AI-generated code can fail in production for the same reasons other code does: it can implement the wrong behavior, mishandle inputs or resources, cross security boundaries unsafely, or pass through review without its assumptions being tested. Studies find varied defects in evaluated code samples, but the evidence cited here does not establish a representative rate of production incidents caused by AI-generated code.

What makes generated code fail after deployment?

A code suggestion is produced from a prompt and its surrounding context; it is not proof that the implementation matches the system’s requirements or operating conditions. A defect can become a production problem when an untested assumption about inputs, interfaces, permissions, resource limits, or deployment behavior turns out to be false. That is an engineering explanation of how failures can escape, not a causal finding measured by the studies below.

It can be functionally wrong while looking plausible

Code may compile and still return the wrong result, mishandle an edge case, or violate an API contract. A reviewer who checks only whether the code looks familiar—or whether one happy-path example works—may not catch a mismatch with the actual requirement.

It can omit defensive checks

Nogueira, Vieira, and Campos analyzed 86,726 generated code samples that already had compilation or runtime errors. The samples came from seven large language models and four compiled languages. Their 2026 study found that error patterns varied substantially by language and model, and noted that generated code could omit basic input validation or memory-safety checks. Such omissions can contribute to overflow, resource exhaustion, or other reliability and security problems.

That dataset is selected for samples with errors. It describes patterns among those samples; it does not tell you what share of all generated code fails.

It can mishandle security-sensitive operations

In a 2025 comparison of more than 500,000 human- and AI-authored Python and Java samples, Cotroneo, Improta, and Liguori reported more high-risk security vulnerabilities in the AI-generated code in their evaluated dataset. They also found differences in code structure: generated samples were generally simpler and more repetitive, while human-authored samples showed a greater concentration of maintainability issues. The result is specific to the study’s languages, samples, and evaluation; it is not a prediction that AI-generated code will always be less secure or maintainable in a deployed system.

It can inherit a weak review or release process

Generated code can be missed or misunderstood just like code written by a person. A 2026 observational study by Khalid and co-authors examined 100 participants working on four C linked-list tasks, with five generated suggestions per task, and interviewed 23 participants. Those figures describe the study design; they do not establish a general rate at which developers catch or miss defects.

What the evidence can—and cannot—tell you

The findings show that defects and vulnerabilities occur in evaluated generated-code samples, and that results vary by model, language, task, and measurement method. They do not establish a representative production-failure rate, the most common cause of real-world incidents across industries, or how much a particular checklist reduces incidents. Benchmark findings, sample counts, and participant-study designs should not be treated as production incident statistics.

NIST’s AI security overview places these concerns in the broader software-security context: some cybersecurity risks related to AI systems are common or identical to risks across software development and deployment. The practical implication is not that AI code needs a magical new standard of proof; it is that the ordinary security and reliability controls must apply to the generated change as well.

How to review AI-generated code before deployment

Use the same delivery lifecycle as for other code, with attention proportionate to the change’s impact and security exposure. NIST’s DevSecOps reference model says AI-generated outputs should go through established processes, including peer review, security validation, automated testing, and approval workflows.

  1. Confirm the requirement and boundaries. Compare the change with the ticket, interface contract, and surrounding code. Identify what inputs it accepts, what it is allowed to access or modify, and what should happen on invalid or unexpected input.
  2. Inspect the implementation, not just the explanation. Trace the code paths that handle errors, empty or extreme values, permissions, secrets, external commands, and resource use. Check that validation and memory-safety assumptions are appropriate for the language and system.
  3. Run tests for both expected behavior and failure conditions. Use the project’s automated test suite and add cases for relevant boundaries, malformed inputs, error handling, and authorization. Tests suggested by an AI tool can be useful, but independently assess whether they actually cover the requirement and failure modes.
  4. Perform security validation. Review data handling and security-sensitive operations, and use the security checks required by your organization. A clean test run does not by itself establish that a change is safe.
  5. Require accountable review and approval. Have a reviewer assess the change in context before it is merged or released. Treat generated fixes, configuration edits, and operational actions as proposals rather than automatic authority.
  6. Release through the normal change controls. Preserve the team’s approval and deployment gates. Monitor the change according to its operational risk and use the established rollback or incident process if it behaves unexpectedly.

How to apply NIST guidance to AI-assisted development

NIST SP 800-218A supplements the Secure Software Development Framework (SSDF) version 1.1 with practices, tasks, recommendations, and considerations specific to AI model development across the software development life cycle. It is intended for model producers, AI-system producers, and acquirers. It supplements the existing SSDF; it does not replace secure software development practices for the surrounding product or workflow.

NIST’s DevSecOps reference model also says AI-generated corrective actions should be reviewed and approved before they change software or system state. That applies to proposals that could modify code, configuration, or operations: generation should not silently bypass the controls used for other changes.

What teams should take away

There is no single defect profile for AI-generated code, and the available studies do not justify a universal claim that it is either safe or unsafe to deploy. Evaluate the specific change in its language, task, and risk context; verify its behavior and security; and keep review, testing, and approval in the delivery path. Those controls are prudent workflow guidance, not a quantified guarantee that incidents will be eliminated.
