A reliable LLM agent is not an unconstrained loop with a vector database attached. It is a bounded, observable workflow that classifies the request, selects the right data source, retrieves and verifies evidence, controls tool use, and refuses or escalates when the evidence is insufficient. Advanced retrieval improves access to private and changing data, but reliability remains an end-to-end systems property.

The practical design is: validate and classify the request, rewrite or decompose it, route to vector, keyword, SQL, graph, web, or API retrieval, retrieve broadly, rerank and compress, grade evidence, generate a cited answer or controlled action, verify the result, and record the complete trace.

What makes an LLM agent reliable?

A chatbot generates text. A RAG application adds external evidence. A tool-using agent can act. An agentic RAG system combines retrieval, reasoning, and actions. A production-reliable system adds authorization, bounded execution, verification, observability, and recovery.

The RAG literature commonly describes naive, advanced, and modular RAG. Advanced RAG adds pre-retrieval and post-retrieval techniques such as metadata filtering, query transformation, reranking, and context compression (survey of RAG techniques).

Failure can occur at every stage

  • Input: ambiguous requests, prompt injection, or malicious document text.
  • Planning: incorrect decomposition, unnecessary tools, or repetitive loops.
  • Retrieval: missing files, poor chunking, lexical mismatch, stale or unauthorized results.
  • Context: duplicates, contradictions, excessive passages, or buried evidence.
  • Generation: unsupported claims, incorrect synthesis, or mismatched citations.
  • Tools: wrong selection, invalid arguments, timeouts, rate limits, or partial results.
  • State and operations: lost memory, tenant leakage, high cost, missing traces, or silent regressions.

Choose the simplest adequate architecture

Anthropic recommends starting with the simplest solution and adding agentic complexity only when it is genuinely needed (Anthropic’s agent guidance). A fixed workflow is usually more predictable for known steps, strict formats, and regulated actions. An agent is justified when requests vary materially, tool choice is exploratory, or adaptive retrieval is valuable.

Retrieval adds latency, cost, and failure modes, so do not force every question through it.

Reference architecture for bounded agentic RAG

  1. Validate input and screen for prompt injection.
  2. Classify intent, risk, freshness requirements, and authorization.
  3. Choose direct response, clarification, deterministic tool, or RAG.
  4. Rewrite or decompose the query while preserving the original.
  5. Route to the appropriate source and apply security filters before retrieval.
  6. Run hybrid or specialized retrieval.
  7. Deduplicate, rerank, expand parent context, and compress.
  8. Grade evidence for relevance, completeness, freshness, and conflict.
  9. Retry with a bounded policy, ask a targeted question, or refuse if evidence is inadequate.
  10. Generate a grounded answer or a structured tool action.
  11. Verify claims, citations, schemas, policies, and side effects.
  12. Pause for approval when an action is irreversible, financial, privacy-sensitive, or external.
  13. Trace the request, evidence, decisions, tools, latency, tokens, cost, and outcome.

A state machine or graph makes these branches explicit. OpenAI’s documentation distinguishes a lower-level Responses API, where the application owns loops and branching, from the Agents SDK, which supplies agent loops, handoffs, sessions, guardrails, resumable approvals, and traces (Agents SDK documentation).

Build a trustworthy ingestion pipeline

Retrieval quality starts before the user asks a question.

  1. Collect source files and preserve stable source identifiers.
  2. Parse PDFs, HTML, office files, tables, images, and scanned pages; use OCR where needed.
  3. Normalize encoding, whitespace, headings, lists, tables, headers, and footers.
  4. Store metadata such as document_id, parent_id, title, section, page, source_url, created_at, updated_at, tenant_id, access_scope, and document_version.
  5. Split by document structure rather than one universal character limit.
  6. Create embeddings and build both vector and lexical indexes.
  7. Test representative queries against the parsed and indexed corpus.
  8. Version the corpus and index, and define update and deletion procedures.

Keep child chunks for precise matching and parent sections for contextual expansion. A chunk may require its heading, definitions, exceptions, table caption, or surrounding policy to be interpretable.

PDF reading order, flattened tables, repeated headers, and OCR errors are common sources of silent retrieval defects. Deleting a document must remove derived chunks, embeddings, caches, and search records. Enforce tenant and access-scope filters before results enter the model context; a prompt instruction cannot repair unauthorized retrieval.

Advanced retrieval techniques

Query rewriting

Resolve pronouns, add domain terminology, extract entities and filters, and generate several search formulations. For “What changed in the retention policy after the 2025 update?”, variants might include “retention policy 2025 update changes,” “data retention policy revised 2025,” and “retention period amendment effective date.” Log the original and every rewrite, and reject rewrites that invent entities, dates, or constraints.

Query decomposition

Split multi-part questions into independently retrievable subquestions. A pricing comparison may require the 2024 rule, the 2025 rule, the differences, and the affected customer segments. Preserve dependencies when one answer must be established before another.

Hybrid and metadata-aware retrieval

Combine dense similarity with BM25 or another lexical method. Dense search helps semantic matches; lexical search protects exact error codes, SKUs, contract numbers, legal citations, and policy phrases. Apply metadata constraints for tenant, date, jurisdiction, product, status, and permissions before or during search.

