Neither adult diapers (absorbent briefs) nor incontinence pads are best for everyone overnight. A well-fitting high-absorbency brief is often the more appropriate starting point for moderate-to-heavy leakage, limited mobility, or urinary and fecal incontinence together. An overnight pad held securely by close-fitting underwear may work for some ambulatory users who prefer pads. The right choice depends on leakage, mobility, fit, skin health, and the wearer’s preference.
How to choose between an overnight brief and a pad
Specialist guidance makes a conditional distinction rather than naming one universal winner. The Wound, Ostomy and Continence Nurses Society (WOCN) recommends nighttime disposable briefs for nonambulatory women with moderate-to-heavy urinary, fecal, or dual incontinence, and for men with moderate-to-heavy nighttime urinary, fecal, or dual incontinence. For ambulatory women in its moderate-to-heavy nighttime group, it considers a disposable pad with close-fitting underwear an acceptable alternative. These are consensus recommendations, not proof from a head-to-head trial that briefs work better for every person.
Use the guidance as a starting point, then judge whether the product contains the wearer’s leakage comfortably and reliably. A product’s absorbency label or manufacturer description is not an independent comparison of real-world overnight performance.
What matters most for overnight use
Mobility and changing
A brief may be more practical when the wearer has limited mobility or needs caregiver assistance. Tabbed briefs can be applied and removed without pulling clothing up and down. Pads can suit an ambulatory wearer who can position one and keep it close to the body with snug-fitting underwear.

Leakage type and amount
Consider whether leakage is urinary, fecal, or both, and how much is likely overnight. The WOCN recommendations include briefs for several moderate-to-heavy nighttime groups, including people with fecal or combined incontinence. A pad is not automatically limited to light leakage, but it needs to remain in place and contain the wearer’s output.
Fit and containment
For either product, check that the absorbent area sits where it is needed and that the product is comfortable. With briefs, consider sizing and leg gathers: they should help contain leakage without feeling constrictive. With pads, check coverage and placement, and make sure the underwear holds the pad securely. A shifted pad or loose underwear can leave gaps and contribute to leaks.

Skin health, sleep, and changing
WOCN says change timing should be patient-centered, taking skin health, odor, sleep, elimination patterns, dignity, and product properties into account—not just a routine or caregiver convenience. That guidance does not set a fixed wear duration or a schedule for waking every wearer. Use gentle cleansing and suitable skin protection; some ointments can impair product absorbency, so more cream is not always better.
When to consider a booster or bed pad
Booster pads for high-volume output
For high-volume urine output, WOCN advises considering a booster pad as an adjunct to an absorbent brief or pull-up. It may help with containment and sleep interruption, but it is an extra layer—not a replacement for the primary worn product.

Bed pads for bedding protection
A washable bed pad sits on the mattress and can catch leaks as additional protection. It does not replace a worn brief or pad. The NHS describes absorbent pads worn inside underwear as a common incontinence product and provides UK-specific information about products and services.

- Start with the wearer’s needs. Note mobility, whether assistance is needed, the type and likely amount of leakage, and personal preference.
- Choose a suitable worn product. Consider a high-absorbency brief for limited mobility, caregiver-assisted changes, or moderate-to-heavy urinary, fecal, or dual leakage. For an ambulatory woman who prefers a pad, WOCN identifies a pad with close-fitting underwear as an acceptable alternative for its specified moderate-to-heavy nighttime group.
- Check fit and containment. Make sure the absorbent area is positioned correctly, the product stays in place, and it does not fit uncomfortably. Reassess if leaks occur.
- Add bedding protection only if useful. A washable bed pad can protect the mattress; for high-volume output, a booster may be considered with a brief or pull-up.
- Seek help if the problem persists. Ongoing leaks or skin irritation are reasons to reassess product type and fit and ask a healthcare professional or continence service for advice.
In the UK, access to products supplied through NHS services may depend on the local integrated care board and professional assessment. Check with the relevant local service rather than assuming eligibility is the same everywhere.
