For a useful post-support survey, ask about one recent interaction, choose one main goal, and keep the form short. Start with a satisfaction rating, then add a relevant optional question—such as whether the issue was resolved or what made getting help difficult. The examples below help you measure the experience without confusing support quality with broader customer loyalty.

Choose the question that matches what you need to learn

Customer service surveys work best when each question measures something the customer could observe in a particular support interaction. Decide what action the answer should inform before choosing a metric. A satisfaction score can flag a poor experience; a resolution question can identify cases that still need help; an effort question can reveal process friction.

Measure What it asks Useful for What it cannot tell you by itself
CSAT How satisfied was the customer with this interaction? A post-ticket, post-call, or post-chat satisfaction pulse Why the customer felt that way, or whether respondents represent all customers
CES How easy or difficult was it to get help or resolve the issue? Finding friction in support processes Overall loyalty or the full quality of the interaction
NPS How likely is the customer to recommend the company, product, or service? Tracking broader relationship or loyalty intent Whether this particular contact resolved the problem

Use one primary measure in a short survey. Add another only if its result will lead to a distinct decision; asking CSAT, CES, and NPS together by default can make a post-contact survey longer without clarifying what to fix.

Copy-ready customer service survey questions

Replace bracketed wording with a detail the customer will recognize, such as a recent case or interaction. Use only questions that fit the support experience and your follow-up process.

#1 Best Overall
Salon Service Ticket Receipt Book
  • PREPRINTED: These receipts are pre-printed with common salon services but also include a blank line for added flexibility. There is a space for operator's name as well as the amount due per service and a total amount due.
  • PERFORATED: The pad is perforated on top allowing one for your records and other for check out at the register.
  • BULK PACK: Each pad includes 100 sheets that are numbered in sequence for easy reference.
  • CONVENIENT SIZE: The pad measures 5.25" tall and 3" wide making it easy to use and store.

Interaction satisfaction (CSAT)

  • “Overall, how satisfied were you with the support you received?”
  • “Based on your recent interaction regarding [incident], how satisfied or dissatisfied were you with our service team member?”
  • “How would you rate your overall satisfaction with the service you received today?”

SurveyMonkey illustrates a five-point scale labeled “very unsatisfied,” “unsatisfied,” “neutral,” “satisfied,” and “very satisfied.” Keep response choices ordered and clearly labeled. If using this scale, a common CSAT calculation is: (number of valid responses choosing 4 or 5 ÷ total valid responses) × 100. Report the scale and denominator alongside the percentage so the result has context.

Resolution and effectiveness

  • “Did we resolve your issue today?” (Yes / Partly / No)
  • “Did you get the help you needed?”
  • “Was the solution clear and complete?”
  • “Do you still need help with this issue?”

These questions distinguish a pleasant interaction from one that actually met the customer’s need. Decide in advance what happens after a “Partly” or “No” response—for example, routing the case for follow-up—so feedback can trigger service recovery rather than remain only a reporting metric.

Effort (CES)

  • “How easy was it to get the help you needed today?”
  • “How easy or difficult was it to resolve your issue with us?”
  • “What made getting help easier or harder?”

Use a consistent, anchored scale and make its direction unmistakable. If a high score means “easy” in one report but “difficult” in another, comparisons become confusing. There is no single universally established CES wording or scale in the cited guidance, so document the wording and response labels you adopt.

Agent interaction quality

  • “How courteous or uncourteous would you say our team member was?”
  • “How knowledgeable or unknowledgeable would you say our service team member was?”
  • “Did you feel confident in our agent’s ability to help you?”

Choose one or two traits linked to a coaching goal instead of rating every aspect of an agent in every survey. SurveyMonkey provides examples about courtesy, knowledge, and confidence. Attribute an answer to an individual agent only when the interaction sample and operating context support a fair interpretation.

Optional open-ended follow-up

  • “What was the main reason for your rating?”
  • “What could we have done better?”
  • “What can our employees do to improve?”
  • “Is there anything else you’d like us to know about this interaction?”

Open text can explain a rating, but it asks more of the respondent and is harder to summarize than a fixed-choice answer. Make it optional when the main objective is a quick rating, and use a focused prompt rather than asking several open-ended questions at once.

Broader relationship (NPS)

  • “How likely are you to recommend [company/product/service] to a friend or colleague?”
  • “Based on your interaction with our support team, how likely are you to recommend our company to a friend or colleague?”
  • Follow-up: “Why or why not?”

