Defect is the more formal, standards-oriented term; bug is common in everyday developer conversation. The more important distinction is between a human error, a defect in a work product, and a failure that may happen when the defect is activated.
Bug vs. defect: the short answer
There is no universal technical rule that bugs are code-only while defects include requirements or other artifacts. The ISTQB groups bugs with defects and faults in its testing terminology. Its current glossary defines a defect as “An imperfection or deficiency in a work product where it does not meet its requirements or specifications or impairs its intended use.” (ISTQB Glossary, “defect,” Version 3.)
In practice, choose the word your team uses in conversation or its tracking workflow. When precision matters, describe the flaw and its consequences rather than relying on the label alone.
How errors, defects, and failures differ
These terms describe different concepts in a cause-and-effect model, not three interchangeable names for the same event. ISTQB CTFL Foundation v4.0 material puts it this way: “Human beings make errors (mistakes), which produce defects (faults, bugs), which in turn may result in failures.”

A defect does not always produce an observed failure. It may fail whenever the relevant code runs, fail only under certain conditions, or never cause a failure in practice. A failure is the visible outcome; the defect is the underlying flaw.
One example from requirement to user impact
Suppose a developer misunderstands a date requirement and writes a validator that rejects a date the system should accept. The misunderstanding is the human error; the faulty validator is the defect or bug; a user who is rejected when entering that date experiences a failure.

The requirement itself could also contain a defect—for example, if it contradicts the intended behavior. Defects can exist in requirements and specifications as well as code, tests, documentation, builds, and other supporting work products.
What to call an issue in a team or report
“Bug report” and “defect report” are overlapping, established terms. ISTQB glossary material also uses terms such as “bug tracking tool” and “bug report.” The glossary lineage supports their overlap; it does not establish a single naming convention that every organization must follow.

- Use the label your team’s tracker and workflow expect.
- Describe the expected behavior and the observed behavior separately.
- Include the conditions that reproduce the issue, since a defect may cause a failure only in particular circumstances.
- Identify the affected work product when it is known; the flaw may be in a requirement, test, document, or build rather than in application code.
For standards context, ISTQB describes its glossary as a shared terminology resource for the testing community. IEEE/ISO/IEC 24765-2017 is a broader systems and software engineering vocabulary standard; its catalog notes that definitions can be traced to active source standards.
