A scheduler’s run count does not prove that an agent completed its intended work. Verify the trigger, the job’s eligibility, its execution, the resulting state change, and any expected delivery as separate outcomes. The exact assertion behind the “40 times” incident is not established here, so it should not be attributed to the incident without the original code or notes.
What a scheduled run count does—and does not—tell you
A configured schedule shows intent. A run record may show that a trigger or execution event occurred. Neither alone proves that the intended data changed, an artifact was created, or a result reached its destination.
Debug the chain in order: confirm the platform delivered a trigger; confirm the job or callback was eligible; verify that the intended code ran; inspect the state it was meant to change; and check whether the result was delivered. This separates scheduling failures from condition, execution, and output failures.

Diagnose the failure one layer at a time

Confirm that the trigger actually arrived
Inspect scheduler or workflow history for trigger events rather than relying on the configured cron expression. For GitHub Actions, scheduled workflows use POSIX cron, run in UTC by default, and execute on the default branch. GitHub also warns that schedule events can be delayed under load and that some queued jobs may be dropped during periods of high load. Check whether the intended job or step was eligible
A trigger can arrive while a job or step is skipped by a condition. In GitHub Actions, inspect the job log’s condition-evaluation details for the expanded values and results that explain a skip; the troubleshooting guide describes this diagnostic.

Run the underlying script or callback directly
Test the command outside the scheduler to distinguish a broken script or command from a scheduling problem. Microsoft recommends this direct test in its Task Scheduler troubleshooting guide.

Read execution records and errors
For Windows Task Scheduler, review the task’s status, History, Last Run Result, and the Task Scheduler Operational event log. Microsoft’s guide includes the case where a task completes but its expected actions do not occur. That distinction matters: “completed” is not necessarily the same as “did the work.”

Assert the job’s meaningful postcondition
Make the job fail visibly when its actual goal is absent. Depending on the job contract, a useful postcondition might verify that a record was updated, an artifact was written, or a downstream acknowledgement arrived. Do not assert only that the process started or returned without throwing; those checks can pass while the intended outcome is missing. The Microsoft guide supports checking expected actions and execution records, but it does not identify the assertion from the title’s incident.

Separate side effects from output delivery
A scheduled agent can perform tool calls or writes while its prompt output is discarded. Vercel Eve documents this behavior for scheduled prompts, so a missing chat response alone does not establish that no work happened. Check the runtime’s logs and the target state, as well as its delivery behavior: Vercel Eve’s scheduled prompts documentation.
Scheduling behavior depends on the platform
Do not assume that one platform’s schedule lifecycle applies to another. Check the actual system’s timing, execution context, persistence, overlap rules, and output handling.

What would establish the assertion that fixed this incident?
The reported title does not identify the scheduler, agent framework, source code, or assertion, and the available platform documentation does not verify that the agent ran 40 times. The exact assertion should therefore not be guessed or presented as independently confirmed. To name it accurately, the original code or incident notes would need to show both the assertion and the condition it checked.
For a reproducible diagnosis, record the trigger event, the condition or skip decision, the execution logs, the asserted postcondition, and the resulting state or acknowledgement. That evidence lets you distinguish a missed trigger from a skipped job, ineffective work, or undelivered output.