Multi-query retrieval

Retrieve for multiple query variants, merge results, deduplicate by document or parent section, then rerank. Recall often improves, but latency, token use, and contradictory results can also increase.

Reranking and parent expansion

Retrieve a broad candidate pool, remove duplicates, use a cross-encoder or model-based reranker, retain a smaller evidence set, and expand selected child chunks to their parent sections. Similarity scores are not calibrated probabilities; thresholds must be tuned on representative evaluation data.

Contextual compression

Compress passages to the question while preserving numbers, dates, exceptions, definitions, negations, source identity, and scope. Evaluate compressed context against the original because compression can omit or distort decisive details.

Iterative, temporal, and graph retrieval

A corrective loop can retrieve, grade evidence, rewrite or broaden the query, and retrieve again. Set a hard attempt limit and per-request budget. Temporal retrieval should interpret “as of” dates, effective windows, versions, and conflicts between old and current records. Knowledge graphs help with relationships and multi-hop connections, but add extraction, synchronization, and schema-maintenance costs; they complement rather than replace text retrieval.

Control routing, tools, and state

Use typed schemas, allowlisted tools, authentication, authorization, timeouts, exponential backoff, circuit breakers, idempotency keys, output validation, maximum tool calls, and maximum loop iterations. Deterministic code should enforce permissions, numeric limits, dates, and transaction rules. Models can classify intent, grade relevance, detect contradictions, and select tools, but model checks alone are insufficient for high-impact actions.

Keep retrieved documents as untrusted data. Separate their content from system instructions, and ensure document text cannot authorize a tool call. Persist durable state explicitly and isolate it by user and tenant.

Grounded generation, verification, and refusal

A grounded answer policy should answer from sufficient evidence with citations, distinguish facts from inference, identify source dates, and expose conflicts. If evidence is incomplete, ask a targeted question or perform bounded retrieval. If it is absent, state that the indexed sources do not establish the claim. If a draft fails claim-to-passage verification, regenerate or refuse rather than silently retaining the unsupported statement.

Advanced RAG can reduce unsupported answers and make failures detectable; it cannot guarantee zero hallucinations. More context is not automatically safer: redundancy, contradiction, distraction, and token pressure can reduce quality.

Evaluate retrieval, generation, and agent behavior separately

Retrieval evaluation

  • Recall@k, precision@k, hit rate, MRR, or nDCG.
  • Document and passage relevance.
  • Metadata-filter correctness and permission leakage rate.
  • Freshness and temporal correctness.

Include normal, ambiguous, multi-hop, exact-match, no-answer, conflicting-source, access-control, and stale-document cases.

Generation evaluation

  • Correctness, faithfulness, helpfulness, and refusal accuracy.
  • Citation correctness and completeness.
  • Contradiction handling and schema compliance.

LangSmith distinguishes reference-based and reference-free evaluation and treats document relevance, faithfulness, helpfulness, correctness, and pairwise comparison as separate targets (LangSmith evaluation approaches).

Agent evaluation

  1. Final response: did the task complete?
  2. Single step: was the right tool selected with valid arguments?
  3. Trajectory: was the path acceptable, bounded, and non-repetitive?
  4. Evidence use: were the right sources retrieved and used?
  5. Safety: was refusal or approval requested at the right point?

Do not require one exact trajectory when several are valid. Test invariants, acceptable tool sets, action limits, and semantic outcomes. OpenAI’s evals guidance describes a data source plus testing criteria or graders (OpenAI evals documentation).

Production metrics

Monitor retrieval failures, unsupported-answer and citation-failure rates, tool errors, approvals, retries, loop termination, P50/P95 latency, tokens and cost per successful task, user corrections, and escalations. Segment by tenant, document type, query type, model, prompt, retriever, and index version. Replay traces against proposed changes before deployment.

Choose based on control, data sensitivity, scale, and existing systems rather than branding.

Prices and limits can change, and inference, embeddings, reranking, storage, telemetry, parsing, and deployment are often billed separately. A relational database with vector support may be preferable when joins, transactions, and permissions dominate. Managed services reduce operations; custom stacks provide more control over parsing, ranking, residency, and cost. Start with one agent and explicit tools; add multiple agents only when roles, permissions, context, or evaluation boundaries genuinely differ.

Before-production checklist

  • Define which requests bypass retrieval, require SQL, or need approval.
  • Validate parsing, OCR, tables, headings, metadata, versions, and deletion.
  • Enforce tenant and access filters before search results reach the model.
  • Combine lexical and semantic retrieval where exact terms matter.
  • Calibrate reranking and evidence thresholds on labeled data.
  • Bound retries, tool calls, tokens, latency, and cost.
  • Require typed tool arguments, idempotency, timeouts, and rollback or compensation.
  • Implement citations, claim verification, contradiction handling, and explicit refusal states.
  • Evaluate retrieval, answers, tools, trajectories, safety, and production traces separately.
  • Version prompts, models, indexes, and corpora; replay traces before rollout.
  • Provide human escalation and an incident-response path.

The Bottom Line

Build agentic RAG as a constrained evidence pipeline, not an autonomous prompt. The strongest systems retrieve with the right route and permissions, reduce and verify context, limit actions, expose uncertainty, and measure every decision in production.
