Japan is not trying to automate eldercare with humanoid robots alone. Its government has spent more than a decade encouraging a broader mix of care technologies—including transfer and mobility aids, monitoring systems, ICT and task-specific robots—to support older people and care workers. The policy commitment is clear; the available figures show public support and participation, not proof that the technologies have reduced workloads or improved care outcomes nationwide.

What Japan means by care technology

In Japan’s policy framework, technology for long-term care is a wide category. It can include physical aids that help with movement or transfers, sensors and communication systems, and tools intended to support particular care tasks. That is a different proposition from replacing a caregiver with a humanoid robot.

The practical question is what job a device is supposed to do and how its use fits into care. A monitoring tool, for example, is intended to help staff observe or communicate about a person; a transfer aid addresses a physical task. Neither category, by itself, tells you whether a device suits a particular person, reduces work overall or improves care.

How the policy framework developed

The Ministry of Economy, Trade and Industry (METI) and the Ministry of Health, Labour and Welfare (MHLW) formulated the original “Priority Fields in the Use of Robot Technology for Long-term Care” in 2012. They revised it in 2014 and 2017. On June 28, 2024, the ministries broadened the name to “Priority Fields in the Use of Technologies for Long-term Care,” reflecting the role of ICT and other tools alongside robots. The revised framework began operating in April 2025.

It identifies 16 items across nine areas. The 2024 revision added three areas:

  • Support for functional exercise.
  • Assistance with eating and nutrition management.
  • Daily-life and long-term-care support for people with dementia.

The ministries also reviewed definitions in existing areas, including transfer assistance, toileting estimation and detection, monitoring and communication in facilities and homes, bathing assistance, and support for care work. The framework sets priorities for development and adoption; it does not require every care facility to install every kind of technology.

Why Japan is investing in automation

A Government of Japan age-tech task-force analysis describes care providers facing persistent staffing shortages alongside a declining birth rate and an aging population. It also points to an investment constraint: care facilities rely substantially on care fees, much of which goes to labor, while the industry’s overall profit level is low compared with other industries. The same analysis says differences in care systems and cultural contexts make overseas expansion difficult.

The task force reported that long-term-care insurance benefit costs reached ¥11 trillion in fiscal year 2022. That figure indicates the scale of public long-term-care spending; it is not a measure of spending on robots or of technology’s effectiveness.

Its chart reports the following public support for introducing nursing-care robots and ICT. National-treasury distributions and estimated total distributions are separate measures, in billions of yen:

What adoption figures do—and do not—show

MHLW reports that 5,371 establishments received subsidies through an ICT subsidy program in FY2021, compared with 2,560 in FY2020 and 195 in FY2019. These are counts of establishments receiving support under that program. They are not national adoption rates for robots, do not say how extensively recipients used the technology, and do not measure effects on care.

Keep those counts separate from the age-tech task force’s subsidy-distribution figures: one series counts establishments in an ICT program; the other reports amounts of public support for introducing nursing-care robots and ICT. Neither is an outcome study.

What is known about whether the tools help

MHLW describes an ongoing approach to evaluation: conducting demonstrations in care facilities, analyzing data and collecting evidence to inform policy. The ministry says proposed initiatives are assessed for expected effects, including whether care quality can be maintained while staff burden is reduced. This describes how the government is evaluating technologies, not a finding that the intended effects have already been achieved.

The official materials cited here do not give a single national causal estimate for how much care technology reduces staff workload or improves care quality. Subsidies, priority status and demonstrations should not be treated as substitutes for that evidence. Results can depend on the task, the person receiving care, the setting, staff training and how a device changes the rest of the workflow.

A qualitative account illustrates why implementation matters. In a March 7, 2024 review, Andrew Hundt discusses James Adrian Wright’s ethnography Robots Won’t Save Japan. It describes reported cases in which robots introduced to help staff became counterproductive, demanded care themselves, increased workloads or were returned. The review also discusses PARO, a plush robot seal, and differing interpretations of its role. These accounts raise questions about fit and assumptions; they do not show that most care robots fail, nor establish therapeutic effects for PARO.

How to judge a care technology in practice

For a family, provider or policymaker considering a device, the useful comparison is not simply “robot versus no robot.” It is whether the tool addresses a real need without adding more work or compromising the person’s care.

  • Task: Is it intended for mobility, transfer, monitoring, toileting, bathing, exercise, eating or nutrition, dementia support, or another defined activity?
  • Setting: Is it designed for a person’s home, a care facility, or both? A facility system should not be assumed suitable as a consumer product.
  • Workflow: What must staff or family caregivers do to set it up, monitor it, clean it, maintain it or respond to alerts?
  • Person and goal: Does it fit the individual’s needs and abilities, and does it support a specific safety, independence or quality-of-life goal?
  • Evidence: What outcome was actually measured, in which setting, and was the evidence from a demonstration or routine use?
  • Practical fit: Is training available, is the device compatible with existing routines, and are procurement or reimbursement arrangements relevant?

The government sources cited here do not provide comparable device-level prices or independent head-to-head trials. MHLW’s care-technology pages link to material from different fiscal years, so historical subsidy counts should not be used as a guide to current eligibility or funding terms.

What Japan’s experiment says so far

Japan’s long-running program shows a sustained policy effort to bring technology into care work and a widening view of what counts as relevant technology. It also makes the distinction between ambition and demonstrated results essential: public support can encourage trials and adoption, but whether a tool helps depends on its real-world fit and on evidence about the care task it is meant to improve.
