A companion app may offer conversation or wellbeing features, but that does not make it therapy or emergency care.
What an AI companion app can—and cannot—offer
“AI companion” can describe products built for friendly conversation, simulated relationships or general wellbeing. Their intended purposes and features vary, so start with the app’s own description rather than assuming that supportive language signals a mental-health service.
Companionship and wellbeing are not the same as clinical assessment or treatment. If an app claims to diagnose, treat or manage a mental-health condition, examine those claims more closely and check what evidence or regulatory oversight applies where you live. Do not assume evidence for a specialist mental-health chatbot applies to a general-purpose or romantic companion app.
The American Psychological Association’s health advisory cautions that mental-health products vary in evidence, expert input and post-market monitoring. It also notes that many self-directed wellness products are not regulated for safety or efficacy and may not be subject to healthcare privacy laws. The legal position depends on the product and jurisdiction; an app’s wellness label alone does not settle it.

How strong is the evidence for emotional support?
The International AI Safety Report 2026 describes evidence about AI companions’ psychological effects as emerging and mixed. Some studies associate heavy use with loneliness, emotional dependence or less interaction with people; others find reduced loneliness or no measurable effect on dependence or social health. Effects may differ according to the user, product design and pattern of use. The report also notes that research is limited by small samples, short follow-up and a lack of long-term studies.
That evidence does not establish that AI companions generally help or harm users. When an app makes a specific health claim, look for an evaluation that matches that claim—not just testimonials, popularity, a demonstration or promotional language.
- Who was evaluated? Check whether participants resembled the people the app is intended to support.
- What outcome was measured? A study of engagement or satisfaction does not by itself show improvement in a mental-health condition.
- For how long, and by whom? Note the evaluation’s duration, whether it was independent, and whether qualified clinical input was involved.
- What remains untested? Look for disclosed limitations, including whether results apply to the app’s current version or to long-term use.
The International AI Safety Report says some purpose-built mental-health chatbots may help with low-risk symptom management, including small-to-moderate improvements in depression management. That finding is not proof that a general companion app has the same effects. The report also describes inconsistent performance by both general and specialist models in simulated suicide-related scenarios.
What to check before choosing an app
Purpose, claims and oversight
Read the app’s description, terms and health-related claims. Is it presented as a conversation companion, a wellbeing coach or a tool intended to address a condition? If it claims to diagnose, treat or manage a condition, look for evidence specific to that use and find out whether it is registered or otherwise cleared in your jurisdiction. A lifestyle or wellbeing product may not have undergone the same checks as a regulated medical product.

For UK users, the Medicines and Healthcare products Regulatory Agency (MHRA) says some digital tools making claims to diagnose, treat or manage a condition are medical devices that must meet safety standards and display CE or UKCA marking. The MHRA advises checking its public register. This is UK-specific guidance, not a universal test of an app’s quality or status elsewhere. The MHRA’s consumer guidance was published on 27 January 2026 and updated on 28 January 2026.

Evidence and expert input
Look for a clear account of evaluations, who conducted them, what they measured and what limitations remain. Ask whether the app has clinical input and whether it monitors problems after release. Expert involvement or a published study can inform a decision, but neither alone guarantees that a product is effective or suitable for you.
Privacy and control over your data
Read the current privacy notice and terms before sharing anything you would not want stored or reviewed. Check each of these points rather than relying on a broad promise such as “private”:
- What conversation details and account information are collected, and whether voice, images or other sensitive data are included.
- Where and for how long information is stored, and how to delete conversations or close the account.
- Whether conversations may be used to improve or train models, and whether you can opt out.
- Who receives or can access the data, including service providers or other third parties.
- Whether memory or personalization can be reviewed, changed or switched off.
Policies and settings differ by app and may change. Verify the current terms for the particular service and your location; do not infer confidentiality protections from the fact that a conversation feels personal.

Crisis response and human support
Check what the app says it can do if you mention self-harm, suicide or immediate danger. Look for clear limits, appropriate signposting to human or crisis support, and a straightforward way to report an unsafe response. A conversational system may miss context or respond inconsistently, so do not rely on an AI companion as emergency care. If you or someone else is in immediate danger, contact local emergency services or a qualified human support service.
Age suitability and safeguards
Confirm the stated age group, how the service checks age, what protections apply to younger users and whether parental controls are available where appropriate. An app intended for adults should not be assumed suitable for a child or teenager just because it is easy to access.

In an eSafety Commissioner announcement in 2026, 8% of surveyed Australian children aged 10–17 said they had used an AI companion; the regulator estimated this represented around 200,000 children in Australia. Separately, 79% said they had used either an AI companion or an AI assistant. Those are Australian survey findings, not global usage estimates, and the combined figure should not be mistaken for companion use alone.
Transparency and ways to step back
Check whether the app clearly identifies itself as AI, explains relevant limitations, and makes it easy to adjust personalization, report problems or disengage. Consider whether reminders, memory or simulated intimacy suit your preferences. A product’s design can affect how the interaction feels; do not assume that a human-like tone means the system understands you as a person would.

What regulators have reported—and what that does not prove
Regulatory findings can identify issues worth checking, but a report about selected services is not a ranking of all companion apps or a guarantee about any service’s current state.
Australia: selected companion services
The Australian eSafety Commissioner reported on Character.AI, Nomi, Chai and Chub AI after issuing transparency notices. The report described inadequate age checks, weaknesses in self-harm support and content monitoring, and staffing or red-teaming gaps at some of the examined providers. eSafety also reported changes since its October 2025 notices: Character AI introduced age-assurance measures for Australian users in early 2026 and removed chat from its under-18 experience; Chub AI withdrew from Australia; Chai restricted free access to companion chats behind a subscription; and Nomi committed to further age-assurance functionality.
These are regulator-reported findings and changes at a particular time. They do not establish that every app has the same shortcomings, or that a reported change is a current guarantee. Check the service’s latest age rules, safety information and availability in your country.

United States: an FTC information-gathering inquiry
In September 2025, the US Federal Trade Commission announced information orders to seven consumer-facing chatbot companies. It sought details on monetization, conversation processing, safety evaluation and mitigation, disclosures, age rules, and use or sharing of personal information. The FTC described the work as a broad study under its 6(b) authority with no specific law-enforcement purpose. The inquiry itself is not a finding that a company broke the law.
A practical way to compare apps
Compare only services you have checked for your location and intended age group. Use the same questions for each so that a polished product description does not obscure missing information.
- Write down the intended use. Separate general conversation or companionship from wellbeing coaching and claims to address a health condition.
- Record the evidence. Note the evaluated users, measured outcomes, duration, independent or clinical input, and limitations.
- Review data terms and settings. Check collection, storage, model-improvement use, sharing, retention, deletion and memory controls.
- Inspect safety and age information. Look for crisis signposting, reporting routes, stated age limits and age-assurance measures.
- Verify current details. Confirm the app’s current version, policies and availability in your jurisdiction before creating an account or disclosing sensitive information.
If the provider does not clearly explain a point that matters to you—particularly a health claim, crisis response or use of private conversations—treat that as an unanswered question, not proof that the app handles it well.
When human support is the better choice
An AI companion may be one way to have a conversation, but it is not a substitute for professional care. Seek support from a qualified professional for mental-health difficulties, symptoms that are worsening or persistent, or questions about treatment. For urgent danger, use local emergency or crisis services rather than waiting for an app to recognize the situation.