The conventional NPS response bands are 9–10 for promoters, 7–8 for passives, and 0–6 for detractors. NPS is calculated as the percentage of promoters minus the percentage of detractors. It is a measure of recommendation intent, not a replacement for asking whether the support issue was resolved.

Build a short survey customers can answer

  1. Choose one decision. Decide whether the survey will measure interaction satisfaction, effort, agent coaching, unresolved issues, or broader loyalty. If a result has no planned use, leave its question out.
  2. Anchor it to a recent event. Refer to a recognizable case, call, chat, or other contact where possible. Ask only about things the respondent could observe.
  3. Ask one thing at a time. Avoid double-barreled wording such as “How satisfied were you with the speed and friendliness of our agent?” One answer cannot show which part influenced the rating.
  4. Use neutral wording and balanced options. Do not imply the answer you want with a prompt such as “How excellent was our agent?” Label response choices clearly and keep their order consistent.
  5. Keep the form focused. SurveyMonkey’s customer-service guidance recommends one quantitative question with an optional open-ended follow-up; its CSAT guidance describes two or three questions as a compact target. NiCE recommends making a survey finishable in under five minutes. These are vendor recommendations, not proof of one universally optimal survey length.
  6. Choose a suitable invitation moment and channel. Feedback is generally more useful while the interaction is still recallable, but the right delay depends on the service context and channel. The cited guidance does not establish one best send interval for every business.
  7. Plan what happens after an answer. Route unresolved cases appropriately, review recurring themes, prioritize service changes, and tell customers what changed when that is suitable.

Interpret scores without overstating them

Keep the calculation consistent

For the five-point CSAT example, divide the count of valid responses selecting the top two positive choices by the total number of valid responses, then multiply by 100. Preserve the same question, labels, respondent population, and calculation when comparing periods. Include response counts alongside percentages where practical; the cited guidance does not specify a universal minimum sample size.

Treat voluntary responses as a partial view

People who complete a survey may differ from those who ignore it. NiCE notes that especially satisfied or dissatisfied customers may be more likely to respond. Track invitation and response context when practical, and avoid treating a voluntary survey as a perfect census of every customer’s experience.

Compare like with like

Review trends by comparable channel and interaction type, and consider sample sizes and operational context. Do not treat one response as a standalone verdict on an agent. Survey results are more useful when combined with relevant case information and used for coaching or service improvement.

SurveyMonkey’s September 23, 2026 CSAT guide reports 74.4% as an average US customer satisfaction score and points readers to the American Customer Satisfaction Index for quarterly industry benchmarks. The guide does not make the measurement period or comparability to support-interaction CSAT clear. That figure should not be treated as a target or contact-center benchmark; use a benchmark only when the question, scale, population, channel, and time period are meaningfully comparable.

Turn feedback into service improvements

  1. Identify recurring themes. Look for repeat explanations behind low ratings, unresolved cases, or effort scores rather than reacting to one comment in isolation.
  2. Assign an operational owner. Route issues to the team able to address them, such as support operations for a process obstacle or a coaching lead for a recurring interaction pattern.
  3. Make and monitor a change. Track the relevant survey measure and case context over time using the same question and scoring method.
  4. Close the loop where appropriate. Follow up on cases that still need help, and tell customers about improvements when it is useful and accurate to do so.

NiCE describes integrating customer feedback into quality management, coaching, and improvement. For implementation, survey tools and contact-center feedback systems are different categories: SurveyMonkey describes building, distributing, analyzing, and acting on survey feedback, while NiCE describes CXone feedback delivery through web, email, and voice channels. Choose an implementation based on the channels, workflow integration, reporting, scale, accessibility, language, data-handling needs, and cost your operation requires.

Should I ask customers for feedback after every support interaction?

That depends on your service context and invitation policy. The cited guidance does not establish a universal frequency; avoid assuming every channel or case should use the same cadence.

What is the difference between CSAT and NPS?

CSAT asks about satisfaction with an interaction, while NPS asks about likelihood to recommend a company, product, or service. Use the one aligned with the decision you need to make.

Is there a universal benchmark for support CSAT?

The cited guidance does not establish a directly comparable universal support-interaction benchmark. A benchmark is meaningful only when the question, scale, population, channel, and period align.
