A form can appear to submit normally without creating a usable CRM lead. Find the break by tracing one controlled test from the browser through the integration and CRM, then checking approval, deduplication, and assignment. A submission count is not the same as a count of leads created.
What “lost” means in a form-to-CRM workflow
A website form submission passes through several stages: the browser sends the data, an integration delivers it, the CRM validates and processes it, and automation may approve, deduplicate, or assign the resulting record. A visible thank-you message only confirms what the page displays; it does not by itself establish that a lead was created or routed correctly.
There is no representative published figure in the reviewed official sources for how often website leads are silently lost, or for their monetary impact. Treat this as a traceable operational problem, not a basis for assuming a particular failure rate.
Trace a controlled test submission
1. Record a known test
Submit one test using values that should pass the form and CRM’s rules. Record the submission time, page, campaign or source, and the test values needed to search for the event. Salesforce recommends testing a Web-to-Lead form, and Google Ads recommends testing whether the CRM actually receives a lead.

2. Check the browser response
Confirm that the page displays an understandable completion or error state. If the form is embedded, check whether it behaves consistently in another browser and whether the embed code is current. For Zoho CRM webforms specifically, Zoho’s troubleshooting guidance also says to check that the submit control retains type="submit". These checks can reveal a browser or embed problem, but a success message is not proof of CRM record creation.
3. Look for a submission or integration event
Check the form platform and integration for evidence that the test arrived. Where available, compare total submissions with records added to the CRM, then inspect statuses such as duplicate, validation failure, approval pending, or double opt-in. Zoho CRM’s Webform Analytics distinguishes submissions from leads added and reports these kinds of outcomes.

4. Verify the delivery configuration
If no destination record appears, check whether the integration is enabled and whether it sent the expected fields. Confirm that mapped field names and types match, required CRM fields are supplied, and the integration supports the form’s encoding, request method, and submission behavior. These are documented troubleshooting areas for Salesforce Web-to-Lead and Account Engagement form handlers; the exact checks and fixes depend on the platform you use.
Google Ads lead forms can deliver through webhooks, APIs, Zapier, CSV downloads, or email. Choose the route configured for your account and verify receipt at its actual destination rather than assuming that a configured connection has delivered the test.

5. Inspect CRM rules and automation
Search for the test in the CRM, including rejected or held records where the system exposes them. Review validation rules, required fields, duplicate handling, and any dependencies between state and country fields. For Salesforce Web-to-Lead, these are among the documented troubleshooting points; another CRM may use different rule names or behavior.
If the CRM uses server-side plug-ins or connected processes, inspect their errors and logs. Microsoft’s guidance for Customer Insights – Journeys forms recommends checking plug-in trace logs and connected processes when submissions fail.

6. Check approval, opt-in, and assignment
A submission awaiting approval or double opt-in is not the same as a submission that never arrived. Check the relevant queues and status views before concluding that the record is missing. If the lead exists but is absent from the expected owner’s queue, investigate assignment rules and downstream routing instead of the initial form delivery.
7. Repeat the test after a fix
Resubmit using a fresh test identifier and compare the same checkpoints: browser response, submission or integration event, CRM record and status, and final assignment. Before replaying an old failed submission, check duplicate risk. Microsoft warns that resubmitting a failed Customer Insights – Journeys form record can create a duplicate contact.

What platform diagnostics can—and cannot—tell you
When choosing or reviewing monitoring, check whether it separates page visits, raw submissions, validated submissions, and records created; exposes duplicates, rejections, approval, and double-opt-in states; can test delivery into the CRM; retains failed submission details or replayable records; and confirms assignment rather than stopping at record creation.
Those capabilities vary by product. Zoho documents submission and lead-status analytics. Google Ads documents CRM delivery tests and says its lead form data is stored for 60 days; that retention period applies to Google Ads lead form data, not CRM records generally. Salesforce Account Engagement says failed form-handler submissions are not retained in its logs, so those logs cannot serve as a complete record of every failed attempt. Microsoft documents submission records, plug-in traces, and resubmission considerations for Customer Insights – Journeys.
Google also cautions that sophisticated bots can bypass browser-side invalid-traffic controls. Its guidance on preventing invalid leads recommends server-side validation of submitted email and phone data before CRM ingestion. Browser checks may help identify problems, but they do not establish that a lead is genuine.
