Check the skin regularly, and involve a team athletic trainer or sports-medicine professional when a blister is painful, open, looks infected, or interferes with grip or release. Any in-game covering or product must also comply with the pitcher’s league rules.
Why pitching can cause fingertip blisters
During a pitch, fingertips press and slide against the baseball’s seams. Repeated friction can injure the skin, particularly on the index and middle or long fingers. A 2016 clinical review identifies the index and long fingers as common sites; a 2025 article by professional baseball athletic trainers describes the middle finger as the most common site in their account, followed by the index finger and, less often, the thumb. Taken together, the evidence points to the fingertips as vulnerable without establishing one universally most-affected digit.
The 2016 review recorded 7 blister-related disabled-list stints among 6 MLB players since 2010, totaling 151 days, and 8 minor-league players placed on the disabled list for blisters since 2012. These historical counts are not current incidence rates and do not estimate an individual pitcher’s risk. (American Journal of Orthopedics, 2016)
How pitchers can maintain nails and calluses
Keep nails smooth
Check fingernails regularly, keep them at a normal length, and file sharp or jagged edges so they are smooth and rounded. The 2025 athletic-trainer article recommends light filing as needed. A fine-grit nail file or emery board is useful for smoothing, but aggressive filing can irritate skin or make a rough area worse. (International Journal of Athletic Therapy and Training, 2025)

Keep calluses small and smooth
Check calluses often. The aim is a small, smooth area without loose edges: a prominent callus can increase friction where it contacts a seam. The clinical review discusses carefully trimming loose borders with clean clippers and cautiously contouring the surface with an emery board. Avoid cutting into attached, tender, or bleeding skin; if a callus is painful or difficult to manage, ask a trainer or clinician for help. (American Journal of Orthopedics, 2016)
Managing damp hands and checking game rules
Damp or clammy skin may increase friction against the seams. A 2025 article by professional baseball athletic trainers discusses approved antiperspirants and drying agents as ways to reduce moisture buildup. That does not mean a particular substance is allowed in every league, or in every game situation; check the rules that apply before using a product on the mound. For MLB, the league’s public rules explainer says a pitcher may not pitch with an attachment such as a bandage, tape, or bracelet on either hand or wrist. It also says that after touching the mouth or lips, the pitcher must wipe the pitching hand dry before touching the ball or pitching rubber. Those MLB requirements should not be assumed to govern youth, amateur, college, international, or other professional competitions; check the current rules for the specific league. What to do when a blister forms
A new blister deserves attention if it hurts or changes how the ball feels in the hand. Seek a team athletic trainer or sports-medicine professional if it is open, looks infected, is painful, or interferes with grip or throwing. Do not treat professional descriptions of sterile drainage or skin covering as a do-it-yourself procedure: the cited articles do not establish a universal at-home method, and they are not a basis for puncturing a blister, applying surgical glue, or pitching through a hand injury. Keep hand care separate from arm workload guidance
Pitch limits and rest recommendations address throwing workload and fatigue-related overuse, not fingertip friction directly. MLB’s Pitch Smart program provides age-specific guidance for amateur players, while the American Orthopaedic Society for Sports Medicine advises youth players not to throw with pain and emphasizes warm-ups, workload monitoring, and rest. These are valuable arm-health recommendations, but they do not show that pitch limits prevent finger blisters. (American Orthopaedic Society for Sports Medicine; MLB pitcher-injury report, 2024)
