When to seek medical advice
Contact a qualified health professional if symptoms are significant, worsening, unusual or not improving. MedlinePlus advises contacting a provider for sudden numbness or tingling, a sudden increase in pain or swelling, or healing that is not proceeding as expected. A general guide cannot determine whether you have a mild strain or a more serious injury. The appropriate assessment, including whether further investigation is needed, depends on your symptoms and clinical examination.
What rest means—and what it does not
In the early phase, stop or modify activities that reproduce substantial pain rather than pushing through them. Avoid repeatedly stretching, sprinting or otherwise testing the sore area. This is activity modification, not a rule that complete rest by itself will heal every hamstring injury. The 2022 clinical practice guideline from the Academy of Orthopaedic Physical Therapy and American Academy of Sports Physical Therapy addresses sports-related hamstring overloading and overstretching injuries: Hamstring Strain Injury in Athletes (CPG+).

Do not use a calendar estimate to decide when to resume normal activity. MedlinePlus cautions that returning too early can cause reinjury. If you are unsure how much activity is appropriate, get advice rather than repeatedly provoking symptoms.
How recovery and rehabilitation progress
Recovery varies with the injury and the activity you want to return to. The 2016 Bern return-to-sport consensus notes substantial individual variability and says predicting an individual athlete’s return time is inaccurate. There is no dependable universal number of days or weeks that clears everyone.

Early loading: protect, then reassess
The 2023 London International Consensus recommends tailoring rehabilitation to the person, sporting demands, involved muscle and injury type or severity. Its panel generally favored protecting injured tissue from high-strain and high-strain-rate loads early, then progressing according to symptoms, capacity and response to previous loading. Later rehabilitation: build toward the task
Later-stage rehabilitation commonly includes progressive strengthening, including work at longer muscle lengths, and running or sprinting that reflects the goal activity. The optimal exercise dose and progression are not fully settled, so these are not a universal home program or a reason to start fast running while it remains painful. Exercises—and optional equipment such as resistance bands—should suit the individual plan.

The London panel’s reported agreement percentages describe agreement with statements, not treatment success: 78.8%–96.3% for rehabilitation statements and 83%–100% for running and sprinting statements. Its proposed flexibility benchmark (40%) and strength benchmark (66.1%) did not reach the panel’s consensus threshold. These figures do not establish a single test or threshold that can clear an individual to return.
Returning to running, work and sport
Think of return as a progression, not a single date. The Bern consensus describes a continuum from return to participation, to return to preinjury sport, and then to return to performance. The level of activity matters: resuming modified participation is not the same as being ready for full-intensity competition.

Use multiple readiness factors
For football, the HIPS-Delphi Group defined return to play as “the moment a player has received criteria-based medical clearance and is mentally ready for full availability for match selection and/or full training.” Its 2017 consensus identifies medical staff clearance, no pain on palpation or during strength, flexibility and functional testing, similar hamstring flexibility, field performance and psychological readiness among relevant factors. It supports shared decision-making involving the athlete and relevant medical, rehabilitation, fitness and coaching staff: HIPS-Delphi Group: football return-to-play consensus.
No single measure guarantees readiness. The Bern consensus reports no strong evidence that MRI predicts an individual’s time to return; clinical findings are more informative, but still cannot predict an individual’s return date with certainty. Decisions should reflect symptoms and performance in the activities you need to resume, guided by an appropriate clinician.

Why a gradual return matters
A 2018 review in Sports Health reported that nearly one third of hamstring strains recur within the first year after return to sport. That is a published review estimate, not a prediction of your personal risk. It is a reason to take progressive rehabilitation and readiness decisions seriously rather than treating the first pain-free day as automatic clearance.
