplanted hearts tracked the recipient’s age, including older hearts placed in younger people. It does not show that the heart’s chronological age was reversed, that it functioned better, or that recipients lived longer. The human evidence is preliminary and small.
What does it mean for a transplanted heart to become biologically younger?
“Biological age” in this study refers to molecular indicators, including DNA methylation and gene expression—not the number of years since a heart formed, nor a direct measure of how well it pumps. The authors compared these indicators in transplanted hearts and found they were more strongly associated with recipient age than donor age.
That pattern included apparent rejuvenation in older hearts transplanted into younger recipients. The authors concluded that “the systemic environment” can drive tissue biological age. This is their interpretation of the findings in the preprint “Transplanted hearts assimilate the recipient’s biological age”, posted on 17 September 2026.
How strong is the evidence in people?
The human tissue comparison was small. Nature reported 11 transplants with substantial donor-recipient age differences: eight involved older hearts transplanted into younger recipients, with gaps of 8–24 years; three involved younger hearts transplanted into older recipients, with gaps of 38–50 years. The authors also examined clinical records and heart-function information, but this comparison is not definitive proof of a clinical benefit.

Nature described the report on 28 September 2026 as a bioRxiv preprint that had not yet been peer reviewed. Its findings should therefore be treated as preliminary unless its publication status is updated. The human observations do not establish what happens across the broader population of heart-transplant recipients.
What did the mouse experiments show?
The researchers studied heterotopic heart transplants—grafts placed in a different position while the recipient’s own heart remains in place—in mice of different ages. The grafts’ measured biological-age indicators were associated with recipient age. In the same experiments, the researchers did not see a matching shift in measured age indicators in the recipient’s native heart, blood, or liver.

These animal results support the possibility that a recipient’s systemic environment affects a transplanted heart’s molecular state. They do not establish the same mechanism or a clinical benefit in people.

Does this mean older donor hearts are safe for younger people?
No. A shift in molecular age indicators is not an organ-allocation recommendation. The preprint does not show that an older graft lasts longer, reverses heart disease, or improves survival in a younger recipient. Decisions about a donor heart involve its clinical condition and the recipient’s circumstances; this study does not rank those considerations or establish a new transplant rule.

A separate 2024 mouse study reported that transplanting old hearts into young or middle-aged recipients was associated with more senescent cells in several recipient tissues, as well as impaired physical performance and spatial learning and memory. That experiment measured different outcomes and does not directly disprove the 2026 finding about graft molecular-age markers. It does, however, caution against interpreting “rejuvenation” as a claim that every effect of receiving an older heart is beneficial.

- Whether the molecular-age pattern can be replicated in larger human studies.
- Whether changes in DNA methylation and gene expression correspond to meaningful differences in heart function or graft durability.
- Whether the findings have any effect on clinical outcomes, including recipient survival.
- How the graft’s molecular changes relate to effects elsewhere in the recipient’s body.
For context, Nature reported 4,636 heart transplants in the United States in 2024, 81.5% more than in 2013. That volume figure describes transplant activity; it does not show that changing donor-age practices would be safe or beneficial.
