Sometimes—but not universally. In one prospective study of adults at elevated risk, AI-assisted digital stethoscope screening detected more cases of moderate-or-worse valve disease than routine listening, while also producing more false-positive results. A large UK primary-care trial, however, found no significant increase in heart-failure diagnoses after a year of routine use. Results depend on the condition, device, patients, and clinical setting; an AI stethoscope is best understood as a screening and decision-support aid, not a replacement for diagnosis.

What does “better detection” mean?

Heart disease is not a single target. An algorithm may be evaluated for reduced left-ventricular ejection fraction (a form of heart failure), atrial fibrillation, or particular valve conditions. Results for one condition do not establish performance for another.

Diagnostic studies often report sensitivity—the share of people with a condition who test positive—and specificity—the share without it who test negative. Higher sensitivity can mean fewer missed cases, but lower specificity can mean more people without the condition are flagged for additional assessment. Neither measure alone tells you whether using a device leads to more diagnoses or better health outcomes in everyday care.

In a US study, AI-assisted screening found more valve disease

A prospective study published in 2026 enrolled 357 people aged 50 or older who were at risk for heart disease at three primary-care clinics. Researchers compared clinicians’ routine analogue auscultation with digital auscultation followed by AI analysis. AI-supported screening identified 12 previously undiagnosed cases of moderate-or-worse valvular heart disease, compared with 6 identified through routine auscultation. Study report

For this study’s target condition and population, sensitivity was 92.3% with AI-assisted digital auscultation versus 46.2% with standard auscultation. Specificity moved in the opposite direction: 86.9% versus 95.6%. That tradeoff suggests the AI-supported approach may flag more cases for evaluation while also prompting follow-up for more people who do not have moderate-or-worse valve disease. Routine-care results were less conclusive

The UK TRICORDER cluster-randomised implementation trial tested an AI stethoscope across 205 general practices. Intervention practices recorded 12,725 examinations across 972 clinical users. During examinations, the device captured 15 seconds of single-lead ECG and phonocardiogram signals; algorithms targeted reduced-ejection-fraction heart failure, atrial fibrillation, and valvular heart disease. The primary outcome was newly coded heart-failure diagnosis. After 12 months, the intention-to-treat analysis found no significant difference in heart-failure detection between intervention and control practices: incidence rate ratio 0.94 (95% CI 0.86–1.02). There was also no significant difference in community- or hospital-based diagnoses. The trial authors concluded: “Implementation of an AI stethoscope in routine primary care did not significantly increase detection of heart failure or increase community-based diagnosis after 12 months of implementation.” Trial abstract

This does not negate the valve-detection findings: the studies asked different questions, in different settings, about different outcomes. It does show why promising performance in a diagnostic study cannot by itself establish that a device will increase diagnoses when introduced into routine practice.

Validation figures depend on the condition and test setting

A 2025 European Heart Journal abstract reported a multicentre prospective validation study in secondary care. For reduced-ejection-fraction heart failure, the AI stethoscope had an area under the curve (AUC) of 0.87 (95% CI 0.84–0.90), with sensitivity of 83% and specificity of 76% at the pulmonary auscultation position. For atrial fibrillation, sensitivity was 84% and specificity was 93%. The abstract also reported results for aortic stenosis and other valve disease. Validation study abstract

An AUC describes how well a test separates people with and without a condition across possible thresholds; it is not the percentage of patients correctly diagnosed at one chosen threshold. These validation estimates are specific to the study’s device, algorithms, population, and testing conditions. They do not prove improved patient outcomes.

What the evidence does—and does not—support

  • Supported: AI analysis of digital stethoscope recordings can improve sensitivity for some specific targets in defined study settings, including moderate-or-worse valvular heart disease in the 2026 US study.
  • Also supported: Higher sensitivity can come with lower specificity, so a positive screen may require follow-up testing.
  • Not established: That every AI stethoscope improves detection of every heart condition, or that results from one device or algorithm apply to another.
  • Not established: That AI stethoscopes reduce mortality, improve long-term outcomes, or replace clinical judgment and confirmatory tests.

Device features and regulatory status are specific

The studies used physical digital or electronic stethoscopes to record signals for algorithmic analysis. The evidence does not identify one common model shared across the studies or establish that every consumer digital stethoscope includes the algorithms tested. A digital recording feature alone does not mean a device has the studied AI capabilities.

The FDA database records a 510(k) decision of “Substantially Equivalent” for eMurmur Heart AI, listed as an electronic stethoscope, dated December 19, 2025. That regulatory record applies to that device and its intended use; it does not confer the same status on other products or demonstrate superior patient outcomes. FDA device record

How to interpret an AI-stethoscope result

  • Check which condition the algorithm is intended to screen for; a result for atrial fibrillation does not answer whether someone has valve disease or heart failure.
  • Ask what follow-up is appropriate. A screening result is not, by itself, a confirmed diagnosis.
  • Consider the setting and population in which the device was evaluated, and whether its evidence came from a validation study or a trial of routine clinical implementation.
  • For a specific product, verify its intended use and regulatory record rather than assuming that all digital stethoscopes offer the same AI functions.
