There is no single accuracy score that proves an AI health app is safe. Judge it against the specific task it claims to perform, the people and settings it is meant for, and the consequences of a wrong answer. Look for independent validation, relevant error measures, clear limits, and monitoring after launch—and treat an app’s output as decision support, not a diagnosis or a reason to delay urgent care.
Start by defining what the app is supposed to do
A symptom checker, an image classifier, a wellness coach, and a clinical decision support tool do not perform the same job. Before comparing scores, identify the app’s claimed task, intended users, required inputs, care setting, and the action a user might take based on its recommendation. Check what the product explicitly says it does not do, too.
This matters because evidence is useful only when it fits the intended purpose. The World Health Organization’s framework for AI-based medical devices covers evidence generation across development and post-market surveillance; it is a useful way to understand why a model score alone is not a safety case. Check whether the evidence matches the intended use
Look for a description of how the app was evaluated, what it was compared against, and who and where were represented in the evaluation. A result from one population, clinic, language, or type of input may not establish performance for another. Independent or external validation—testing beyond the data used to develop the system—can help show whether results hold outside development conditions.

For each evaluation, look for:
- Population and setting: age groups, relevant health conditions, languages, and care environments included, and whether they resemble the app’s intended users.
- Reference standard and method: what counted as the correct answer, how cases were selected, and whether the sample and evaluation method are described.
- Usability: whether users can understand and follow instructions, and whether mistakes in entering information or using the app were considered.
- Limits and edge cases: what happens with incomplete, unusual, or out-of-scope inputs, and whether the product explains when it cannot give a dependable answer.
The FDA’s considerations for digitally derived measures say verification and validation should support fitness for purpose, account for potential sources of error and users’ ability to follow instructions, and consider meaningful clinical outcomes. These are useful questions for readers assessing evidence, not a consumer certification checklist. Read performance measures in context
Ask which measure fits the app’s task and what it reveals about harmful mistakes. For a classification task, sensitivity and specificity may be relevant: sensitivity concerns how often the system identifies cases that are present, while specificity concerns how often it correctly rules out cases that are absent. A high value for one does not automatically tell you how often a user’s result will be right in their situation. The balance between missed cases and false alarms—and the impact of each—matters.

For other functions, different measures may be appropriate. Look for results by relevant subgroup and site, explanations of uncertainty or calibration where meaningful, and details about failure modes rather than one headline percentage. There is no universal pass score established for AI health apps. A high score does not by itself show that an app is safe for a particular person, setting, or decision.

Compare apps using the same questions
Compare products only when they claim to perform the same task for similar users. A single accuracy percentage cannot rank unlike tools, and a percentage without its test population, method, and error details is difficult to interpret.

Check regulatory claims against the app’s exact function
Regulatory status depends on the software’s function and intended use; an AI label, app-store listing, or broad claim such as “FDA compliant” is not proof of FDA clearance or approval. In the United States, the FDA’s January 2026 Clinical Decision Support Software guidance explains its interpretation of criteria for certain non-device CDS software. The agency also says existing digital-health policies continue to apply to functions that meet the device definition, including patient- or caregiver-facing functions. That guidance is U.S.-specific. Rules differ across jurisdictions and depend on the software’s function. Performance can vary as an app is used at different sites, with different populations or inputs, or after system updates. Ask whether the provider monitors performance in deployment, evaluates meaningful changes, and has a process for responding when performance shifts. Use an app without handing it the final decision
- Check the scope: confirm that your question, symptoms, or input fit the app’s stated intended use.
- Review the inputs: make sure the information is accurate and complete, and note anything the app did not ask about that may matter.
- Read the explanation: look for uncertainty, limitations, and instructions for what to do if the result is unclear or symptoms worsen.
- Seek human judgment when needed: discuss consequential advice with a qualified clinician, especially if it conflicts with professional guidance or does not fit your situation.
- Do not wait on the app for urgent help: seek urgent or emergency care for serious symptoms rather than testing an app or waiting for its response.
